Issuance of Permanent Closure Approval for Inactive Reserve Pits located at nine exploration
sites located on the North Slope, SouthCentral, and Interior Alaska
Dear Ms. Pekich:

The Alaska Department of Environmental Conservation (ADEc) has completed review and public notice for nine
inactive reserve pit closure applications and requests for permanent closure approval. The closure applications provided
the required information describing the history of the reserve pit(s), the present condition of the reserve pits(s), results
of any required water sampling and an assessment of the potential risks posed by the drilling waste to human health and
the environment. At these locations, the'reserve pits have been backfilled and there is no exposed drilling waste. ADEC
coordinated review of the closure requests with the appropriate land manager(s).
Public notice requesting comments on the closure requests was published August 3 and 4, 2001 in the Anchorage Daily
News and posted on the State Web Site August 2, 2001.. No public comments were received in response to this public
notice. ADEC has determined that the reserve pits at these sites meet the closure requirements of 18 AAC 60.440.
Closure Approval
Under authority of 18 AAC 60.440 (j), ADEC grants permanent closure approval to Phillips Alaska 'Inc. for the inactive
reserve pits at the following nine exploration sites:

Terms and Conditions
This final closure approval is subject to the following terms and conditions:
In' accordance with 18 AAC 60.440(1), the Departm~ ent will require additional investigation, assessment,
monitoring or remediation if new information regarding conditions at the reserve pit facilities indicates that
further actions are necessary to protect human health or the environment.
2)
The approval granted by this letter is for the inactive drilling waste reserve pit(s) only. Closure for the pad as
a whole (if required) must be coordinated between the owner/operator and the appropriate land owner/manager.
Any person who disagrees with any portion of this decision may request an adjudicatory hearing in accordance with 18
AAC 15.200-310. The request should be mailed to the Commissioner of the Department of Environmental Conservation,
555 Cordova Ave. Anchorage, AK 99501. If a hearing is not requested within thirty (30) days of the date of this letter,
the right of appeal is waived. If an adjudicatory hearing is requested and granted, this decision remains in full effect
during the adjudicatory process.

a dark, lemon gold on core and a dull,

mostly light but with rare darker mica;
current bedding in sandy zone with fore-
set beds; one small piece of core had
paper thin calcite filled fractures and
one calcite surface all with yellow

scattered pinpoint bubbles formed on

On January 31, 1961, recovered balance of fish.
Directionally drilled 8~" hole, taking 7-5/8" spot cores, from 11531' to 121~5', the
total depth, which was reached on Febr~ 17, 1961. On February 18, 1961~ ran
electric log recording from 12140' - 11232'. Attempted to run Sonic log but unable

INSTRUCTIONS
READ CAREFULLY AND COMPLY FULLY
For the purpose of this determination attach hereto a neat, accurate plat, map or sketch of this lease, section, block
or lot locating thereon the proposed site for this location. Plat shall be drawn to a scale which will permit the facile
observation of all pertinent data. Show distances of the proposed well from the two nearest lease and section lines, and
from the nearest wells on the same lease completed in or drilling to the same reservoir. If the location requested is not in
conformance with the applicable well.spacing rules, show all off-setting wells to the proposed well, and the names and
addresses of all adjoining lease or property owners.
In event plat is fried for the purpose of designating the drilling and producing unit, or proration unit, on which the
proposed well is to be drilled~ the boundaries of such unit shall be shown, also the boundaries of all other such units
attributed to other wells on the same lease completed in or drilling to the same reservoir. The acreage contained within
each unit shall also be shown.

Do not confuse survey lines with lease lines. ~ne~ sketch or plat ,should show your entire lease if possible. If it is
not practical to show the entire lease and the plat shows only a section, block or lot out of your lease, you should clearly
show tha~ same is only a part of the lease.
Designate scale to which plat or sketch is drawn. Also designate northerly direction on the sketch or plat.
