Enclosed please find a spreadsheet With a list of wells ftom the Kuþaruk field (KRU). Each of .
these wells was foùud to have a void ¡níÏ1e·conductor.These voids were filled with a corrosion
inhibitor, engineered to prevent water ftom entering the annular space. As per previous
agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments
took place and meets the requirements of form .1 0-404, Report of Sundry Operations.
The cOlrosÌon inhibitor/sealant was pumped August 19,2007. .The attached spreadsheet
presents the well name, top of cement depth prior to filling, and volumes used on each
conductor. .

There is a couple reasons for the excess cement. The depth is calculated by a
string, so if that hangs up on any debris in the OOA, it may give us a false depth.
Also since there is no way to circulate, your right, we rely on the hose to fill up
the DCA. We just wanted to make sure we had enough cement to fill to surface.

I was looking at the packet recently sent on the 6 wells that had the
OOAs topped-off.
It appears that the pumped volumes varied anywhere from 1.3 to 2.2 times
calculated.
Was XS cement circulated to assure that the OOA was full?? That would
seem prudent, since
it may not have been possible to get the hose all the way down.

Dear Mr. Maunder:
Enclosed please find a spreadsheet with a list of wells from the~~il,~~ and Tam fields. Each
of these wells was found to have a void in the conductor by surface casing annulus. The voids
were filled with cement to the top of the circulation ports in the conductor, or in some cases
holes were cut in the conductor to facilitate fill-up. As per previous agreement with the
AOGCC, this letter and spreadsheet serves as notification that the treatments took place and
meets the requirements of form 10-404, Report of Sundry Operations.
The top-fill operation was completed on September 14,2004. Dowell-Schlumberger batch-
mixed Arctic Set I cement to 15.8 ppg in a blender tub. The cement was placed in the conductor
from the bottom-up via a hose ran to the bottom of the void. The attached spreadsheet presents
the well name, top of cement depth prior to filling, and volume used on each conductor.
Note that in most cases these wells<l}ave been previously reported as having surface casing leaks
and have either been repaired by ~xtavation and patching or injection of sealant material.

7/11/2004Chuck Scheve waived witness of surface casing repair.
7/14/2004 Installed external surface casing patch. Patch was 46" long section of 10-3/4" csg, split and
welded to surface casing across corroded casing. See drawing & pictures.
7/15/2004 MITOA to 1820 psi passed. Held for 60 mins. See attached MITOA report. Installed anodes
and coating on the patch and exposed surface casing. See pictures.
7/16/2004 Conductor window patched. Installed hose down OOA for upcomming cmt top job. See
pictures.
7/17/20041 Excavation backfilled and cellar installed.

This application for Sundry approval for well 3B-06 (PTD 1850760) is for the repair of a surface
casing leak. In March 2004 Operations personnel reported the presence of diesel liquids seeping
from the top of the conductor casing into the cellar. After installing spill containment around the
well an MIT -OA was attempted. The casing leak was sufficient to estab"h a liquid rate of 8
gpm @ 2000 psi. The AOGCC was notified of the problem on 4/1/04.
On 4/4/04 a casing sealant (Seal-Tite) was pumped in an attempt to seal the leak internally.
Initially the leak was sealed and passed an MITOA to 2000 psi. However, within approximately
2-weeks the repair broke down and the leak reappeared. The severity of the leak is apparently
beyond repair limits of sealants, so a mechanical fix is justified.
Diagnostics have been performed to locate the leak. Injecting nitrogen down the OA established
the leak depth. Indications were that the leak was very near the surface. A boroscope was used to
inspect the inside of the surface casing and the leak was determined to be located at 7- feet below
the OA annular valve.
Repairing the surface casing will require the following general steps. A more detailed procedure
is attached.
1. Shut-in and safe-out the well with plugs and load with seawater and freeze protection.
2. Remove the wellhouse, platforms, and equipment ftom around the wellhead.
3. Excavate gravel ftom around the well to approximately 2-feet below the leak site. This will
require removal of approximately 5-feet of gravel pad (subject to CP AI safety requirements).
4. Cut away the conductor casing and remove cement around surface casing for inspection.
5. CP AI Mechanical Engineers will determine appropriate patch and! or repair to return the
casing to operational integrity.
6. QA/QC the repair and MIT -OA for final check.
7. Replace the conductor and fill void with cement.
8. Backfill gravel around wellhead.
9. Remove plugs and return the well to production.
10. File 10-404 post repair.

38-06 SURFACE CASING REPAIR PROCEDURE
Objective
Repair the leak in the surface casing. The leak is known to be approximately 7-feet below the
surface casing annular valve. The extent of damage is unknown; the conductor casing will
require partial removal to expose the damaged surface casing. Excavation around the wellhead
will be necessary to provide adequate workroom.
Major Steps
1. Excavate around wellhead as needed to provide workroom. Will require As-built review for
buried lines or utilities. Excavation depth will be approximately 7 -feet below pad level.
Remove cellar box as needed for access. Excavation must conform to Safety Department
requirements.
2. Cut windows in and/or remove conductor to expose damaged surface casing. Remove
cement sheath from surface casing.
3. Inspect surface casing for damage and leak path. Remove additional conductor for access
as necessary. Identify failed or corroded area for repair. Prepare welding repair plan as
needed (seal weld, patch, or other).
4. Repair casing, QAlQC. Cover with corrosion protection materials.
5. Install new conductor casing and back-fill void area with cement.
5. Replace excavated gravel and cellar box as needed. Install wellhouse and floor kit.
General
1. Network number: 10032001 Activity code: ZX12
2. The surface casing to be repaired is 9-5/8" J-55 36# casing. The conductor to be removed
is 16" 62.5# H-40.
3. The well is an oil producer. The safe-out procedure includes displacing the tubing and inner
annulus with water and a diesel cap for freeze-protection. A tubing tail plug will be installed
to secure the well from the formation. The well will be isolated from the facility by closed
wing and lateral valves. Energy isolation lockout & tag-out procedures apply. A nitrogen
purge will be maintained on the inner annulus (IA), outer annulus (OA) and conductor during
hotwork operations with continuous monitoring of 02 and LEL levels.
4. The wellhouse, floor kit and cellar box will require removal for the excavation and repair.
5. Anyon-site personnel have the authority to stop or suspend the job should any safe-out or
procedural conditions become suspect. Safety is everyone's responsibility.
Repair Preparation:
1. Prepare In-Service package for cutting and welding. (WG, ENG)
2. Shut-in and secure well. Set tubing tail plug and dummy gaslift valves. Displace tubing and
IA with seawater. Leave diesel freeze-protection fluids on tubing and IA to 2000'. (WG)
3. Bleed tubing, IA (3-1/2" x 7") and OA (7" x 9-5/8") to zero psi. Verify flow is zero; inability to
maintain zero psi prior or during job will cancel the welding operation. (APC & WG)

4. Prep 9-5/8" casing head: Pull VR plug and install opposing valve on casing head for N2
'purge. (WG & Valve Shop)
5. Prep 7" casing head: Pull VR plug and install opposing valve on casing head for N2 purge.
(WG & Valve Shop)
6. Verify OA fluid level is below repair point; remove fluid via suction hose as needed. (APC &
WG)
7. Excavate around wellhead as needed for access to surface casing below grade.
8. Drill and tap a hole in the conductor pipe near ground level for N2 purge of conductor during
cutting and removal.
9. Install stick scaffold around wellhead for access to wellhead valves.
10. Install purge lines from N2 bottle rack to lA, OA and conductor. Discharge to bleed trailer
will be monitored for LEL and 02 levels and must be accessible. Position bleed trailer
downwind and as far from wellhead as is practical. (APC)
11. Station several hand-held extinguishers around wellhead. (APC & WG)
12. Inspect and remove any combustibles in accordance with Hot Work standard. (APC)
13. Install welding blankets to cover ground around wellhead. (APC)
14. Hold pre-job safety meeting and ensure all permitting is in order. (WG)
Job Procedure
1. Safety department will check entire well area before Hot Work commences. Sniff entire
piping layout around wellhead to check for any potential flammable material sources. (APC
and/or CPA)
2. Purge the lA, OA and conductor with nitrogen, venting to slop trailer. Purged gas is to be
monitored throughout job for LEL and 02 levels. Target 02 level is < 5%. The heating /
cutting operation may commence after target 02 level is reached. (WG) (APC Safety)
3. Establish fire watch and begin hot work, including prepping surface with torch, brushes,
grinding or buffing as required. (APC)
4. Cut windows in conductor to expose surface casing and cement.
5. Chip out and remove cement sheath from surface casing for inspection.
6. Cut conductor away as needed to fully expose damaged area of casing (note-the conductor
can be entirely removed and replaced later).
7. Stop N2 purge and remove low-pressure equipment from well.
8. Inspect casing damage and acquire data to formulate a welding and/or patch plan.
9. Pick up all trash and debris for disposal. Please leave a clean location! (ALL)
Repair Plan Procedure
1. Prepare welding/repair plan as per Facility Engineer recommendations.
2. Repeat Job Procedure steps for purging and energy isolation as described above.
3. Repair surface casing as per Facility Engineer plans.
4. Note: Replacement of the conductor will be performed after the repair is done and QAlQC is
complete.

I looked at the 30-04 file and it does look to be similar. One question
I have (another) is, were there any pictures taken of the surface casing
after the conductor had been cut away?? The information filed with the

Torn: 3B-06 (PTD 1850760) was recently reported to our office as
having a small leak on or near the wellhead. Our crew checked it out
and determined we have another surface casing leak. We tested the
rate through the leak at 8 gpm with 2000 psi differential pressure.
The leak appears to be approximately 43" below the OA annular valve.
This type of leak is too large for a sealant, but is a good candidate
to dig up and repair similar to the 30-04 well we did last summer.
The well has passed a TIFL, and so has good tubing integrity. Packoff
tests have also passed, and we have no indications of IA x OA
communication. I do not have the final word internally yet, but it is
likely we will waiver this well to keep on line until this summer
when we can excavate and repair the casing from the surface. Please
let me know if you have any questions about our plan.

SundaY, January. 7, 2001:1 traveled to the KuParuk River Field and witnessed the
semi-annual safety valve system testing on 3A & 3B Pads.
As the attached AOGcC iSafety valve sYStem Test Reports indicate I Witnessed the
testing of 8 wells and 16 components on 3^ Pad and 6 wells and 12 components on
3B Pad with 1 failure observed: The surface safety'valve on well 3^-08 had a slow
leak when pressure tested. This valve will be serviced and retested by the operator.
John ^rend conducted the testing today he demonstrated good test procedure and
was a pleasure to Work with ~
The wellhoUSes On these two Pads were very clean and all related equipment
appeared to be in good Condition.'

4. Pull WRDP @1944' MD-RKB. Formation sand may be encountered.
5. MIRU E-line unit. Pressure test equipment.
6. RIH with Schlumberger Enerjet strip gun (12 SPF and +/- 30 degree
phasing). At the forman's discretion, add extra weight bars.
7. Tie in with DIL dated 5/19/85.
8. Adjust gas lift rate to barely flow the well for a few minutes and to allow
underbalanced perforating. Use approx. 100-200 MSCFD of lift gas. Check the
weight for any sign of the tool coming up in hole.
9. Perforate intervals 8190'-8210', 8218'-8248' and 8270'-8280' MD-RKB.
POOH. RD equipment.
Flow the well. Obtain well tests at 400 and 800 MSCFD gas lift rates.

Enclosed is the approved application for permit to drill the
above referenced well.
If coring is conducted, a brief description of each core and a
one cubic inch siZe chip from each foot of recovered core is
required. Samples of drill cuttings and a mud log are not
required. A continuous directional survey is required as per
20 AAC 25.050(b)(5). If available, a tape containing the
digitized log information shall be submitted on all logs for
copying except experimental logs, velocity surveys and dipmeter
surveys.
Many rivers in Alaska and their, drainage systems have been
classified as important for the spawning or migration of
anadromous fish. Operations in these areas are subject to
AS 16.05.870 and the regulations promulgated thereunder (Title 5,
Alaska Administrative Code). Prior to commencing operations you
may be contacted by the Habitat Coordinator's Office, Department
of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
Chapter 3, Article 7 and the regulations promulgated thereunder
(Title 18, Alaska Administrative Code,~/~hapter 70) and by the
Federal Water Pollution Control Act, as amended. Prior to
comm~encing operations you may be contacted by a representative of
the Department of Environmental Conservation.

hours prior to commencing installation of the blowout prevention
equipment so that a representative of the Commission may be
present to witness testing of the equipment before the surface
casing shoe is drilled. [qhere a diverter system is required,
please also notify this office 24 hours prior to commencing
equipment installation so that the Commission may witness testing
before drilling below the shoe of the conductor pipe.
In the event of suspension or abandonment, please give this
office adequate advance notification so that we may have a
witness present.

ARCO Alaska, Inc., proposes to drill a Kuparuk River Field development well to approximately 8940' MD (6467'
TVD). A 20", 2000 psi annular diverter will be installed on the 16" conductor while drilling the surface hole.
The 9-5/8" casing will be set through the West Sak sands at approximately 4193' MD(3311' TVD). A 13-5/8", 5000
psi, RSRRA BOPE stack will be installed on the 9-5/8" surface pipe. it will be tested upon installation and
weekly thereafter. Test pressure will be 3000 psig. Expected maximum surface pressure is 2818 psi. Pressure
calculations are based on latest reservoir pressure. Surface-pressure is calculated with an unexpanded gas
bubble at surface, including temperature effects. Selected intervals will be logged. The well will be completed
by the workover rig with 3-1/2", 9.2#, J-55, 8rd tbg. Non-freezing fluids will be left in uncemented annuli
through the permafrost interval. Selected intervals will be perforated within the Kuparuk formation and reported
on subsequent reports. A downhole safety valve will be (see attached sheet)

installed and tested upon completion of the job. 3B-6 will be 53' away from 38-5 at 830'MD. There are no other
close wellbores or crossings anywhere downhole. Top of cement for the production string will be placed at
7307'MD, approximately IO00'MD above the top of the Kuparuk Sands. Reserve pit fluids will be disposed of down
the 9-5/8" x 7" annulus. The 9-5/8" x 7" annulus will be left with a non-freezing fluid during any
interruptions in the disposal process. After the workover rig has moved off location, this well will be
fracture stimulated.
