But the official forms should be using the official name of the well. {n talking with Jim about these two wells, he gave me direction as to how to fill in the 404. To address this, when you change these forms (403 & 404} to include the TVD of the packer, there needs to be a box for a short verbal description of the purpose of the form, not just a check box up top. It would have been easy to add a comment to the form to make it mean more to whoever picked it up Sorry far the confusion...thought these might go to Jim to process as he knew all about them. In response to a request by the AOGCC on a Sundry Approval (No. 302-268) for Dolly Varden Well No. 0-22 (Permit No. 169-108), McArthur River Field, Chevron submits the following information concerning an assessment of the future utility of this well. Well No. 0-22, an injector which was shut-in in December 1982 due to a leaky tree, will continue to be considered as a future workover candidate as a G-Zone i'njector. Please note that this information has been reported to the AOGCC (Mr. Jack Hartz) yearly on an annual shut-in well report. However, in the event that this report is not sufficient as a response to the sundry or has not been reaching you, we are additionally notifying you through this submittal. Attached for informational purposes are copies of the annual shut-in reports for 2002, 2003, and 2004 mentioned above, a copy of the sundry, and a schematic. Chevron will monitor and record daily injection rates and tubing and casing pressures on the well and provide a report to the AOGCC on a monthly basis. Maunder, Thomas E (DOA) From: Thomas Maunder [tom_maunder @admin.state.ak.us] Sent: Wednesday, April 21, 2004 12:05 PM To: Regg, James B (DOA) Cc: Crisp, John H (DOA) Subject: Dolly Inspections r - \ 0 Jim and John, Last week John was out to Dolly and encountered some interesting requests with regard to MITs on D -9RD, D -16RD and D -22. I have looked at the files and offer the following. D -9RD appears to be a normal Class II FOR well. Last MIT was satisfactory and there is no indication of communication. Larry Greenstein's message has a comment that the "inspector is to determine the annulus test pressure ". I am not sure why this would be there. The operator should have the completion information to make the necessary calculation for the applied pressure. D -16RD and D -22 have not been in service for about 20 years. The last information in the files are analyses I did in early 2003. The sundries had a note that by the end of 3rd Q 2003 that Unocal should make some determination of the future utility of these wells. I don't think we ever got anything from them. This "SI MIT" is a 6 day pressure observation to see how a small pressure change effect the monitored casing spaces. I guess since Unocal is now putting all their pressure information into a data gathering spreadsheet that they would like to present that information as proof of integrity. If that is what they would like to do, then it is appropriate for them to make a formal application to do so. These 2 wells are a couple of nearly 90 wells that Unocal lists on the SI well list. As I remember, they list nearly all wells as having future utility for sidetrack which may be true, however is probably wishful thinking. I think we should probably look at how we want these long term idled wells handled. This note evaluates Unocal's request and recommends approval of it. According to the RBDMS database, this well has not been in service since early 1983. The file does not have much information since the original completion in 1970. The well was drilled as an injection well. There are 2 records in the MIT binder that Blair examined the bond log on 9/14/88 and 12/29/92. Although it is stated that MI was indicated in both evaluations, the 2nd would indicated that the liner lap in the well was squeezed and it is noted that there was an improvement in the log presentation. It is unclear why the well has not been in service for so long. The monitoring information provided is similar to that submitted in 1998. In the submitted information, the initially observed pressures are 50/80/0 (tubing, IA, OA). 200 psi is applied to the IA with the resultant observed pressures of 120/330/0. The pressures essentially remain constant through the 4 day observation period. Area Injection Order 5 allows injection into Trading Bay Unit, McArthur River Field. The BHL of well D-22 is located in the covered area. The portion of aquifers beneath Cook Inlet described by a 1/4 mile area beyond and lying directly below the Trading Bay Unit are exempted for Class II injection activities by 40 CFR 147.102(b)(2)(B) and 20 AAC 25.440(c). Granting Unocal's request will not endanger any fresh water. I recommend acceptance of the submitted observation information. Unocal should add this well to the monthly monitoring report and plan to perform another "shut-in MIT" in July 2004. Unocal should be reporting this well as required by 20 AAC 25.115 (a) [Shut- In Wells]. In addition to those requirements, Unocal should provide an assessment of the future utility of this well prior to the end of 3~d quarter 2003.
