Item 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the
data pertinent to such interval.

the following fluids:

SUMMARY
MIRU Sqz unit, Nowcam #4906, BJ, and Peak. PU tools
and PT. Recovered Meth. Killed well and RIH. Tagged
TD, corrected depth, PU 10' and flagged. Laid in CMT
from flag. WOC. RIH and tagged at 10,131'. Mixed
second batch and laid it in from CMT top. WOC. RIH, and
tagged hard CMT at 9934'. Mixed third batch. RIH and

PLUG-BACK SUMMARY PROCEDURE
To prepare for the X-13 A redrill, the following actions will need to be taken prior to
the rig move:
1. Notify an AOGCC representative prior to commencing plug-back operations.
.
Run in with coil tubing while circulating seawater. Lay a cement plug starting
from PBTD. Squeeze down and leave the top of cement at least 100' above the
top perforation but below the packer. Therefore, the top of cement should be
somewhere between 9913' and 9252' MD.
3. With AOGCC representative on location to witness operation, run in with
slickline and confirm that top of cement is at least 100' above the top per[oration.
3a. If the cement is tagged more than 100' below the tubing tail, set a magna-range
plug in the tubing tail to prevent future fluid swap-out when laying a kick-off plug.
4. Run in with a jet / chemical cutter on E-line and cut the tubing in the first full
above the packer PBR based upon the original completion tubing tally.
5. Circulate well over to seawater and freeze protect well with diesel to 2000'MD.
6. Bleed any pressure from the tubing, inner annulus and outer annulus to zero.
7. Install a back-pressure valve. Remove the well house and fiowlines. Level the
pad and dig out the cellar. Prepare the pad for arrival of the rig.

3 3/8" carrier guns at 6 spf:
10013'- 10038' MD BKB (Ref. BHCS, 9/15/80)
A lubricator was installed and tested to 3500 psi for all well work. The well was returned
to production immediately following perforating operations, then tested.

MUD ACID SUMMARY
A bullheaded two-stage mud acid treatment was performed June 17, 1986
on the following perforated intervals in Well X-13.
10116' - 10136' MD BKB
10140' - 10148' MD BKB
10158' - 10173' MD BKB
10184' - 10194' MD BKB
The treatment consisted of pumping the following:
a. 1350 gal. 15% HC1
b. 2700 gal. 12% HCl - 3% HF
c. 2700 gal, 3% NH4Cl water

b. 2700 gal. 12% HCl - 3% HF

e. Repeat steps a through d for a total of two stages,

Permafrost II cement. Installed and tested BOPE. Cleaned out to 2631', tested
casing to 3000 psi, okay. Drilled out to 2723', ran leak-off test to 0.72 psi/ft.

Buttress casing to 9826'. Cemented in two stages: First stage with 1400 cu.ft.

Permafrost C cement preceded by Arctic Pack. Cleaned out to 9783', tested casing
to 3000 psi, okay. Directionally drilled 8 1/2" hole to_10990'. Ran open hole
logs. Ran 40 jts. 7" 29# L-80 Buttress casing to 10886'. Cemented with 548 cu.ft.
Class G cement. Cleaned out to 10833', tested casing to 3000 psi, okay. Ran
inflow test on 7" liner lap - no inflow. Tested lap to 3000 psi, okay. Changed
well over to NaC1. Retested lap to 3000 psi, okay. Ran CBL. Ran 220 jts. 4 1/2"
IPC tubing to 9357'. Landed tubing with ball valve at~. Installed and tested
tree. Displaced tubing to diesel, set packer at 9252'. Tested tubing to 3500 psi,
okay. Released rig at 1300 hours, September 22, 1980.

open hole logs. Ran 40 jts. 7" 29# L-80 Buttress casing to 10886'. Cemented
with 548 cu.ft. Class G cement. Cleaned out to 10833', tested casing to 3000
psi, okay. Ran inflow .test on 7" liner lap - no inflow. Tested lap to 3000
psi, okay. Changed well over. to NaCi. Retested lap to 3000 psi, okay. Ran
CBL. Ran 220 jts. 4 1/2" IPC tubing to 9357'. Landed tubing with ball valve
at 2116'. Installed and tested tree. Displaced tubing to diesel, set packer
at 9252'. Tested tubing to 3500 psi, okay. Released rig at 1300 hours,

classified as important for the spawning or migration of
anadromous fish. Operations in these areas are subject to AS
16.50.870 a/Id the regulations promulgated thereunder (Title 5,
Alaska A~ministrative C~e). Prior to commencing operations
you may be .contacted by the Habitat Coordinator's office,
Department of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
chapter 3, Article 7 and the regulations promulgated thereunder
(Title 18, Alaska A~ministrative Code, Chapter 70 ) and by the

Dry bucket 36" hole to 80' below ground level and cement 30" x 20" insulated
conductor to surface using 11 cu.yds. Arcticset concrete.
Drill 17 1/2" hole to 2700" and cement 13 3/8" 72# L-80 Buttress casing to
surface using 3860 cu. ft. Penm3frost II cement.
Blowout Preventer Program consists of 5000 psi W.P. pipe rams, blind rams,
and annular preventer frc~ 13 3/8" casing point to T.D.
Directionally drill 12 1/4" hole to 8728'+ TVD. Run 9 5/8" 475 L-80 Buttress
casing to 8728' TVD and cement first stage with 1393 cu. ft. cement. Cement
second stage through 'D.V. packer collar at 2600'+ with 130 cu. ft. Permafrost C
cement. 9 5/8" casing will be hung at surface, the 9 5/8" x 13 3/8" annulus
will be Arctic Packed.
Directionally drill 8 1/2" hole to T.D. at 9602 '+ TVD. Cement 7" liner to
500' inSide 9 5/8" casing shoe with 374 cu. ft. C~ass G cement with 18% salt.
Run 4 1/2" 12.55 L-80 tubing with a production packer assembly at 8277 '+ TVD.
A subsurface safety valve with dual control lines will be placed at 211~'+.
Displace tubing to diesel.
Install and test Xmas tree'
Suspend well for perforating subsequent to rig release. Details of proposed
perforations to be ~tted on Sundry Notice to follow.
