RIH w/CT & tagged bottom at 12005' MD, then pumped 5 bbls of neat latex acid resistive cement, followed by 76 bbls latex acid resistive cement w/aggregate to resqueeze the perforations (listed above). Placed 37 bbls of cement behind pipe at 2500 psi maximum squeeze pressure, held for 1 hour. Contaminated cement & cleaned out contaminated cement from wellbore. POOH. Rigged down. 1/18/92 - 3/11/92: Performed underreaming operations of cement nodes & fill from the wellbore down to 11992' MD. 3/15/92: RIH & tagged bottom at 11815' MD. Returned well to production & obtained a valid production test. The well has not yet been reperforated, and is presently producing through leaking squeezed perforations. No plans currently exist to reperforate the well at this time. Note: Oil rate decreased by 368 BOPD, but the decrease in the gas rate provided an incremental rate benefit of 4131 BOPD, resulting in a net oil benefit of 3763 BOPD. RU WLU and make gauge run to PBTD at 12005' MD. . RU CTU and perform /HCL/mud acid wash of perforations 11604-11964' DIL (overall interval). 3. Return well to production for a minimum period of 24 hours. 4. Surge well and clean up by flowing to remove debris. Mr. D. F. Scheve Operations Engineering Manager ARCO Alaska, Inc. P. O.'Box 100360 Anchorage, AK 99510-0360 Dear Mr. Scheve: By letter dated November 24, 1986, ARCO Alaska, Inc. has requested waiver of the productivity profile required by Conservation Order 165, as amended by Conservation Order 213, in PBU well DS 9-34. The Commission has reviewed the need for a productivity profile~ in PBU well DS 9-34, and has determined that since the well is only perforated in a single zone, a profile would not yield any additional information necessary for proper monitoring of the Prudhoe Oil Pool performance. Therefore, by administrative approval authorized by Conservation Order 213, Rule 2, the Commission waives the productivity profile requirement in PBU well DS 9-34. INSTRUCTIONS General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Item 1' Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. Item 5: Indicate Which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. Item 16 and 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Item 21' Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). Item 23' Attached supplemental records for this well should show the details of any multiple stage cement- ing and the location of the cementing tool. Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water In- jection, Gas Injection, Shut-in, Other-explain. Item 28' If no cores taken, indicate "none". I filled out an AOGCC BOPE inspection report which is attached. In summary, I witnessed the successful BOPE test at ARCO's DS 9-34 well. ~.
