Title: Phone Alpine Drillina Team Leader 2.&0- (, It" Date ?{ 8 (o~ Prepared by Sharon Allsup-Drake INSTRUCTIONS General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Item 1a: Classification of Service wells: Gas injection, water injection, Water-Alternating-Gas Injection, salt water disposal, water supply for injection, observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. Item 4b: TPI (Top of Producing Interval). Item 8: the Kelly Bushing elevation in feet abour mean low low water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). Item 20: True vertical thickness. Item 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. Item 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). Item 26: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-in, Other (explain). Item 27: If no cores taken, indicate "none". Item 29: List all test information. If none, state "None", Form 10-407 Revised 12/2003 L . . The permit is for a new wellbore segment of existing well CRU CDI-12, Permit No 204-205, API No. 50-103-20501-00. Production should continue to be reported as a function of the original API number stated above. This permit to drill does not exempt you from obtaining additional permits or approvals required by law from other governmental agencies, and does not authorize conducting drilling operations until all other required permits and approvals have been issued. In addition, the Commission reserves the right to withdraw the permit in the event it was erroneously issued. Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter 25 of the Alaska Administrative Code unless the Commission specifically authorizes a variance. Failure to comply with an applicable provision of AS 31.05, Title 20, Chapter 25 of the Alaska Administrative Code, or a Commission order, or the terms and conditions of this permit may result in the revocation or suspension of the permit. Please provide at least twenty-four (24) hours notice for a representative of the Commission to witness any required test. Well Proximity Risks: Directional drilling / collision avoidance information as required by 20 ACC 25.050 (b) is provided in following attachments. CD1-11 will be the closest well to CD1-12. Well separation is 1416 ft at 11033 ft MD. Drillina Area Risks: There are no high-risk events (e.g. over-pressures zones) anticipated. / Lost circulation: Faulting within the Alpine reservoir section is considered to be the most likely cause of lost circulation. Standard LCM material has proved effective in dealing with lost circulation in previous development wells. Additional care and attention will be given to the higher potential of inducing a kick when drilling horizontally as opposed to vertically. Good drilling practices will be stressed to minimize the potential of taking swabbed kicks. I misunderstood the urgency of reporting sidetrack operations. If the recent wells are any indication, we will likely see more open hole sidetracks in the coming months. On future unplanned sidetracks in the horizontal I will be sure to send an email as soon as feasible after the decision is made to sidetrack. The original leg showed the Alpine C interval was becoming thinner. The primary goal of the sidetrack was to gather data regarding interval thickness, additional reservoir exposure was an added benefit. If you have any questions let me know. Here goes again. Tom ------------------------------------------------------------------------ ------------------------------------------- Vern, The notice is appreciated, however from what you say in your message it appears that the notice should have come a little sooner. I don't know what involvement you would have had over the weekend, but it seems an email could have been sent. The sidetrack was for geologic delineation purposes and there is 'pay quality' rock in both legs. I will send the permit package over for the L 1 leg today. The purpose for sidetracking is to delineate the thickness of the reservoir. There is reservoir quality rock'/ in both legs. The well will be sidetracked at 12515' MD (+/- 12435' MD survey tie on) j and the lateral drilled to +/-12954' MD. The lateral will be drilled in the same plane as the original wellbore, but will be drilled at a slightly deeper TVD. Please reference the permit to drill for CD1-12 for further details. Note that many of the details in the attached form 10-401 reflect the originally planned depths. Please find attached the information required by 20 ACC 25.005 (a) and (c) for your review. Pertinent information attached to this application includes the following: 1. Form 1 0-401 APPLICATION FOR Permit to Drill per 20 ACC 25.005 (a). 2. A proposed drilling program Information pertinent to this application that is presently on file at the AOGCC is: 1. Diagrams of the BOP equipment, diverter equipment and choke manifold layout as required by 20 ACC 25.035 (a) and (b). 2. A description of the drilling fluids handling system. 3.
