ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Original PBTD = 9582' Estimated Cement Top = 9335' CTMD 10/25/97 Set whipstock at 9,274 ft MD with approximately a -7° orientation VVhipstock set well, with a large drop in weight seen. SWS logged out of position and then dropped down to tag the top at 9,263 (+9.7 ft from CCL to 0) putting whipstock at 9,274 ft MD. Run Kinley caliper over liner. 4. Abandon existing perforations by sanding back from PBTD to approx. 9400'. A cement cap will be layed above the sand back to a depth of 9320-9350. 5. RU slickline and tag TOC and pressure test with AOGCC witness. 6. Set whipstock on E-line with tOp of whipstock at 9284'. Performed HCl perforation wash. Pumped cement and squeezed existing perforations. Reversed out after 24 hours. Performed inflow test; failed. Performed resqueeze. Passed inflow test. Rigged up Schlumberger and reperforated the following intervals using 3 3/8" carrier guns at 4 spf: 9426' - 9484' MD BKB 9500' - 9554' MD BKB 9569' - 9600' MD BKB All depths are referenced to BHCS 3/12/83.
