INSTRUCTIONS General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Item 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. Item 16 and 24: if this well is completed for separate production from more than one interval (multiple completion),..so state in item 16, andin item 24 show the producing intervals for. only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Item 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). Item 23: Attached supplemental records for this well should show the details of any multiple stage cement- ing and the location of the cementing tool. Item .27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water In- jection, Gas Injection, Shut-in, Other-explain. Item 28: If no cores taken, indicate "none". Form 10-407 ]{-02A (11-33-12-14/ DESCRIPTION OF WORK COMPLETED PRE SIDETRACK CTU CEMENT PLUG 11150-l1350MD' A Cement Plug Lay was peformed on the interval using the following fluids: SUMMARY Bullhead killed well with 404 bbl seawater. Perfs plugged during kill, so switched to circulating down the coil with retums down the towline while running in the hole. Tagged bottom at 11,353 CTD. Repeat tag was at 11,366. Appeared to be pushing the bridge plug downhole. Laid in eight bbls cement 5' above f'mal tag. Closed wing and monitored WHP. WHP rose from 190 to 230 psi in four minutes, due to gas coming out of solution. Final overbalance would be about 600 psi, assuming a full water column and 3500 psi in the reservoir. Did not pressure up further, due to risk of blowing plug downhole. Pulled out and freeze protected wellbore to 2000 ft. Put 1 bbl MEOH into towline to F/P S-riser. Lay a cement plug from the bridge plug at 11,342' MD to 100' above the top perforation (top perforation at 11,330' MD). Notify an AOGCC representative pdor to commencing plug-back operations. 3. With an AOGCC representative on location to witness the operation, please tag the top of the cement to ensure sufficient cement is in place. 4. RIH and cut the 5-1/2" L-80 tubing with chemical cutter at approximately 8350' MD. Planned kickoff depth is 8000' MD. 5. Displace the tubing and wellbore to 8.5 ppg seawater. Freeze protect to 2000' MD with diesel. 6. Bleed tubing and annuli pressures to zero. Set a back pressure valve in the tubing hanger. · Remove the well house and flowlines. If necessary, level the pad and dig out the cellar. Please provide total well preparation cost on the handover form. NOTE: The proposed kickoff depth for this sidetrack is 8000' MD. Since the kickoff depth is a considerable length above the liner cement plug, a 9-5/8" bridge plug will be set at 8300' MD after the tubing is pulled. The kickoff plug which will be placed above the bridge plug and will also serves as an additional P&A plug. Gas Cons. C0mmissi0r~ WELLHEAD: McEVOY BF.
