GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in
other spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).

surface shoe (which will not affect the isolation below), what sundry approvals do we

Subject: MPS-01 (Well Suspension)
On 11/02/2005, workover operations on MPS-01 were curtailed as the objectives of the
workover were considered to be unachievable. Therefore, with verbal approval of the
suspension from Tom Maunder with the AOGCC, well suspension operations were initiated on
11/01/2005 with the completion of those operations being on 11/03/2005. The suspension of
the well included the plugging of the upper NE lateral of the well with a 20 barrel cement
squeezed from below a retainer in the 7 5/8" main-bore casing at 7200' MD (5 bbls of cement
(-100') were also placed above that retainer) while the lower two laterals (the OA and OB
laterals) remained isolated by mechanical barriers which had been previously installed in the
well. The cement plug was then pressure tested to 2050 psi for 30 minutes.
MPS-01 was an Ugnu/Schrader Bluff tri-Iateral production well with laterals in the Schrader
Bluff OB (the lower lateral), the Schrader Bluff OA (the middle lateral) and the Ugnu NE (the
upper lateral). As the lower two laterals (OA & OB) were isolated by a packer assembly with a
plug in the tail-pipe prior to drilling the upper lateral (NE) of the well in October of 2003, the
intent of this recent workover was to remove the plug in the tail-pipe (the PXN plug in an XN
nipple at 7578' MD) which would allow resumed production from the two lower OA and OB
laterals of the well.
In summary, as the workover progressed all attempts to access to the main-bore of the well
below the upper lateral (as required to recover the plug in the tail-pipe at 7578' MD) proved to
be unsuccessful due to what appears to be mechanical problems at the liner hanger for the
upper lateral. It is generally concluded, that all of our clean-out work efforts were either
diverted into the upper lateral or possibly to some open-hole sidetracking event. The deepest
clean-out on our final run into the well (using a 4 %" mill assembly spaced-out below a 6 %"
string mill) bottomed-out at 7592' failing to encounter any of the designed "no-go's" which
would have been an indicator that we indeed had access to the main-bore of the well. That
being the case, it was decided to suspend the well as detailed in this notice leaving the well
above the suspension as a candidate for a future MPS-Pad sidetrack.
(Note: A total of 3 Sundry notices will be submitted for the suspension of this well, one for
each of the 3 laterals which are now isolated below the cement/retainer in the 7 5/8" casing in
the main-bore of the well-7202'.)

Enclosed is the approved application for permit to redrill the above development well.
This permit to redrill does not exempt you from obtaining additional permits or approvals
required by law from other governmental agencies, and does not authorize conducting drilling
operations until all other required permits and approvals have been issued. In addition, the
Commission reserves the right to withdraw the permit in the event it was erroneously issued.
The permit is for a new wellbore segment of existing well MPS-01, Permit No 203-034, API 50-
029-23141-00. Production should continue to be reported as a function of the original API
number stated above.
Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter 25 of the
Alaska Administrative Code unless the Commission specifically authorizes a variance. Failure
to comply with an applicable provision of AS 31.05, Title 20, Chapter 25 of the Alaska
Administrative Code, or a Commission order, or the terms and conditions of this permit may
result in the revocation or suspension of the permit. Please provide at least twenty-four (24)
hours notice for a representative of the Commission to witness any required test. Contact the
Commission's North Slope petroleum field inspector at 659-3607 (pager).

· Hydrates have not been seen on S pad, but has been encountered in the surface holes on
other pads, especially nearby H-Pad. Hydrates generally occur just below the Permafrost (+1-
1800 ss) but may be present in shallower SV sands and also as deep as 3000 ft. Be alert for
the possibility of shallow gas and/or hydrates. Use recommended practices for drilling
hydrates. Increase mud wt; Maintain cool mud « 60 deg F); Limit ROP and reduce pump rate
while drilling hydrates; Circulate connections thoroughly. See Well Program and
Recommended Practices for specific information.
· Another item to watch out for is deeper gravels below the Permafrost or gravels that calve off
within the permafrost and will tend to drop out when the aerated hydrate gas cut mud is being
circulated out. Maintain adequate viscosity. In the past several Milne Pt wells have
experienced stuck pipe from this situation.
Lost Circulation
· We do not expect losses while drilling this well.
· Offset wells encountered losses while running and cementing casing and only one well had
losses while running a hole opener. Below is a list of the wells and the lost circulating problems
associated with them. H-06 (1995): Lost 100 bbls running 9-5/8" casing, good returns during
cementing except no returns for last 20 bbls, contaminated cement to surface, cement 12 ft
down, no top job. Follow the LOST CIRCULATION DECISION MATRIX in the well plan if fluid

losses occur while drilling. Break circulation slowly in all instances and get up to recommended
circulating rate slowly prior to cementing.
Expected Pressures
· It is unlikely there will be unexpected overpressure when drilling to the Schrader Bluff formation.
Reservoir pressure is expected to be (8.35 Ib/gal EMW). In this well, the Schrader Bluff
formation will be drilled with 9.1-lb/gal mud.
