Chris,
KRU 1D-107 (PTD 200-177) was last reported 7/14/2018 after the well had failed a shut in non -witnessed 4 -year
MITIA. Subsequent the report Coiled tubing removed an ice plug from the tubing, a tubing plug was set, the well bore
was circ'd out and a diagnostic MITIA passed on 8/30/2018. It is believed that all the integrity issues were resolved at
that time, but the well remained shut in non -integrity reasons. The Production group now wants to bring the well back
online to test interactions with 1D-143. The plan is to bring the well online on water injection and perform a witnessed
MITIA once the well has stabilized. Additionally the well will be monitored for 30 days to ensure no further signs of
AnnComm are observed. A wellbore schematic, and 10-426 of the passing MITIA are attached. The 90 day TIO is also
attached, but the well has been shut in since 2/12/2017 so it is essentially blank.
Please let us know if you disagree or have any questions with this plan.

Jan Byrne

Dear Mr. Maunder:
........ . .
Enclosed please find a spreadsheet with a list of wells from the Kuparuk field eKRD). Some of
these wells were found to have a void in the conductor by surface casing annulus. The voids that
were greater than 3 feet were first filled with cement to a level of approx. 1.0 foot. The
remaining volume of annular void subsequently was filled with a corrosion inhibiter engineered
to prevent water from entering the annular space. As per previous agreement with the AOGCC,
this letter and spreadsheet serves as notification that the treatments took place and meets the
requirements of form 10-404, Report of Sundry Operations.
Please M.J. Loveland at 907-659-7043, if you have any questions.

Dear Mr. Trantham:
Enclosed is the approved application for permit to drill the above referenced well.
The permit to drill does not exempt you from obtaining additional permits required by law from other
governmental agencies, and does not authorize conducting drilling operations until all other required
permitting determinations are made.
Annular disposal has not been requested as part of the Permit to Drill application for Kupamk River Unit
Well No. ID-107. Please note that any disposal of fluids generated from this drilling operation which are
pumped down the surface/production casing annulus of a well approved for annular disposal on Drill Site
1D must comply with the requirements and limitations of 20 AAC 25.080. Approval of this permit to
drill does not authorize disposal of drilling waste in a volume greater than 35,'000 barrels through the
annular space of a single well or to use that well for disposal purposes for a period longer than one year.
The blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25.035; and the
mechanical integrity (MI) of the injection wells must be demonstrated under 20 AAC 25.412 and 20 AAC
25.030(g)(3). Sufficient notice (approximately 24 hours) of the MI test before operation, and of the
BOPE test performed before drilling below the surface casing shoe, must be given so that a representative
of the Commission may witness the tests. Notice may be given by contacting the Commission petroleum
field inspector on the North Slope pager at 659-3607.

Subject: Help! Need expediting for Permit!
Hi Tom,
I've just returned from vacation to find two things: 1) Anticipated trouble
time with Nordic 3's operations did not materialize, and 2) My associate, Mark
Chambers, is out sick following surgery.
Consequently, operations on Rig 3 have speedily progressed to the point that the
rig could be ready to spud West Sak injector well 1D-107 sometime early tomorrow
morning. And Mark's absence has led to a failure on our part to properly
shepherd our permit application for 1D-107.
I am hoping that you can assist us, if possible, by helping to expedite the
review and approval of our permit application for this well, hopefully, by the
close of business today.
My apologies for this inconvenience. Please call me at 265-6826 if you think it
will be impossible to issue the permit today, so that I can make other plans.
Thanks in advance for your understanding of this situtation.
Kindly,
Steve McKeever

Dear Sire:
Phillips Alaska, Inc. hereby applies for a Permit to Drill a development well to the West Sak 1 D-107.
The well will be drilled and completed using Nordic Rig 3. Please utilize documents on file for this rig.
Subject to AOGCC approval, fluids and cuttings generated from drilling the well will be pumped down an
approved and existing annulus on Drillsite 1D, or hauled to an approved Prudhoe or GKA Class II disposal
well.
Also note that Phillips Alaska does not anticipate the presence of H~S in the formations to be encountered
in this well. However, there will be H~S and combustible gas monitoring equipment functioning on the rig as
is the standard operating procedure in the Kuparuk River Unit drilling operation.

Reservoir Pressure
Based on actual well test data from the nearby WS1-01 well, expected reservoir pressures in the West Sak sands in
the 1D pad area vary from 0.43 to 0.46 psi/il, or 8.2 to 8.8 ppg EMW (equivalent mud weight), depending on the
TVD of the sand layer being evaluated. Assuming a 100 psi overbalance is required to safely drill and trip in this
formation, then a required mud weight of 9.3 ppg can be predicted as a maximum mud weight required to safely drill
this formation, assuming a TVD of 3,653'.
Experience in the area has proven that these zones can be safely drilled using only a diverter system for well control
purposes. The 'rig diverter system will be filled with water or mud and function tested prior to spudding the well. Since
the combination surface / production casing will not be drilled out, leakoff test and HASP calculations are not
necessary for drilling ahead. However, HASP calculations for cased hole operations a~e in order.
Calculating HASP based on the highest recorded reservoir pressure from the WS1-01 well, and assuming a gas
gradient of 0.11 psi/il from the West Sak base to surface, the following HASP can be computed:
HASP =(4084 il)(0.46 - 0.11 psi/il) · =1429 psi
Based on this calculation for HASP, it will only be necessary to test the BOP equipment, ailer installation, to a
maximum of 3,000 psi.. The 5-1/2" 15.5# L-80 BTC casing to be used has a burst rating of 7,000 psi; 70 % of this
burst rating is 4,900 psi. Ailer testing, a 3-1/2" tubing tieback string will be installed in the well to be used as the
injection string.
Drillinq Area Risks:
Drilling risks in the West Sak 1D-107 area are limited to lost circulation and potential borehole in~ty in and below
the permafrost formation. Lost circulation will be addressed with lost circulation mater~ (LCM) and reduced
circulating rates as needed. Based on previous Drill Site 1D drilling results, 9.4 ppg mud should be more than
adequate to control both the Ugnu and West Sak formation pressures without resulting in lost circulation.
Close Crossing and Offset Well Znterception Risks
Positional uncertainty calculations were performed on the proposed 1D-107 wellbore using worst case 3D elliptical
error models. Based on these calculations the following wells have center lines which may lie less than 30 feet from
the centerline of the proposed well bore. For more information see attached Sperry Anti-collision Report.

Annular Pumping Activities:
Incidental fluids generated from drilling operations will be hauled to the nearest, permitted disposal well or
will be pumped down the surface / production casing annulus of a permi~-~d-existing well on DS-1 [~. That
annulus will be left with a non-freezing fluid during any extended shut down (> 4 hr) in pumping operations.
The West Sak 1D-107 surface casing / production tubing annulus will be filled with diesel and/or non-
freezing brine after setting the 3-1/2" tubing tieback string and prior to the drilling rig moving off of the well.
