were concluded for the day at 4:30 p.m. with 10'wells being tested.
Thursday, Nov.e.mber 20.t 1986: The tests began at 7:00 a.m. and were
concluded for the day at 7:00 p.m. with 114' wells being tested.
..Fridayt November 21t 1986: The tests began at 6:30 a.m. and were
concluded for the day at 4:30 p.m. with ZO:'wells being tested.
Saturday, November 22, 1986: The tests began at 6:30 a.m. and were
concluded at 1:00 p.m.; and were resumed at 6:00 p.m. and concluded
for the day at 9:00 p.m.
As the attached mechanical integrity test ~ports show,

were tested, all of which met the/~AOGCC regulations.
In summary, I witnessed the mechanical integrity tests on Standard
Alaska Production Company's injections wells listed on the attached
test forms.

Pluaback Procedure for U4A Redrill
Procedure Outline' (Refer to attached wellbore schematic)
1. Notify an AOGCC representative prior to commencing operations.
2. Rig up coiled tubing unit. RIH with coil tubing. Kill well by circulating seawater.
Lay a cement plug from current PBTD of _+ 11,713' MD to through tail pipe to top
of permanent packer at +_ 10,910' MD
3. RU slickline unit. RIH and confirm cement top at least 100' above top
perforations. Verify squeeze integrity by pressuring to 1500 psi. Tag to be
witnessed by AOGCC representative.
4. RU E-line unit. RIH with jet cutter and cut tubing above packer at 10900'.
5. Circulate well over to seawater and freeze protect well to 2000'.
6. MIRU Nabors 18E. Kill well. NU & test BOPS.
7. Verify tubing cut above packer. Spot a 100' balanced cement plug on top of
packer from 10,910' to 10,810'. POOH with tubing.
8. Pressure test squeeze to 1500 psi.
9. RIH with section mill. Tag cement top & verify plug integrity with 20,000# set
down weight.
Note: Tag top of cement on subsequent mill run and pressure test plug to
1,500 psi with AOGCC representative present to witness.

RIH with Mill Master and cut a 75 ft window from 9,070 ft MD to 9,145 ft MD.
Spot a 500' of kick-off cement plug from 9345'- 8845' MD.
RIH w/steerable system. Dress off top of cement plug and kick off at 9,070 ft MD.
Control directional drill 8-1/2" hole to top of Shublik formation at 11,459 ft MD.
Run and cement a 7-5/8" FL4S drilling liner across the Kingyak formation.
RIH with 6-3/4" packed assembly and drill to TD at =11,986 ft MD.
Run open hole logs.
Run and cement a 5-1/2 FL4S 13 chrome production liner in 6-3/4" hole with +/-
250 ft of lap.
Clean out liner to PBTD and test to 3,000 psi. Test tubing X casing annulus to
3,000 psi. Displace well to seawater.
RIH with 4-1/2" 13CR completion string positioning 9-5/8" packer @ 8,774' MD.
Space out, circulate inhibitor and freeze protect, land completion, set packer.
Set two-way check valve. ND BOP's. NU and test new 4-1/16" 5M tree to 5,000
psi with diesel. Pul two way check valve.
Set BPV. Release rig. Run case hole logs. Perforate well.

CTU CEMENT SOUEEZE/HYDRAULIC FRAC OPERATIONS
Coiled tubing cement squeezes of existing perforations were performed as detailed below:
12/21/87 Rigged up coiled tubing unit and pressure tested BOP's to 4000 psi. Performed mud acid
perforation wash and sump cleanout.
12/3 1/87 Performed a cement squeeze on existing perforations. Contaminated cement; WOC.
Reversed out after 24 hours and tested squeeze.
2/29/88 Perforated the following Sag R interval using Dresser Atlas 3 3/8" Omnicone guns at 6 spf:
11242' - 11292' MD BKB
Set a bridge plug at 11373' to shut off lower Ivishak perforations. Returned well to
injection and tested.
8/12/88 Performed a hydraulic fracture of the above Sag R. interval. Returned well to injection and
tested.
8/27/88 Retrieved bridge plug from 11373'.
8/29/88 Perforated the following Ivishak intervals using Gearhart 3 1/8" carrier guns at 4 spf:
Add'l. 11393' - 11396' MD BKB
Reperf 11396' - 11426' MD BKB
Reperf 11436' - 11496' MD BKB
Reperf 11510' - 11535' MD BKB
Returned well to injection and tested.
NOTE: Verbal approval to perforate Sag River interval and frae the Sag received from Lonnie Smith,
AOGCC, on 1/27/88.

Well U-04i Workover Proposal
The following coiled tubing cement squeeze of existing intervals and a
channel is proposed. Following CTU operations, all existing intervals will
be reperforated. Varied shot densities will be used to distribute the
water injection more evenly across the zones of interest.
Prior to the cement work, a sump cleanout will be performed. The fill has
reduced the sump depth by 100' and consists mainly of hydrocarbon by-
products from the produced water injection.
1. Perform pre-workover surveillance.
2. Rig up CTU and support equipment.
3. While injecting, flush produced water 'sludge' out of sump using a
HC1/xylene acid system. Either pump the partialy dissolved
hydrocarbon and solids into the channel/thief zone or reverse
circulate it up the coiled tubing (depending on bottom hole sample
analysis and solids size).
4.
Clean perforations with xylene and mud acid. Overflush the treatment
fluids away from the perforations with 4% ammonium chloride water
followed by seawater.
5. Fluid pack the well.
6. Cement squeeze the perforations.
7. Contaminate cement. WOC.
8. Clean out contaminated cement, pressure test squeeze.
9. Reperforate' all intervals:
11396' - 11426' MD BKB (30') at 6 spf
11436' - 11496' MD BKB (60') at 2 spf
11511' - 11550' MD BKB (39') 1 shot every 2 feet
10. Return the well to injection.

11. Run follow up diagnostic surveys to evaluate injection profile.
Additional perforations in existing intervals may be required to
increase capacity or improve profile conformance.

CTU SAND PACK ATTEMPTS
As proposed in our Application for Sundry Approvals dated 6/8/87,
the following additional intervals were perforated in June 1987:
11488' - 11496' MD BKB @ 4 spf
11511' - 11550' MD BKB @ 1 spf
Immediately following the perforating, a sand-pack operation was
unsuccessfully attempted to eliminate a channel to the OWC. The
well was returned to injection.
In October, 1987 a second sand-pack was attempted, preceded by a
xylene tubing wash. This also was unsuccessful. It is now pro-
posed to cement squeeze existing perforations and channel, details
of which will be forthcoming in an Application for Sundry Approvals.
The well is currently on injection.

STIMULATION OPERATIONS
A bullheaded multi-stage mud acid treatment was performed on the
following perforated intervals in water injection well U-4 on
July 15, 1986. ~~
11396' - 11426' MD
11436' - 11488' MD
The program consisted of the following:
1. Pumped the following:
a. 1400 gal. 15% HC1
b. 2800 gal. 12% HC1 - 3% HF
c. 2800 gal. 3% NH4C1 water
d. 700 gal. gelled 3% NH4Cl water w/700# BAF
2. Repeated steps a through d two additional times for a total of
three stages, eliminating the gelled NH4C1 water and benzoic
acid flakes in the final stage.
3. Displaced fluids to perforations with 1512 gal. methanol.
4. Put well back on injection and tested.

STIMULATION PROPOSAL
A bullheaded multi-stage mud acid treatment is proposed for the
following perforated intervals in injection well U-4.
11396' - 11426' MD
11436' - 11488' MD
The program is as follows:
1. Pump the following:
a. 1400 gal. 15% HC1
b. 2800 gal. 12% HC1 - 3% HF
c. 2800 gal. 3% NH4C1 water
d. 700 gal. gelled 3% NH4C1 water w/700# BAF

Repeat steps a through d two additional times for a total of
three stages, eliminating the gelled NH4C1 water and benzoic
acid flakes in the last stage.

The above fluids will be displaced to the perforations with
1512 gal. methanol.

United Drilling Rig 2, to witness a BOPE test. The drilling
contractor was in the process of finishing the hole.

Tuesday, October 2 1984: The BOPE test co~enced at 9:45 am and

I filled out the AOGCC BOPE inspection report which is attached to
this report.

Well History
Spudded well at 1930 hours September 28, 1984. Vertically drilled
17-1/2" hole to 533', kicked off and directionally drilled 17-1/2"
hole to 2913'. Ran 74 joints 13-3/8" 72# L-80 Buttress casing to
2903'. Cemented with 4230 cu. ft. Permafrost II cement. Performed 32
cu.ft. Permafrost E top job. Installed and tested BOPE. Cleaned out
to 2858', tested casing to 3000 psi, okay. Drilled out to 2705' TVD,
ran leak-off test to 0.60 psi/ft, gradient.
Directionally drilled 12-1/4" hole to 11227'. Ran 285 joints 9-5/8"
475 L-80 Buttress casing to 11222'. Cemented with lead slurry of 2136
cu.ft. Class "G" cement with 8% Gel followed by tail slurry of 230
cu.ft. Class "G" neat. Installed and tested packoff to 5000 psi,
okay. Tested BOPE, okay. Pumped 175 bbls 9.3 ppg NaCl down 9-5/8" x
13-3/8" annulus. Tested 9-5/8" casing to 3000 psi, okay. Cleaned out
float equipment and drilled 8-1/2" hole to 11761'. Ran open hole
logs. Ran 19 joints 7" 265 L-80 IJ4S liner to 11761'. Pumped 35 bbls
Sepiolite followed by 300 cu.ft, class "G" with 18% Salt to cement
liner.'
Downsqueezed the 9-5/8" x 13-3/8" annulus with 282 cu. ft. Permafrost
"C" cement followed by 125 bbls Arctic Pack. Cleaned out to 10990'.
Tested liner lap to 3000 psi, okay. Cleaned out cement from 11588' to
11713'.
Changed well over to NaCl. Tested 7" liner to 3000#, okay. Ran CBL.
Ran 274 joints 5-1/2" 175 L-80 Buttress IPC tubing with 4-1/2"
tailpipe assembly to 11039'. Landed tubing with check valve at
2058'. Installed and tested tree. Displaced tubing to diesel, set
packer at 10910'. Tested tubing to 3500 psi, okay. Released rig at
0600hours October 20, 1984.
Ran gyroscopic survey, tested and secured well October 24, 1984.

Enclosed is the approved application for permit to drill the
above referenced well.
If coring is conducted, a one cubic inch chip from each foot of
recovered core is required. Samples of well cuttings and a mud
log are not required. A continuous directional survey is
required as per 20 AAC 25.050(b)(5). If available, a tape
containing the digitized log information shall be submitted on
all logs for copying except experimental logs, velocity surveys
and dipmeter surveys.
Many rivers in Alaska and their drainage systems have been
classified as important for the spawning or migration of
anadromous fish. Operations in these areas are subject to
AS 16.05.870 and the regulations promulgated thereunder (Title
Alaska Administrative Code). Prior to commencing operations you
may be contacted by the Habitat Coordinator's Office, Department
of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
Chapter 3, Article 7 and the regulations promulgated thereunder
(Title 18, Alaska Administrative Code, Chapter 70) and by the
Federal Water Pollution Control Act, as amended. Prior to
commencing operations you may be contacted by a representative of
the Department of Environmental Conservation.
To aid us in scheduling field work, please notify this office 24
hours prior to commencing installation of the blowout prevention

Proposed Well U-4 in relation to Term Well B.
Please attach this additional information to the Application
for Permit to Drill dated September 4, 1984. The information
on this map is taken directly from the same data base that
calculates the closest approach between wells, and is already
summarized in the Permit to Drill application. At this time,
we do not have the computer software necessary to plot on a
scale larger than 1" = 250'. If this map is unacceptable then
we will plot the data by hand on a 1" = 100' scale as specified
by State regulations.

Install and cement 80' of 20" x 30" insulated conductor to surface.
Drill 17 1/2" hole to 2700' and cement 13 3/8" 725 L-80 Buttress casing to
surface.
Install blowout prevention equipment consisting of 5000 psi W.P. pipe rams,
blind rams, and annular preventer. Maximum possible bottomhole pressure is
4390 psi at 8800' TVDss based on original reservoir pressure. Maximum
possible surface pressure is 3330 psi based on a gas column to surface from
8600' TVDss and an oil colum~ to 8800' TVDss.
Directionally drill 12 1/4" hole to the Sag River Sandstone. Run 9 5/8" 475
L-80 Buttress casing to within 5' of 12 1/4" TD and cement in two stages:
First stage using Class G cement around the 9 5/8" shoe. The second stage
to be downsqueezed in the 13 3/8" x 9 5/8" annulus followed by Arctic Pack.
Directionally drill 8 1/2" hole to TD. Run 7" liner from TD to 250' inside
9 5/8" casing shoe and cement using Class G cement with 18% salt.
Clean out to 7" float equipment. Change over to 9.6 ppg. NaC1 completion
fluid. Test casing and lap to 3000 psi. Run CBL.
Run 9 5/8" production packer on internal plastic coated tubing with a
sliding sleeve. Set packer +50' above top of liner. A subsurface safety
valve with dual control lines--will be placed +50' below base of .permafrost.
Install and test Xmas tree. Displace tubing to--diesel. Set and test packer.
Suspend well for perforating subsequent to rig release.
