INSTRUCTIONS
GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska.
ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation,
injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any
attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the
producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data
pertinent to such interval.
ITEM 21: Indicate whether from ground level (GL) or other elevation (OF, KB, etc.).
liEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool.

Dear Mr. Archey:
Enclosed is the approved application for permit to drill the above referenced service well.
This permit to drill does not exempt you from obtaining additional permits or approvals required
by law from other governmental agencies, and does not authorize conducting drilling operations
until all other required permits and approvals have been issued. In addition, the Commission
reserves the right to withdraw the permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter 25 of the
Alaska Administrative Code unless the Commission specifically authorizes a variance. Failure
to comply with an applicable provision of AS 31.05, Title 20, Chapter 25 of the Alaska
Administrative Code, or a Commission order, or the terms and conditions of this permit may
result in the revocation or suspension of the permit. Please provide at least twenty-four (24)
hours notice for a representative of the Commission to witness any required test. Contact the
Commission's North Slope petroleum field inspector at 659-3607 (pager).

Well Control:
Production hole will be drilled with a diverter.
· Maximum anticipated BHP: 1,963-psi @ 4,366-ft TVDrkb - Top of "DB"
· Maximum surface pressure: 1,527-psi @ surface ('"
(Based on BHP and a full column of gas from TD @
0.1-psi/ft)
· Planned BOP test pressure: Function test diverter
· Kick tolerance: NI A
· Planned completion fluid: 10.0-lb/gal brine I 7.0-lb/gal Diesel (2,000-ft MD)
Drilling Hazards & Contingencies:
Hydrates
· Hydrates have not yet been seen on S pad, but have been encountered in the surface
holes on other pads, especially nearby H-Pad. Hydrates generally occur just below the
permafrost (+1- 1 ,800-ft TVDss) but may be present in shallower SV sands and also as
deep as 3,000-ft. Be alert for the possibility of shallow gas and/or hydrates. Use
recommended practices for drilling hydrates.
· Watch out for deeper gravels below the permafrost or gravels that calve off within the
permafrost and tend to drop out when the aerated hydrate gas cut mud is being
circulated out. Maintain adequate viscosity. In the past, several Milne Pt wells have
experienced stuck pipe from this situation.
Lost Circulation
· Offset wells encountered losses while running and cementing casing and only
one well had losses while running a hole opener. Below is a list of the wells and
the lost circulating problems associated with them. We do not expect losses
while drilling this well.
· MPS-27: Lost returns cementing 7.0-in casing at approx. 800-ft MD. SuccessfuI1.0-in
top job.
· MPS-21: Lost returns cementing 7.0-in casing. Used TAM port collar at 1,OOO-ft with still
no returns. Successful 1.0-in top job.
· H-06 (1995): Lost 100-bbl running 9-5/8" casing, good returns during cementing except
no returns for last 20-bbl, contaminated cement to surface, cement 12 ft down, no top
job.
· H-05 (1995): Lost returns drilling at 5,360-ft (3,700-ft TVD), 40-bph to keep full, treated
wi LCM pills for two days, total losses 838-bbl, made hole opener run with some
continued losses.
. H-05 (1995): Ran 9-5/8" casing, lost 46-bbl conditioning mud.
. H-07 (1995): Lost 50-bbl running 9-5/8" casing.
· E-24 (1998): Made hole opener run, worked tight spots 1965, 2620, 3465, 5900 and
6000, lost 72-bbl while circulating.
· H-08A (1997): Had some minor losses while running 4-1/2" liner.
3
,
Miln 'pint MPS-11 Well ProQram
· Follow the LOST CIRCULATION DECISION MATRIX in the well program if fluid losses
occur while drilling. Break circulation slowly in all instances and get up to recommended
circulating rate slowly prior to cementing.
Expected Pressures
· It is unlikely there will be unexpected overpressure when drilling to the Schrader Bluff
formation. Reservoir pressure is expected to be (8.65 Ib/gal EMW). In this well, the
Schrader Bluff formation will be drilled with 9.1 to 9.2-lb/gal mud.
· There is a possibility of overpressure from MPE-29i via transmissive faults. However,
MPS-07 and MPS-09 were drilled into fault blocks between the MPS-11 location and the
MPE-29 injector and both MPS-07 and MPS-09 were normally pressured.
· Flow-check the well when the reservoir is entered.
Pad Data Sheet
· Please read the MPU S-Pad Data Sheet thoroughly.
Breathing
. N/A
Hydrocarbons
· Hydrocarbons will be encountered in the Ugnu through the Schrader Bluff. Free Gas
may be encountered below the Upper Ugnu.
Faults
· No faults are predicted to be crossed along the well trajectory.
· The well will be drilled into the graben between two faults.
· There is a chance of a faulted OBO section; the well is placed to minimise risk.
Hydrogen Sulfide
/
· Milne Point S-Pad is not designated as an H2S Pad.
Anti-collision Issues
· None, except for consideration of planned wells.
Integrity Testing
/
· Will be drilled on diverter and no FIT/LOT is required.

Winton,
As per our phone conversation today, we are now planning to run surface
casing on our next well, MPS-19. The directional plan and bottom-hole
location will remain the same. Changes to the drilling and casing program
and the cement program are detailed below:
12.25-in hole to 2,500-ft MD
9.625-in surface casing to 2,500-ft MD
8.5-in hole to 8,199-ft MD
7.0-in intermediate casing to 8,199-ft MD. L{sq~ TV';>
Surface casing cement program:
20-bbl Alpha Spacer
418-bbl Permafrost L cement to surface
45-bbl Premium G cement to 500-ft above casing shoe
Intermediate casing cement program:
20-bbl Alpha Spacer
29-bbl Permafrost E cement to 500-ft above top Ugnu sand 73(,5' -"'l {p f to )""'
45-bbl Premium G cement to 500-ft above top NA sand
Please let me know if you require any further details,
Cheers,
