INSTRUCTIONS
General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Submit
a well schematic diagram with each 10407 well completion report and 10-404 well sundry report when the downhole well design is changed.
All laboratory analytical reports regarding samples or tests from a well must be submitted to the AOGCC, no matter when the analyses are conducted.
Item Ia. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is
a completion.
Item ib. Well Class Service wells: Gas Injection, Water Injection, Water-Altemating-Gas Injection, Salt Water Disp, Water Supply for Injection, Observation or Other.
Item 4b. TPI (Top of Producing Interval).
Item 9. The Kelly Bushing and Ground Level elevations in feet above mean sea level. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
Item 15. The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
Item 20. Report true vertical thickness of permafrost in Box 20. Provide MD and TVD for the top and base of permafrost in Box 29.

There is a Sierra-Nevada sand package on top of the HRZ on the next 2ES sidetrack. The Geo's are
asking for some OHL's and sidewall cores now and a change in the completion as specified below, if the
sand is indeed worth saving. Would we need to submit a revised 10-403 before completing if the sand
proves up, or can we wait until the rig completes the well?

Phil,
Here is the TENTATIVE mandrel spacing design for F-34B should the Sierra Nevada look interesting
enough to make us want to preserve access. For this design, we assumed a* packer depth of 8077' MD
(-200' MD above the top of Sierra Nevada, to allow room for the tailpipe)*. We'd like to minimize the
amount of 7" interval in the well to better our hydraulics, so we'd like set our 4-1/2" liner top at -8350' MD
(bottom of Sierra Nevada is -8330' MD). We probably need to add wireline entry guides to facilitate
entry into the liner and tubing tail.
Let me know if you have any questions/concerns about these plans.

Operations SUmmary
Current Condition:
1 . There is T X IA communication. LDL determined leaks at -9,767', 10,019', and 10,073'. (9/30/02)
2. There is an 'XX' TTP set in the lower 'X' nipple at 10,739' SLM (10,758' tally). TTP set 1/02/03.
3. AN MITOA to 2000 psi failed on 1/17/05 with LLR of 0.5 gpm (120 psi loss in 15 minutes).
Pre-Riq Operations:
1. R/U slick-line. RIH and retrieve the TTP from the 'X' nipple at 10,739' SLM.
2. R/U coiled tubing. RIH and plug and abandon the open perforations. Leave top of cement at -10,650'.
The cement volume required is -30.0bbls. This is based upon 1,212' of 4 W', 12.6# liner, and 135' of
4 W', 12.6#, tubing, and approximately 10.0bbls to squeeze the open perforations.
3. Drift the 4 W' tubing to top of cement plug at -10,650'md, below the planned tubing cut depth of
-7,700'md. Confirm top of cement.
4. R/U SWS E-line. RIH with appropriate chemical cutter and cut the 4 W' tubing at -7,700'md.
5. Circulate the well to seawater through the tubing cut, until returns are clean. Conduct a CMIT T X IA to
3,000psi for 30 minutes.
6. Freeze protect to -2,200' MD with diesel.
7. Bleed casing and annulus pressures to zero.
8. Function all casing and tubing LDS and re-torque as recommended by Cameron. Test 9 5/8" packoff.
9. Set a BPV. Secure the well. Remove the well house and flow line. Level the pad as necessary.

2. Pull BPV. Circulate out diesel freeze protection and displace the well to seawater. Install TWC.
3. Nipple down the tree. Inspect tubing hanger threads.
4. Nipple up and test BOPE to 3,500psi. R/U DSM lubricator and pull TWC.
5. R/U. Pull the 4 W' tubing from the tubing cut at -7,700'md and above. Be prepared to spool up the
HES SSSV control line.
6. R/U SWS E-line and drift to the EZSV setting depth. M/U a HES 9 5/8" EZSV bridge plug. RIH with
EZSV and set at -7,520'md. POOH and RID E-line.

24. R/U and run a 4 1/2", 12.6#, 13Cr-80, VAM TOP completion, stinging into the 4 1/2" liner tie back /'
sleeve.
25. Space out and land the completion. RILDS.
26. Reverse circulate the well to seawater with Corrosion Inhibitor.
27. Drop the ball and rod. Set the packer and individually test the tubing and annulus to 3,500psi for 30 //
minutes. Record each test. Shear the DCR valve in the uppermost GLM.
28. Install a TWC.
29. Nipple down the BOPE. Nipple up the tree and test to 5,OOOpsi. Pull TWC.
30. R/U LRHOS. Reverse in sufficient volume of diesel to freeze-protect the well to -2,200'md. Allow the/
diesel freeze protection fluid to U-tube to equilibrium.
31. Install BPV. Secure the well.
32. Rig down and move off.

1. MI I RU slick line. Pull the ball and rod I RHC plug from landing nipple below the production packer.

~ An objective of this drilling proposal is to complete this well with 50 feet of perforations in quality
Zone 4 sand. The well may include perforations in upper Zone 3 if the OWC occurs in Zone 3.
~ F-34B is expected to cross a single, small fault at the Shublik depth. The F-34 parent well bore
penetrated the same fault mapped at TSHU and no losses were recorded.
~ No significant thinning section recorded above Shublik which indicates the throw of this fault is
minor. There is a high possibility that this fault dies out and did not reach the new wellbore.
~ Hard lines for the drilling polygon include: 1) ALL POLYGON LINES ARE HARDLlNES. To the
north there area 200 ft away from E-W fault trend (30ft throw), this also applied to the east and
west side. The southern part is determined by the distance to the original F-34 wellbore.
