GeNeRA~: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Submit a well
schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed.
ITeM ia: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection,
Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely
segregated. Each segregated pool is a completion.
IreM 4b: TPI (Top of Producing Interval).
ITeM 8: The Kelly Bushing and Ground level elevation in feet above mean sea level. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
IreM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
ITeM 20: Report true vertical thickness of permafrost in Box 20. Provide MD and TVD for the top and base of permafrost in Box 28.

04/24/09 MIRU, MU WOT 3.5" cement stinger. Change out slip bonnet on BOPE. RIH above retainer. Do rate test
C 0.5, 1, 1.5 , 2.0 bpm @ 2220 psig. Stung into retainer. -7.Ok down on retainer. Establish pump in rate
below cement retainer Cam? 9101' out the holes punched C~3 9108' into the 7" X 3.5" and out holes punched
C~ 8405'. PIR @ 0.5, 1.0, 1.5, 1.8 bpm Cagy 3900 psig. Call for cement. Monitor well pressure while
pumping min rates.
04/25/09 Continue to monitor well while pumping 1 % KCL below Halliburton cement retainer. Wait on cement,
fluids on location. Schedule here at 8:00 am. Cementers and all fluids on location at 9 am -Mix /pump
24 bbls Class G cmt at 15.8 ppg. Placed 20.5 bbl thru retainer lay in 4 bbls in tbg. Contaminate and
wash cement to 9110 ctm (cement retainer at 9116 ctm). Make three wash passes from 8900 to 9110
across WS set depth. Chase cement out of hole with biozan and washing across jewelry. Trap 100 psi
on well. RDMO. Job complete. Notify PCC / PO of well status. Note; Cement at 1000 psi compressive
strength at 1730 hrs.
04/26/09 WELL SHUT IN ON ARRIVAL. DRIFTED & TAGGED C~ 9081' SLM W/ 6'x2.72" DRIFT, 2' STEM, 2.71"
CENT., 2' STEM, 2.73" CENT. (13' OAL). ATTEMPT TO MIT-T TO 2500#. UNABLE TO REACH TEST
PRESSURE. RAN OUT OF FLUID. WELL TURNED OVER TO DSO.
04/27/09 T/I/0=450/620/20 CMIT TxIA PASSED to 3000 psi (Pre CTD) Pumped down Tbg with 41 bbls diesel to
reach test pressure for CMIT TxIA. T/IA lost 40/20 psi in the 1st 15 mins and 10/10 psi in the 2nd 15 mins
for a total pressure loss of 50/30 psi in 30 min test. Bled TxIA. FWHP's = 200/640/20. Wing shut, Swab
shut, SSV open, Master open, IA and OA open to gauges. Operator Notified.

04/06/09 T/IA/OA=SI/0/0 Temp=cold PPPOT-T (PASS) PPPOT-IC (PASS) (Pre CTD)
PPPOT-T RU 2 HP BT onto THA test void 0 psi, flushed void until clean returns achieved. Pressured to
5000 psi for 30 minute test, 5000/ 15 minutes, 4950/ 30 minutes. Bled void to 0 psi, RD test equipment.
Cycled and torqued "ET style" all were in good working condition
PPPOT-IC RU 2 HP BT onto IC test void 0 psi. flushed void until clean returns achieved. Pressured to
3500 psi for 30 minute test, 3500/ 15 minutes, 3500/ 30 minutes. Bled void to 0 psi, RD test equipment.
Cycle and torqued LDS "ET style" all were in good working condition.
04/15/09 WELL SHUT-IN ON ARRIVAL (E-LINE TUBING PUNCH). INITIAL T/I/O: 380/600/0. FIRE 1.56"
SMALL CHARGE TUBING PUNCH FROM 8405'-8407' (9 HOLES). TIED INTO TUBING TALLY AND
LDL DATED 24-FEB-2009.
04/16/09 T/I/0=500/640/40 Assist E-Line with injectivity test. (Pre CTD) Pumped 12.7 bbls crude down Tbg, IA
tracked, T/IA pressured up to 1500 psi, held pressure for 15 min. Bled T/IA. Repressured Tbg/IA to 3000

04/16/09 INJECTIVITY TEST AT 1500 PSI AND 3000 PSI SHOWED NO INJECTION THROUGH TUBING PUNCH
HOLES INTO FORMATION. SECOND TUBING PUNCH PERFORATED FROM 8391' - 8393' WITH
SMALL CHARGES (9 SHOTS). SECOND INJECTIVITY TEST ALSO SHOWED NO INJECTION AT
1500 PSI OR 3000 PSI. RIH WITH RST IN GRAVEL PACK MODE. MAKE BASELINE GR PASS FROM

crude and 8.4 bbls diesel down Tbg in attempt to establish injectivity. Tbg & IA pressured up to 3000 psi.

Jack up and move rig to F Pad. Route of travel is from DS-04,
past PBOC, to West Dock Rd to Oxbow to Spine Rd to F Pad.
Spot Rig over well.
Accept rig at 14:30 hrs. NU BOP.
Grease tree valves.
Test BOP. Test witnessing waived by Chuck Sheve of AOGCC.
Valves /Rams tested are 2" Combi, 2-3/8" Combi, Upper 2"
Combi, Blind /Shear, Annular, Kill line valves on Stack, HCR,
Choke manifold valves, and Inner Reel valves. Also both TIW
valves.
Pump forward to get diesel and KCI water out of reel.
Pump backwards then cut 324 ft of CT to find cable.
Test continuity and resistance of wire, good.
Install CT connector, pull test to 35K, good.
Visual inspection of CT connector shows some dimples in
wrong spot. Cut it off and re-install it.
Pull test to 35K is good.
Install BHI cable head.
Pressure test inner reel valve and CT connector to 3500 psi,
good.
PJSM M/U BHA #1 - 2.74" Diamond window mill, 2.74" String

Re: F-45A Perforation Abandonment and new CTD Sidetrack Sundry Approval Request
Sundry approval is requested for F-45A. Isolation of the existing perforations with a CIBP (cast iron bridge plug)
followed by a new coil tubing drilling sidetrack is proposed. The CTD side track will target the Zone 1 A sands in
an offset fault block to the east. 1,200' of perforations in the Zone 1 A sands is anticipated.
WELL CURRENT HISTORY AND STATUS:
The original well, F-45, was a conventional Z2A producer drilled in 1991 targeting the south central GD portion of
F-Pad, and then completed with cemented 7" liner, and 4-1/2" x 3-1/2" tubing. No loss of returns were
encountered during drilling. It was initially perforated in a straddled 26P lens and at the base of the 21 P, which
flowed at 2700 bopd, 1100 GOR. The well flowed full time until 1993, when cycling for high GOR became
necessary. Perforations were added in 1993, and again in 1996, both times with no appreciable benefit. The
original well produced 2.7 MMbo (SLP) until 2003 when it was abandoned for a sidetrack option.
The first sidetrack, F-45A, was drilled in 2003. The sidetrack procedure included setting a cement plug in the
original wellbore from TD up to 9200', with all perforations to be closed behind cement. The new wellbore was
drilled, with a kickoff point at 9250. F-45A, also a Z2A producer, was completed with 2-3/8" liner from 9148', to
TD of the new wellbore at 11151'. The liner was set in the original 3-1/2" production tubing.
F-45A came online, fulltime, in May 2003 with an initial rate of 1400 bpd at 1800 GOR. The well began to cycle
in early 2004 when production dipped to 600 bpd at 40,000 GOR. The well cycled regularly until late 2006 when
GOR increased above marginal to 70,000 GOR. Attempts to cycle continued throughout 2006 and 2007,
however on time over the past two years has been 33%; with the last 1 year at 23%. Production rates have
stabilized at approximately 500 bpd.
The well produced 20 mmscfd from the beginning, leading to speculation about a liner top packer being
improperly set. In July 2006, a Production Log was ran to verify the leaky packer. There was an attempt to lower
gas rates by pulling and resetting the liner top packer in October 2006. Additionally perfs were added in 2006
between 9550' - 9600'. GOR did not decrease after the above well work.
Original production estimates for F-45A have proven conservative. F-45A was expected to produce 521,000
barrels over its life; to date F-45A has produced over 600,000 barrels.
REASON FOR SUNDRY AND SIDETRACK:
F-45A has seen its % on time decrease significantly over the past two years. For 2008, F-45A has been online
only 15% of the time. The well shows little healing, even after extended periods of being shut in. It is anticipated
that F-45A's on time will decrease even further as the "healed" GOR's remain above 50,000.

valves, upper/lower pipe slip rams, blind rams and choke
manifold valves to 3500 psi/5 min & 250 psi/5 min. Annular to
2500 psi. Perform accumulator draw down test.
Make-up BHA #1 on catwalk.

injector. Prep tugger line to coil.

bio to active pits to compensate for dilution from seawater in
wellbore. Pumping 10 bbl sweeps of gelled up KCL every 50
bbls while drilling.

high side. Kick on pumps. Free spin 2100 at 1.5 bpm. Flag

Summary of Operations:
Phase 1: Prep work
1. Integrity testing
2. Service coil to lay 17 ppg cement from 9464' to approx. 9;96' to provide for a cement whipstock.
A minimum of 10bbls of 17 ppg cement will be used. /
Phase 2: Mill window, Drill and Complete sidetrack: Planned for March 27, 2003.
Drilling coil will be used to mill the window at 9250' MD (8909'ss)from a cement ramp then drill and
complete the directional hole as per attached plan. Maximum hole angle will be 90 degrees.
Mud Program:
· Phase 1: Seawater
· Phase 2: Mill with seawater/biozan, drill with Flo-Pro (8.6 - 8.7 ppg) /
Disposal:
· All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
· All Class I wastes will go to Pad 3 for disposal.
Casing Program: /
The production li~e, Hi)be 2 3/8" ST-L from TD to approx. 9190'MD (8862'ss). The liner will be cemented
with a minimum 0 9,(S to bring cement up to top of liner.

Well Control: .
· BOP diagram is attached. /'
· Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 3500 psi.
· The annular preventer will be tested to 400 psi and 2500 psi.
Directional
· See attached directional plan. Max. planned hole angle is 90 deg.
· Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging
· MWD Gamma Ray will be run in..the open hole section. Memory CNL will be run inside the liner.
Hazards /
· The maximum recorded H2S on F Pad is 30 ppm on well F-43. .
· Zones 2 conglomerates may cause slow ROP, possible bit damage.
· Three small faults will be crossed, lost circulation is considered medium risk
