Pre wireline meeting was preformed @ 8:20 am it was also asked if this intended work would require a sundry-NO,the intended work scope was to perform a gauge run then change out#3,4 &5 GL valve then return to production. Changed out the above GL valve on 09/08/13 and returned well to production for testing,the night of 09/08/13 the well u-tubed gas again. 09/6/134400W - Shot fluid level the next morning 09/09/13 @ 6:am,5710'.Spent the day filling the well with clean fluids.Asked wireline to get a D&D hole finder onboard to find the leak, Production Forman went home after that request. 09/10/13-Tuesday Production Forman was informed @ 4PM on 9/10/13 that the hole was located @ 5742'and that we could utilize the patch removed from D-30 that was dressed and ready at the Shop, Production Forman approved the proposed plan without thinking of the required sundry. Patch was set in tubing @ 9 om from 5748-5720,returned well to production. While reviewing field reports this morning we became aware of this work and discovered that a sundry request was never filed. Per our telephone discussion we will file a 10-404 ASAP with the details. We will also review well activities requiring Sundry application with our production operations team including both production foremen and lead operators Here is the timeline of events from the field: Wireline arrived onboard Dolly Varden 9/8/2013 Pre wreline meeting was preformed @ 8:20 am it was also asked if this intended work would require a sundry- NO, the intended work scope was to perform a gauge run then change out#3,4&5 GL valve then return to production. Mr. Grimaldi arrived on Dolly Varden Platform and observed the well flowing to the clean-up tank at 30 psi. The well was shut in, allowed to build pressure and opened to the clean-up tank through 4 cycles over a total observation period of 4 hours. A maximum build up of 9 psi was noted. When the well was opened to flow, a maximum gas rate of 300 scf/hr was recorded which quickly decreased to below the meter threshold of 180 scf/hr if 4 minutes or less. At the end of the observation periods, there was 0 pressure and the gas rate was below the meter threshold of 180 scf/hr. No flow was confirmed using a "bubble bucket". Throughout the observation periods, no liquid flow was observed and only a slight gas flow was noted. The observed performance of TBU well D-32 (170-037) demonstrated that the well is incapable of unassisted flow of hydrocarbons to the surface. It has been concluded that TBU well D-32 (170-037) meets the criteria of a "no-flow" performance test and as such, is considered incapable of unassisted flow to the surface. The well's performance meets the criteria at 20 AAC 25.265 (b). Therefore, the well is not required to have a fail-safe automatic SSSV, however, the pilot and SSV must be maintained in satisfactory operating condition. If for any reason the well again becomes capable of unassisted flow of hydrocarbons, the SSSV must be returned to service. A copy of this letter should be maintained in the respective well file on board Dolly Varden.
