Sorry it took a little time to find the correct person that was on tower 3/28/10.
The Nordic 2 tourpusher said, "The problem was a failing 3" shock hose on the kill side. We had found some
rubber at the shaker and after inspection of the 3" shock hose, it was determined to be shedding internally. We
then rigged down the 3" flexible kill hose and replaced it with 3" hardline with Graylock clamps. At no time have
we ever used threaded 1502 type connections in either our choke or kill lines."
The Nordic 2 WSL amended the openwells report to reflect that the hardline had graylock connections.

As I was reading the drilling report for Z-16 I noticed on 3/28/10 on the initial BOP rigup that the shell test had
mentioned a failed PT on the Kill line. It appears that it was replaced with a new section but that also failed.
Finally "hardline" was installed and passed a pressure test. This happened before the official witnessed BOP
test. Can you elaborate on the type of pipe used ... was it knock-up 1502 type connections? Regulations require
the kill line to be rigid pipe without hammer unions (20 AAC 25.285 (e) There is no mention of this in our BOP
report submitted by the inspector.

Enclosed please find multiple copies of a spreadsheet with a list of wells from GPB Z-
Pad that were treated with corrosion inhibitor in the surface casing by conductor
annulus. The corrosion inhibitor is engineered to prevent water from entering the
annular space and causing external corrosion that coufd result in a surface casing leak
to atmosphere. The attached spreadsheet represents the well name, API and PTD
numbers, top of cement depth prior to filling and volumes of corrosion inhibitor used in
each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10-404,
Report of Sundry Operations.

Sundry approval is requested for well Z-16. Z-16 is a conventional waterflood well drilled in 1992.
This well has suffered from abnormally low gross fluid rate and low PI even after acidizing and
reperforating. This well is no longer competitive for the limited gas lift in the area. After being shut in
from 2006 to 2008 it has seen minimal on time recently.
REASON FOR SIDETRACK
Given the low fluid rates and high water cut Z-16 is a good candidate for a coil sidetrack.
The proposed plan for Z-16A is to drill a through tubing sidetrack with the SLB/Nordic 1 Coiled
Tubing Drilling rig on or around October 1, 2009. The existing perforations will be isolated by a
cement downsqueeze after setting the whipstock, as well as the primary cement job. The sidetrack, Z-
16A, will exit the existing 5-1/2" liner and kick off in Shublik and drill an S shape well in Zone 4. The
following describes work planned and is submitted as an attachment to Form 10-403. A schematic

Saturday, April 21, 2001; I witnessed the three-month SVS retests on BPX Z pad
in the Westem Operating Area of the Prudhoe Bay Field.
Merv Liddelow performed the SVS tests today. The test results (one SSV Failure)
are noted on the accompanied SVS test spreadsheet. All well houses were in "OK"
condition.
This was the first experience I have had with the new SV control system on Z pad
and found it to operate quite well. The temperatures today were somewhat mild
(+15) and it remains. . to be ~seen how.. they will hold up dudng next winter's cold
season. Wells Z-5 and Z-8 had slow clOsing SSV's (+ 2 minutes), Merv called the
Automation crew and these were being worked on before I left the pad.
The pad is being used as a staging area for some pipeline fabrication and has
some equipment and material stored on it. This was kept for the most part neat and
orderly.
SUMMARY: I witnessed the three-month SVS retests on Z pad in the Prudhoe Bay
Field.

Friday, JanUary 19,.2001' I traveled to BPX's Z Pad in the Prudhoe Bay Field and
witnessed surface safety valve system'testing.
Merv Liddelow was the BPX representative in charge of testing. As the attached
AOGCC Safety_ Val.~e System Test Report indicates I witnessed the testing of 21
wells and 43 components with 6 failures. The pilot on well Z-03 failed to trip and the
pilot on well Z-08 was frozen and once thawed it passed a retest but the SSV on
this well had an unacceptable closure time (12-min.). The SSV on well Z~18 also
had an unacceptable closure time (15-min.) and the SSV's on wells Z-22B and Z-24
failed to pressure test. The high failure rate on Z Pad requires it be placed on a 90-
day test interval.
Mr. Liddlow indicated to me today that all failures have been repaired except the
slow SSV closure of wells Z-08A and Z-18. The hydraulic lines on these wells need
to be flushed and he has arranged for the maintenance crew to perform the work.
Wellhouse inspections were conducted on the 21 wells tested with no exceptions
noted.
Summary: I witnessed SVS testing at BPX's PBU Z Pad.
21 wells, 43 components, 6 failures. 13,9% failure rate.

Stimulation Operation
A HCL Treatment was performed to remove scale using the
following fluids:
OPERATIONS SUMMARY
Run in hole with tubing end locator and 2.5" combo nozzle.
Locate tubing tail. Run in and tag fill @ 11411. Wash54 4 ft of
hard fill (1.5 hour), then could not make more progress. Pumped
5 bbl 15% acid and broke though. Cleaned out fill to 11460 with
biozan & sea wtr with gas lift. Noticed tight spot @ 11418 that
wasn't jetting away. Jetted perfs with 19 bbl 15% HCL. Let acid
soak. Concentrated jetting on lower set of perfs where tight spot
was. Let acid soak for 30 min. Tight spot gradually disappeared so
it must have been scale buildup in casing. Flushed biozan, solids,
and acid out of hole with filtered sea wtr while tripping out.
Picked up additional riser and Baker SST tool string.pressure test
all eqpt. 4000 psi pump 100bbl filtered sea water dn. tbg. (well
dead.) Test set SST above perfs & worked spotting valve. Tried to
reset SST @ 1st set point-would not inflate. Picked up to tail pipe
to wratchet FCV. Ran back in. Still could not set. Ran back to tail
and wratchted again and successfully set SST below tail. Ran back
to Set #1 and set SST. Inj into formation @ .3 bpm 3000 psi.
Opened spotting valve and spotted fluids to SST. Closed spotting
valve and pumped acid as per program max rate was 0.70 BPM at
3190 psi. Moved up the hole to 11,398 and set packer. Spotted
fluids and commenced injection. Injection rste was brought up
to 1.0 BPM. After 27 bbls of acid out, the pressure dropped on the
packer side and increased on the annulus. Unable to release
packer. Sheared disc at 5,000 psi and POOH. ALL OF THE TOOL
STRING BELOW THE EMERGENCY DISCONNECT LEFT IN HOLE W/3" GS

Dear Ms Thomas:
Enclosed is the approved application for permit to drill the
above referenced well.
The permit to drill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and
does not authorize conducting drilling operations until all
other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance
with 20 AAC 25.035. Sufficient notice (approximately 24 hours)
of the BOPE test performed before drilling below the surface
casing shoe must be given so that a representative of the
Commission may witness, the test. Notice may be given by

Install and cement 80' of 20" conductor to surface.

The Drilling fluid program will consist of an 8.5 - 10.2 ppg spud mud in the 13-
1/2" hole, a 9.5 - 10.5 ppg water base mud in the 9-7/8" hole and a 9.3 ppg
water based mud in the 6-3/4" hole.
Drill 13-1/2" hole to 3000'. Run and cement 10-3/4", 45.5 #,'NT-80LHE Buttress
casing to surface.
Install blowout prevention equipment consisting of 5000 psi W.P. pipe rams,
blind rams, and annular preventer. Maximum possible bottomhole pressure is
4390 psi at 8800' TVDss based on original reservoir pressure. Maximum
possible surface pressure is 3330 psi based on a gas column to surface from
8600' TVDss and an oil column to 8800' TVDss.
A formation leak-off test will be performed 10' below the 10-3/4" shoe.
Directionally drill 9-7/8" hole to the 7-5/8" casing point above the top of Sag
River Sandstone. Run a 7-5/8" casing string of 29.7# NT-95HS NSCC to within
10' of 9-7/8" TD and cement with Class G cement around the 7-5/8" shoe. The
annulus will be freeze protected by dry crude.
Directionally drill the 6-3/4" hole to TD. Run 5-1/2" liner from TD to 250' inside
7-5/8" casing shoe and cement using Class G cement.
Clean out to 5-1/2" float equipment. Change over to NaCI completion fluid.
Test casing and lap to 3500 psi.
Run 7-5/8" production packer on tubing. Set packer 50' above top of liner. A
subsurface safety valve with dual control lines will be placed 100' below base of
permafrost. Install and test Xmas tree. Displace tubing to diesel. Set and test
packer.
The well will be perforated after the rig has been released.
Fluids incidental to the drilling of a well will be pumped down the 7-5/8" x 10-
3/4" annulus of the well.

4-1/2", 12.6# L-80, TDS tubing with 5 GLM's.
