Re:
Administrative Abandonment Action
Middle Ground Shoal A-31-14 Permit # 77-0031
State of Alaska
Oil and Gas Conservation Commission
June 20, 1994
This memo will remain at the front of the subject well file.
Well A-31-14 was replaced by A31-14LW (permit # 94-0044, APl # 50-733-20305-62) when the permit
was approved 3/23/94 and operations completed in May, 1994. The replacement well incorporates a
lateral with the existing well bore. The lateral along with the original hole will be used for production of
hydrocarbons. A review of both well files indicates administrative abandonment of the originating well
bore is appropriate according to current AOGCC procedures for wells where additional permitted laterals -
are drilled from an existing well bore.
20 AAC 25.005(e) requires a Permit to Drill for each hole drilled below the conductor unless drilled to
straighten the original hole, sidetrack junk, or correct mechanical difficulties. In addition, AOGCC data
bases require each conductor has only one permit number and APl number. Administrative abandonment
of the original permit only eliminates duplication in the databases. Information for the original well and its
replacement will be maintained in their respective files.
The operator has been notified of this action. All future correspondence with the AOGCC should reflect
the new permit number, APl number and well data.

Jeff,
This message follows on our ongoing discussion regarding the repairs to conductor and in some cases surface
casing on Platform A. We have received form 10-404s for first 4 wells listed below. You have indicated that well
#5 wil{ repaired soon. What are the plans for wells 6 and 7? Also in the submitted report far A11-12, tfie °smali"
drawing is rather dark. Do you have a better image? When the document is scanned, the poor image wilE
essentially be totaDy dark.
Thanks in advance. I look forward to your reply.

exposing an 8" band of corrosion ranging from 0.100" to 0.350" wall thickness remaining on the 10-3I4" surface
casing (nominal wall thickness 0.350"). Plan to repair area of corrosion and re-fill annulus with grout.
3. A22-~14LS (194-071) - No visible exterior corrosion on conductor. Canductor UT readings revealed corrosion
(minimum of 0.300" wall thickness remaining} which is limited to the top of the conductor. Graut was chipped
down 6" and surface casing is ready to UT test. If UT test is good then pian to re-grout surFace csg x conductor
annulus.
4. A42-14 {167-041) - No visibie exterior corrosion on conduc#or. UT readings showed scattered corrasion near
top with minimum wall thickness reading of 0.300". Grout was chipped ta 10" depth. UT readings on the surface
casing looked good with wall thickness readings ranging from 0.333"-0.350" (nominat wall thickness of 10-314"
surface casing is 0.350"). Plan to re-flll with grout.
5. A44-11 (167-035) - Two hales found in conductor 15" from top and 7" apart. UT readings show that corrosion
extends downward. At 36" from top minimum wall thickness is 0.100". Grout was chipped down and found smalt
holes in surface csg. Chipped down another 2 feet and found a void in the surface casing x conductar annulus
that extends down +t-35'. This void was filled w/ water. Cut window lower in conductor and drained water.
Surtace casing looks good at this depth. Plan to weld repair surface casing and fill conductor with grout to
surface.
6. A31-14~W {994-044) - No visible exterior corrosion on conductar. Conductor UT readings showed scattered
corrosion extending fram top #0 44" down. Drilled holes in the thin spots of the conductar and found 3-4' void full
of water. Cut a 9" x 10" window in conductor approximately 54" fram top. Surface casing looks good at this
depth. Void extends to approximately 5-6' from top of conductor. Chipped grout from top of conductar and found
1-1/4" x 8" hole in surface casing at 20" from top. Cut window in conductor in-fine with surface casing corrosion to
get a better look at the carrosion and 7-5/8" intermediate casing. The 7-5/8" intermediate casing looks good.
Surtace casing corrosion appears to be limited to the top of the void area. Plan to repair surface casing by
welding fabricated sleeve over area of corrosion then fill void with grout.
7. A32-11R {193-092) - 3 small hoEes in conductor. U7 readings shpwed corrosion limited to top 36" of conductor
with 0.160" minimum wall thickness remaining. Chipped groui dawn from surface of conductor and faund small
holes through surface casing. Waiting on more UT numbers to determine extent of surface casing corrosion.
Plar~ to find extent of corrosion, weld repair surface casing and re-fill annulus with grout.

Jim Regg and 1 have examined the information you have provided and it will not be necessary to submit sundries
for the work. We will treat your message as equivalent notification. You are reques#ed to submit 404 sundry
reports for each well covering any examination worMc and the u!#ima#e repair(s). In arder to have a complete
re~rd of the work, it would be helpful if you could submit before and after photos that document the condition of
the surFace casings. It appears that with the exception of A44-11 that any damages are ~shallow'. Prior to
finalizing the repairs on A44-11, ptease have the platform contact the Inspectars so that one might travel to the
platform and examine this well. I will ptace a copy of this message in the various wel! fi1es. 1~ will look far the
404s documenting the repairs.
A further question, has anything similar been no6ced on Platfonn C? Ca11 or message with any questions.

Tom,
Per our conversa#ion Tuesday and your request here is a summary of what we have found. We are unsure if we
need to file a sundry notice to repair the surtace casing. Please look this over at your earliest cpnvenience and let
us know if we need to file a sundry. We would like to begin repairs ASAP. Please contact me with any questions.
tn the summer of 2007 production personnel on the piatfiorm nodced a small area of bubbling paint and rust stains
on the conductor of A32-11R. On July 16, 2007 an ultrasonic wall thickness inspec~on was performed on all
conductors on both plat#orms (A & C). Seven conductors on Ptaiform A were iderttified as having possible
corrosion and flagged for further inspection. The remaining conductors on A and all conductors on C appear to
k,~ in good condition.
The annular space belween the outside of ifie surface casing and inside af the conductor pi~ is filled with
grout/cement to the top of the conduc#or. Starting in June 2008 this grout/cement has ~en chipped out to
varying depths on the seven wells in question to determine if any corrosion exists an the surface casing. Below is
the summary of findings for each well:
(Conductor pipe is 24" C?D w/ 0.500" - 0.520" nominal wall thickness)
A11-12 (166-044) - Ultrasonic Thickness (UT} showed hole in conductor 1.5° below top and other corrasion
{0.160" wail thickness remaining) was noted. Conosion limited to top of condactor. GrouUcement was chipped
down to 20" to inspect surtace casing. Plan to weld repair conductor and re-fill canductor with grout.
A11-13RD2 (187-060j - No visible exterior c:orrosion on conductor. Conductor UT readings shawed corrosion

exposing an 8" band af carrosion ranging fcom 0.100" ta 0.350" wall thickness remaining on the 10-3/4" surtace
casing (nominal wall thickness 0.350"). Plan to repair area of co~rosion and re-fill annulus with graut.
A22-14LS {194-071) - No visible exterior corrosion on conductor. Conductor UT readings revealed corrosion
(minimum of 0.300" wall thickness remaining) which is limited to the top of the conductor. Grout was chipped
down 6" and surface casing is ready to UT test. If UT test is good then plan ta re-grout surFace csg x conductor
annulus.

A31-14LW (194-044) - Na visible exterior corrosion on conductar. Conductor UT readings showed scattered
c;orrosion extending from top to 44" down. Drilled holes in the thin spots of the conductor and found 3-4' void full
of water. Cut a 9" x 10" window in conductor appro~timately 54" from top. Surface casing looks good at this
depth. Void extends to approximately 5-6' from top of conductor. Chipped grout from top of conductor and found
1-1/4" x 8" hole in surtace casing at 20" from top. Cut window in conduc#or in-line with surface casing corrosian to
get a better look at the coROSion and 7-5/8" intermediate casing. The 7-5/8" intermediate casing looks goai.
Surtace casing corrosion appears to be limited to the top of the void area. Plan to repair surface casing by
welding fabrica#ed sleeve over area of corrosion then fill void with grout.
A32-11 R(193-092) - 3 small holes in conductor. UT readings showed conosion limited to tap 36" of conductor
with 0.160" minimum wali thickness remaining. Chipped grout down from surface of conductor and found small
holes through surface casing. Wa~ing on more UT numbers to determine e~ctent of surFace casing corrosion.
Plan to find exten# of coROSion, weld repair surFace casing and re-fill annulus with grout.
A42-14 (167-041) - No visible e~erior corrasion on conductor. UT readings showed scattered corrosion near
top with minimum wail thickness reading of 0.300". Grout was chip~d to 10" depth. UT readings on the surface
casing looked good with wa1! thickness readings ranging from 0.333"-0.350" (nominal wail thickness of 10-3/4"
surtace casing is 0.350"). Plan to re-fill with grout.
A44-11 (167-035) - Two holes found in conductor 15" fram top and 7" apart. UT readings show that c~rrasion
extends downward. At 36" from top minimum wall tt~ickness is 0.100". Grout was chipped down and found small
holes in surFace csg. Chipped down another 2 feet and found a void in the surfac~ casing x conductor annulus
that extends down +J-35'. This void was filled w/ water. Cut window lower in conductor and drained water.
Surface casing looks good at this depth. Plan to weld repair surface casing and fill conduotor with grout to
surtace.

In June, 2008 during conductor inspections requested by XTO Energy, I performed visual and
ultrasonic examinations on the 24" A31-14 conductor.
While there was no evidence of external corrosion to the conductor, upon UT examination there
were isolated patches of internal corrosion found. An inspection port was cut into the conductor
in order to inspect the 10" surface casing and a small hole was found in the casing. This area of
corrosion was scanned by UT and the corroded area of steel was removed. Inspection of the
7.625"intermediate casing was made through the cut-out and found to be free of corrosion. The
10" surface casing was repaired with a 46" sleeve and re-inspected and the conductor inspection
port was replaced and the conductor was then re-grouted and a plate seal-welded at the top to
prevent ingress of corrosive fluids.

RE: Sundry Well Operations Reports: Platform A Conductor & Surface Casing Repairs
Dear 1VIr. Norman,
Enclosed are Sundry Operations Reports (Form 10-404) for conductor and surface casing repairs recently
performed on XTO Energy Platform A in the Middle Ground Shoal Field of Cook Inlet, AK. All
conduc~or's on Platforms A& C were inspected using ultrasonic thickness measurements after production
personnel noticed a small area of bubbling paint and rust stains on the A32-11R conductor. Seven
conductors on Platform A were identified as having possible corrosion near the wellhead and flagged for
furth~r inspectimn. •The remaining conductors on Platform A and all conduciors on Flatform ~ appear to be
in good condition.
Grout that fills the annular space between the conductors and surface casings was chipped out on the seven
wells in question to determine the extent of corrosion. Some corrosion was found on the surface casings
and was repaired by welding patches or applying a composite material wrap. The annular space was refilled
with grout and a seal plate was welded to the top of the conductor to prevent corrosive materials from
entering at the surface.
Sundry i eports for four of the seven wells are included in this mailing. Of these four, only two, the A 11-12
and Al T-13RD2, required repairs to the surface casing. The A22-14LS and A42-14 were inspected with no
notable corrosion present. The Sundry Reports for the remaining three wells will be filed when repairs are
completed.
Please find attached the following information for your files:
1. Ultrasonic Thickness Inspectors Qualifications
2. Information on A+ pipe Wrap.
3. Photograph of Repaired and Seal Plated Conductor from Wellhead Deck

MGS FIELD WELLS
CHANGE WELL STATUS FOR WELLS WITH LATERALS
FEBRUARY 16, 2006
The following wells status was changed from SQZED to I-OIL. These
wells had laterals drilled against them and production was assigned to all
laterals in Cook Inlet when the wells were first drilled. Since then, AOGCC
changed the policy about lateral wells and assigning production against
lateral API numbers. This policy is like the lateral wells on the North
Slope. Today I notice wells producing in the MGS field with a status not
conducive to the well status and changed it to I-OIL. All the wells show oil
production over the years. The status date reverted back to the date the well
was completed as an oil well. The following wells were corrected:
MGS A12-01
MGS A13-01
MGS A14-01
MGS A22-14
MGS A23-01R
MGS A31-14
MGS A41-11
PERMIT 177-055
PERMIT 166-015
PERMIT 167-079
PERMIT 185-068
PERMIT 187-093
PERMIT 177 -031
PERMIT 185-174
All production has been assigned back to the original well API number, no
production is assigned to a lateral well API number.

Al3-01 Permit # 66-015 A41-11 Permit # 85-174
This letter reflects the lateral well that replaced the original well by well number, permit number and API number:
A12-01LN (permit # 94-120, API# 50-733-10081-62) permit was approved 9/29/94.
A13-01LN (permit # 94-105, API# 50-733-10079-61) permit was approved 8/05/94.
AI4-01LN (permit # 94-119, API# 50-733-20067-61) permit was approved 9/27/94.
A22-14LS (permit # 94-071, API# 50-733-10085-62) permit was approved 5/13/94.
A31-14LW (permit # 94-044, API# 50-733-20305-62) permit was approved 3/23/93.
A33-1 ILS (permit # 93-180, API# 50-733-20383-61) permit was approved 11/30/93.
A41-11LN (permit # 94-087, API# 50-733-20378-61) permit was approved 6/28/94.
The replacement well incorporates a lateral with the existing well bore. The lateral along with the original hole will be
used for production of hydrocarbons. A review of both well files indicates administrative abandonment (SQZED, AOGCC
status) of the originating well bore is appropriate according to current AOGCC procedures for wells where additional
permitted laterals are drilled from an existing well bore. Production will be reported to the original API wellbore (-00, -01
or etc) and not to the lateral API wellbore (-60, -61 or etc).
20 AAC 25.005(e) requires a Permit to Drill for each hole drilled below the conductor unless drilled to straighten the
original hole, sidetrack junk, or correct mechanical difficulties. In addition, AOGCC databases require each conductor
have only one permit number and API number. Administrative abandonment of the original permit only eliminates
duplication in the databases. Information for the original well and its replacement will be maintained in their respective
files until plugged and abandoned according to 20 AAC 25.105.
Please notify other cognizant personnel in your organization so that the well will be correctly identified on subsequent
reports and applications submitted to the Commission.

Status'
Well producing 430 BDO (30% cut) through the HK/HN, HN/HR
and HR/HZ zones.
Objective'
Clean out perforations in the HN/HR and HR/HZ zones using
acid placed with coiled tubing.
Procedure- Note' Check diesel and acid compatibility before job.
1. M.I.R.U. wireline and lubricator. Test lubricator to 3000 psi.
Run in with wireline and retrieve bull valve at 307'. P.O.O.H.
Make drift run with 2 1/4" tools to bottom (_+9547'). P.O.O.H.
and rig down wireline equipment.
2. Shut well in at surface-do not bleed casing pressure.
3. M.I.R.U. coiled tubing unit and .Dowell acidizing equipment.
Test tree and coiled tubing to 5000 psi. Test hydraulic system
and BOP.
4. Run tubing in the hole slowly (no more than 100 FPM). Tag
bottom (_+9547'). If fill is above bottom perforations at
9534', notify T. S. Burns (713) 870-4465 or (713) 980-4393
(home).
5. With coiled tubing at 9534', pump 205 gal. of 7% HC1 with 0.3%
W-35, 0.2% F-75N, and 1.0% A-200. MoWe coiled tubing uphole to
treat entire perforated interval (9534'-9260') while pumping.
6. R.I.H. tubing to 9534'. Pump 410 gal. of 7 1/2%-1-1/2%.
HC1-HF with 0.3% A-200. Move coiled tubing uphole to treat
entire perforated interval (9534'-9260') while pumping. Stop
coiled tubing at 9260'.
Note- Do not leave mud acid standing in wellbore.
7. Clear coiled tubing with 19 bbls of diesel. Pump an additional
15 bbls of diesel to displace acid into perfs.
8. P.O.O.H. and rig down. Leave tubing full of diesel (19 bbls)
pending further use of the coiled tubing unit.
9. Put A31-14 back on gas lift production. Test well for several
days until well is producing at stable rates. Report rates to
T. S. Burns in Houston. Return well to regular test rotation.

John Works, Shell Oil, called to report that they keep
losing mud in the hole to the formation. They do not
believe they can lay a cement plug across the perforatiOns~~
as previously planned. I gave him verbal approval to set
a bridge plug in the 7" liner above the perforations and
then lay a cement plug to at least 50' above the liner lap.
I asked that the cement be checked and the top noted in
their report.

Attention O. K. Gilbreth, Jr.
Gentlemen'
We should appreciate administrative approval for completion of
subject well in the "G" Pool as an exception to Rule l(b) and pursuant
to Rule 6(d) of Conservation Order No. 44. Wells No. A-32-14 and A-42-
14 are the two permitted "G" Pool producers in the NE/4, Sec. 14-8N-13W,
SM.
Our plan is to deepen idle Well No. AWS-1 which was drilled
and completed during 1967 as a salt water source well but has never been
used for that purpose. Form 10-401 and related data are attached as
required for a re-entry. Also attached is Form 10-403 for abandonment
of the original completion with a diagrammatical prognosis for squeezing
off the salt water beari~ng interval. A plat of the area is an additional
attachment for reference in this connection. Subject well and its
proposed subsurface location are shown together with the other wells and
well tracks in the vicinity.
The proposed well is located in an area of leaseholds owned in
undivided 1/3 interests by Shell, Chevron and Arco. As shown on the
attached plat, the location is approximately 2-1/4 miles from the nearest
common line with another producing lease operated by Amoco.
Justification for this well is based upon the recovery of
additional oil and gas reserves which would not otherwise be produced
within the economic life of the project. The additional drainage point
is designed to achieve a better balance in the operation and enhance
the pressure maintenance program.

Your early consideration will be appreciated to enable prompt use of
