END 4-32 CSG Repair Summary
3/1: Found bubbles coming from the conductor and determined it was a casing leak. Shut in well. Bled
IA to zero
3/2: Fluid packed OA to try and determine where leak may be at. Started seeing more bubbles in
conductor after pumping about 85 gallons of diesel in OA. Pumped total of 95 gallons in OA and
pressured up to about 200psi. Did not hold pressure and pressure dropped after about 5-10 mins.
Possible leak around 3-5 feet?
3/3: Pulled VR plug on back side of OA and connected N2 and pushed any reaming fluid out front side of
OA. Then pulled back side off of OA and lowered string down to try and find fluid level. Was only able to
lower about 10” to 12” due to muddy grease like substance.
3 /4 : Removed front OA valve and tried to lower string to check fluid level and was packed with mud.
Hooked up diesel from back side to try and determine fluid level by calculating the volume of diesel to
fluid pack OA. Took about 16 gallons and pressured up to 100psi and started seeing fluid bubbling from
conductor. Total of 28 gallons pumped.
Safed out well for well house and flowline removal.
Maintenance started disconnecting electrical and instrumentation to prep for wellhouse removal and
flowline removal.
3/5: Slickline showed up and rigged up and pulled SSSV and set plug and bled tubing to zero.
3/6: Pulled wellhouse, flowline and G/L line. Well is blinded at wing valve.
3/7: NES removed flooring and started snow removal behind well. Removable planks covering well
cellar.
3/8: Pushed N2 from back of OA to front OA for about 10 mins and got nothing but N2 coming though
OA. Took back side of OA off and dropped a bobbie into the hole to try and see the fluid level. Bobbie
went down about 12” and hit fluid. Also pulled cap off cement return to verify there was no fluid in
there and it was full of cement.
3/12: T/IA/OA= 0/0/0. BPV installed
3/13: Cellar floor cut out & removed
3/14: Excavated cellar to 88” below OA vlv & installed temporary shoring wall.
3/15: Excavated to total depth of 112” below OA vlv & lowered temporary shoring wall.
Wind conditions did not allow for use of crane to move well support structure.
3/16: Crane to arrive today to move the well support structure.
3-17: Will be hooking up the electrical to the control shack & installing cross brace on WSS to the swivel
on wellhead.
3-18: We cut circumferential cuts on conductor. As we were finishing up those cuts the cuts started
closing up. Added more pull, up to 42,000 lb. Re-evaluate this morning and start on vertical cuts on
conductor. Late start yesterday due to gen-set issues. We are now on shore power.
3-19: Removed conductor and left landing ring in place. All cement chipped off. Did not find the top of
the leak. Saw bubbles coming up thru gravel. Determined we will have to excavate another 4 feet or so
along with cutting of more conductor to get to the bottom of the leak
3-20: Excavated another three feet, total of 15.8 feet. Encountered a lot of water after 1.5 feet.
Working water issues w/vac truck, shut down for night, cut casing and chip cement in AM.
3-21: Cleaned hole out from buffing and chipping, Mistras inspected, drew on grid, pipe looked good for
5 foot patch, working water issues. Plan forward: State inspection this am. Next step weld wedding
bands.
3-22: AOGCC came by and did his inspection at 9am and then we vacuum out fluid from OA and then
started welding wedding bands on. Should finish welding wedding bands this morning.
3-23: Finished welding wedding bands on and inspection done 24 hr timer started at 10:30am will start
welding clam shell on after 24 hr wait is done and inspection is done.
3-24: Welded on External Clamshell
3/25/2021: Pre MIT done blue goo and rapped and started fitting up conductor
3/26/2021: Finished fitting up conductor and welded conductor. Plan forward: State MIT and then back
fill hole.
3/27/22021: State MIT passed and filled hole with rock
3/28/2021: Filled conductor with cement left 6 to 12 in to add blue goo and removed cage and cross
from well support. Put shipping bolts in to be ready for crane to lift off of well.

Surface Casing Leak
DIU SDI 4-32
Hilcorp Alaska LLC
PTD 1950690; Sundry 321-109
3/22/2021: 1 traveled out to Endicott Satellite Drilling Island to see the surface casing
damage and leak on DIU SDI 4-32.
- Refer to report dated 3/2/2021 from AOGCC Inspector Matt Herrera for details
about the observed leak at surface.
I arrived on location and met with Larry McBride and John Allan (both Hilcorp). There
was a giant surface gantry lift rigged up using the tubing head as the connection point.
The well had been excavated and the conductor cut away exposing the surface casing.
Inspection revealed extensive corrosion and two leaks 14 feet down from surface.
Hilcorp's plan forward was to weld on a 4-1/2 ft long sleeve around the damaged area.
The operation looked very good and all well -organized.

Wellhead Leak
Duck Island Unit SDI 4-32
PTD 1950690
3/2/2021: On 3/2/2021 1 traveled to Hilcorp's Endicott Satellite Drilling Island to
investigate a wellhead leak found by Operations during wellhouse checks. I met with
Drillsite Operator John Houser and discussed what was found at Well 4-32 and their
plan forward to address the issue. The cellar was dry and clean with no debris. The
wellhead had what looked like calcium stains but no obvious leaks from piping or
flanges. The casing head flutes had water in them and that was where the bubbling was
coming from. Mr. Houser could not confirm if any LEL readings were taken. The
bubbling was slight — maybe once every minute and a half; nothing to warrant or
suggest an explosive or unsafe atmospheric condition. Operations suspects a surface
casing leak and their plan forward is to have Slickline set a tubing plug and conduct
MIT -T, MIT -IA and MIT -OA to try and determine which casing string is leaking and
where communication is coming from. Well is currently shut in and is being monitored.

Re: Duck Island Unit Well Renaming Request SCANNED SEP 0 12016
Dear Commissioner Foerster,
Hilcorp Alaska, LLC (HAK) requests a change to the naming convention of Duck Island
Unit wells to remove the unused suffixes at the end of each well name. Well names
currently include a forward slash "/" followed by an obsolete bottom hole location
designator. HAK would like to retain the well number as well as the "A, B, C...." sidetrack
designations but remove the "/suffix". For example, "DUCK IS UNIT SDI 1-09A/L21"
would become "DUCK IS UNIT SDI 1-09A". The attached spreadsheet includes original
and new names for all Duck Island wells.
While performing the review of Duck Island well names, HAK found a single well, "DUCK
IS UNIT SDI 4-20/M35" (PTD# 1861730), that should also be modified to include an "A"
label to reflect the fact that the well is a sidetrack from the original "DUCK IS UNIT SDI
4-20/T34" (PTD # 1861500). The original wellbore, abandoned 10/13/1986, was
sidetracked to a new bottom hole location completed on 9/9/1987. This well appears to
be one of the first sidetracks performed at Endicott. While the bottom hole code was
changed, no letter was added to reflect the sidetrack. "DUCK IS UNIT SDI 4-20A" is also
included on the attached spreadsheet.
Sincerely,
Wyatt Rivard

Subject: Corrosion Inhibitor Treatments of Endicott Row 4
Dear Mr. Regg,
Enclosed please find multiple copies of a spreadsheet with a list of wells from Endicott
Row 4 that were treated with corrosion inhibitor in the surface casing by conductor
annulus. The corrosion inhibitor is engineered to prevent water from entering the
annular space and causing external corrosion that could result in a surface casing leak
to atmosphere. The attached spreadsheet represents the well name, API and PTD
numbers, top of cement depth prior to filling and volumes of corrosion inhibitor used in
each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10 -404,
Report of Sundry Operations.
If you require any additional information, please contact me or my alternate, Gerald
Murphy, at 659 -5102.
Sincerely,
Mehreen Vazir

Leak-OffTests at Endicott
DATE' 21 Oct 96
There is a possibility that some leak-off tests within the Endicott Field may not have been
properly conducted. ~ ~'5 ~ [~ci
Specifically, leak-off tests ofi 1-3~, 3-37 and 4-32. Commissioner Johnston and I have
discussed this matter and re'qued2 that you initiate an investigation to determine whether
these tests were properly conducted under our standard of good oilfield engineering
practices.
Please feel free to rely on Mr. Wondzell and the inspectors for assistance. The allegations
are serious and your investigation should be conducted with deliberate speed and
tempered by discretion.
Commissioner Johnston and I will assist you in any way possible.

Dear Mr. Clark:
Enclosed is the approved application for permit to drill the above
referenced well.
The permit to drill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and does not
authorize conducting drilling operations until all other required
permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with
20 AAC 25.035. Sufficient notice (approximately 24 hours) of the BOPE
test performed before drilling below the surface casing shoe must be
given so that a representative of the Commission may witness the test.
Notice may be given by contacting the Commission petroleum field

LOGS WITNESSED BY: P. BARKER & E. VAUGHN.
TD NOT LOGGED DUE TO POSSIBLE HOLE PROBLEMS.
DENSITY-NEUTRON COMBINATION ATTEMPTED WITH DIFL BUT
UNABLE TO OBTAIN LOGS DUE TO MUDBALLING OF PAD
TOOLS. (RAN SEPARATELY)
PER CLIENT'S REQUEST, BOREHOLE & TORNADO CHART CORREC-
TIONS WERE APPLIED TO THE DIL DATA. THE DIL DATA WAS
CORRECTED FOR BOREHOLE SIZE (9.875"). THE CORRECTED
DATA WAS THEN USED IN THE ATLAS TORNADO CHART TO GET
"RT" & "DI" CURVES.
RT = TRUE FORMATION RESISTIVITY.
DI = DEPTH OF INVASION.
CALIPER DATA WAS NOT USED FOR DIL BOREHOLE CORRECTIONS
BECAUSE THE CALIPER READ UNDER GAUGE THROUGH MUCH OF
THE LOGGED INTERVAL DUE TO MUD BUILD UP ON THE TOOL.
NEUTRON LOG PARAMETERS: SANDSTONE MATRIX,
CALIPER ~ .~RECTED,
CHISM FILTERING IS ACTIVE,
CASING CORRECTIONS OFF,
SALINITY = 0.00 PPM.
