Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.

Pulled BPV. Accepted rig at 2200 hrs on 6/4/89.
Killed well with 9.8 ppg brine.
ND tree. NU & tested BOP's to 5000 psi.
Pulled 4-1/2" tubing and PE straddle packer patch.
Pulled 7" heater string.
RIH to retrieve 9-5/8" packer-tailpipe assembly.
ND BOP's and tubing spool. Replaced 9-5/8" packoff.
RIH w/ 8-1/2" packer mill. (Tight at 3350') RIH w/ 8-1/4" swage to
3535'. RIH w/ 8-1/2" swage. Swage through tight spots @ 3350' -
3356', 3485' - 3486' and 3500'- 3335'.
10. RIH w/ 8-1/2" packer mill. Bottom hole assembly parted at top of
boot basket while milling. RIH with spear assembly to recover fish.
11. RIH w/ 8-1/2" bit and 8-1/2" string mill.
12. Made 6 unsuccessful attempts to recover fish.
13. RIH w/ 8-1/2" mill with 3 boot baskets. Milled from 3332' - 3682'.
14. Washed to 3800'. Unable to get back to 3800'. POH to 3515'.
Circulated. POH. Stuck at 3202'. Jarred free. POH. Boot baskets full.
15. RIH 9-5/8" RTTS. Unable to get below 3273', so set at 3273'. Tested
9-5/8" casing to 2500 psi - ok.
16. RIH 8-1/2" bit and 9-5/8" casing scraper and washed from 3252' to
3450'. POH.
17. RIH with 9-5/8" RTTS. Unable to work below 3294'. Cleaned out to
3361'.
18. Decision was made to abandon well.
19. RIH 9-5/8" bridge plug and set at 3272'. Layed 60 sks plug. POH.
20 RIH w/ 8-1/2" bit to tag cement top. /~
21. Tagged cement top @ 3104' with 25,000//. AAOGC waived witness of

Moved rig in. Killed well. Installed 2-way check. Nippled down tree.
Installed and tested BOPE to 5000 psig.
Pulled 4~" tubing.
Rigged up wireline. RIH and set plug in 4~" X-nipple at 3694'. Attempted to pressure
test packer. Packer would not test.
RIH with seal assembly on 3~" DP and stabbed into PBR. Tested DP x 9 5/8" annulus to
2500 psig.-Pulled out of PBR. Re-entered PBR. Retested 3!~" x 9 5/8" annulus to 2500
psig, okay. Pulled out of PBR. Observed well. POH with seal ass'embly.
Rigged up wireline. RIH and retrieved X plug. Rigged down wireline. Cleaned out to TDI.
Ran new 9 5/8" packer with tailpipe seal assembly. Stabbed into original packer PBR.
Set new packer at 3535'.
Ran 7" heating string to 2209'.
Ran new seal assembly and new 4½" tubing. Stabbed into new PBR.
Installed 2-way check. Nippled down BOPE. Nippled up and tested tree to 5000 psig.
Established injection rate. Tested annulus and packer to 2500 psig. Ran and set
X plug. Tested tubing to 3500 psig. Pulled X plug. Set BPV.

4. Rig up wireline. RIH and set plug in 4%" x-nipple at 3,694'.
5. Pressure test 9 5/8" casing and 9 5/8" gravel-pack packer to 2,500 psig. /lnc~i0ra~je
6. Pull plug from 4½" x-nipple. Rig down wireline.
7. RIH with new seal assembly on drill pipe. Stab into packer and pressure test
'. D.P. x 9 5/8" annulus to 2,500 psig. Monitor D.P. for conm~unication.
8. Establish injection down drill pipe with clean brine. .~.~nitor D.P. x 9 5/8"
annulus for con~unication. ,
9. Based on results of pressure tests'in Steps #5, 7 and 8; complete the well
with a new seal assembly and if required a new 9 5/8" packer (set above present
packer) and 4½" IPC tubing to surface.
10. Install BPV. Remove BOPE. Install and test 4½" tree to 5.000 psig.

SPudded well at 1800 hours, July 19, 1982. Drilled 17 1/2" hole to 2700'.
Ran 65 its. 13 3/8" 72# L-80 Buttress casing to 2689'. Cemented with 3720
cu.ft. Arcticset II cement. Installed and tested BOPE. Cleaned out to
2687'. Tested casing to 3000 psi, okay. Drilled out to 2700'; ran leak-
off test to 0.7 psi/ft, gradient. Drilled 12 1/4" hole to 3893'. Ran 95
its. 9 5/8" 47# L-80 Buttress casing to 3893'. Cemented in two stages:
First stage with 420 cu.ft. Class G cement; second stage through D.V.
packer at~ 2605' with~ cu.ft. Arcticset I cement preceded by Arctic Pack.
Cleaned out'D~.v.; tested casing to 3000 p~i, okay. Cleaned out float
equipment and drilled 8 1/2" hole to 4115'. Ran open hole logs. Under-
reamed 8 1/2" hole to 15". Ran screen assembly and 4 1/2" liner to 4112'.~
Set packer at 3634'. Pumped.39,000 lbs. 20-40 gravel pack. Washpipe
twisted off While attempting to reverse circulate; fished and recovered.
Pumped an additional 12 bbls. of' 20-40 gravel pack before achieving a
screen-out at 1500 psi. Ran 57 jts. 7" 26# L-80 IJ4S heater string to
2209'. Ran 116 its. 4 1/2" 12.6# L-80'A/B modified Buttress tubing with
seal assembly to 3644'. Installed and tested tree. Displaced 9 5/8" x
7" x 4 1/2" annulus to 60/40 glycol. 9 5/8" x 7" annulus and 4 1/2"
tubing is filled with 9.6 ppg. NaC1. Released rig at 0900 hours, July 31,

anadromous fish. Operations in these areas are subject to

(Title 5, Alaska Administrative Code). Prior to commenetn~
operations you may be contacted by the Habitat Coordinator's
Office, Departument of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
Chapter 3, Article ? and the =egulations promulgated thereunder
(Title 18, Alaska Administrative Code, Chapter 70) and by the
