ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. Form 10-407 Rev. 07-01-80 A-32 DESCRIPTION OF WORK COMPLETED PRE-SIDETRACK CEMENT PLUG 06/19/99 A cement plugback was performed on the interval using the following fluids: SUMMARY Moved in rig up DS CTU #2, squeeze equipment and HB&R. Rig in hole with cementing nozzle. Attempt to circulate well without success. Shoot IA FL at 210'. Tag with nozzle at 10327' (corrected). Sweep tubing and cool down with 2% KCL water. Injectivity with water = 2.2 bpm at 350 psi. Pump 98 bbls diesel down IA. Pump 50 bbls diesel down tubing. Positive pressure on IA and tubing. Lay in 3 bbls neat cement, followed by 10 bbls with 10 ppb aggregate and 5 bbls neat (additives listed below). Cover perforations with choke open. Close choke and build 1000 psi squeeze pressure. Hold 1000 psi and lay in remainder. Placed approximately 4 bbls cement behind pipe. Calc'd cement top = 9925' Hold 1000 psi WHP by pumping pipe displacement and POH. At surface bleed WHP to 500 psi and shut well in with 500 psi on tbg & IA. Good Squeeze. RD CTU #2, HB&R and squeeze equipment. IA and tubing freeze protected to 2000' with diesel. To: From: Subject: Reference: Blair Wondzell - AOGCC Date: June 10, 1999 William Isaacson A-32 Application for Sundry Permit - P&A for Sidetrack Operations APl #50-029-21395 Well A-32 is currently producing from Zones 3 & 4 of the Sadlerochit, with excessive water production coming from Zone 3. The well has several holes in the lower portion of the tubing, and is producing under a variance from the AOGCC. The well is planned to be sidetracked to a new bottom hole location with the primary target being upper Zone 4b. The A-32A sidetrack is currently scheduled for Nabors 7ES following the current workover operations on NGI-05. The rig is expected to finish operations on NGI-05, Friday June 11th. Planning for the A-32A sidetrack is still in progress and we expect the rig to be on stand-by for several days until the pre-rig work and the required permits are submitted and approved. The pre-rig work for the A-32A sidetrack is planned to begin as soon as the attached Application for Sundry is approved. Pre-Rig Work: * Notify the field AOGCC representative with intent to plug and abandon * 1. RIH with CTU and clean-out any fill to PBTD at 10,670' MD. Perform a cement P&A of 15.8 ppg class 'G' cement with adequate cement volume to fill the 7" liner from PBTD to a minimum of 100' above the top perforation at 10,184' MD, plus squeeze 5 bbls to the perforations. Estimated minimum cement volume needed is 28 barrels. Note: Top of cement must be at a minimum of 10,084' for AOGCC compliance. Top of cement may extend up to the tubing tail as long as it does not interfere with the tubing cutting operations. . Rig up slickline, tag and document top of cement and drift tubing for upcoming tubing cut. Liquid pack the 9-5/8" x 4-1/2" annulus to surface. Test the integrity of the cement plug and 9-5/8" casing by simultaneously pressure testing both the tubing and 9-5/8" annulus to 3000 psi for 30 minutes (tubing has two identified leaks at 9692' and 9734'). Increase tubing pressure prior to the 9-5/8" annulus to avoid possibility of tubing collapse. Note: This step should be done prior to the tubing cut to allow additional trouble-shooting if either the cement plug or 9-5/8" casing is leaking. 4. Cut the 4 ¥2" L-80 tubing as close to 8130' MD as possible using a chemical cutter (based upon 8/7/85 Tubing Summary). This cut will not be fished, however avoid cutting a tubing collar. Displace the well to clean seawater by circulating down tubing through the tubing cut. Continue to circulate until clean seawater returns to surface. Freeze protect top 2200' MD with diesel. 5. Test the McEvoy 9-5/8" mandrel packoff and tubing hanger seals to 5000 psi for 30 minutes. 6. Bleed tubing and annuli pressures to zero. Set a BPV in the tubing hanger and test from below to 1000 psi. Secure well. 7. Remove the well house and flowlines. If necessary, level the pad. Please provide total well preparation cost on the handover form. Ri.q Operations: 1. MIRU Nabors 7ES. 2. Circulate out freeze protection and displace well to seawater. Set TWC. 3. Nipple down tree. Nipple up and test BOPE. 4. Pull and lay down 4-1/2" tubing from E-line cut and above. 5. RU slickline and make GR/JB run to top of tubing cut. 6. RIH with 9-5/8" Baker bottom-trip anchor whipstock assembly. Orient whipstock as desired and set on tubing stub. Mill window in 9-5/8" casing at -8100' MD. POOH. 7. Kick-off and drill the 8-1/2" intermediate section to Top Sag @ +9966' MD (8767' TVD). 8. Run and cement a 7", 26# intermediate drilling liner, fully cemented into the 9-5/8" casing. 9. Make up and RIH with a 6" directional assembly to the landing collar. 10. Test the 9-5/8" casing and 7" liner from the landing collar to surface to 3000 psi for 30 minutes. 11. Drill the 6" production section building 16°/100' to horizontal, targeting the Zone 4 sands. Drill ahead to an estimated total depth of 12,565' MD (9049' TVD). 12. Run and cement a 4 ½", 12.6# solid production liner throughout the 6" wellbore. 13. Test the 4-1/2", 7", and 9-5/8" casing from the 4-1/2" landing collar to surface to 3000 psi for 30 minutes. Displace well to clean seawater above liner top. 14. Pick up USIT/CBT on 2-3/8" x 4-1/2" DP and RIH to top of liner. WOC as required, and then log 4-1/2" liner for cement evaluation. Spot high vis perf pill in liner and POOH. 15. Pick up perf guns on 2-3/8" x 4-1/2" DP and RIH to perforate -300' of selective intervals. Perforate well with -200 psi underbalance to the reservoir. POOH LD DP. 16. Run 4-1/2", 12.6# L-80 BTC completion tying into the 4-1/2" liner top. 17. Set packer and test the tubing to 3500 psi and annulus to 3000 psi for 30 minutes with 1000 psi on the tubing. 18. Nipple down the BOPE. Nipple up and test the tree to 5000 psi. 19. Freeze protect the well to -2200' and secure the well. 20. Rig down and move off. The purpose of the log was to record the static temperature gradient within the wellbore. A normal formation temperature gradient was observed in the well. The source of communication is corrosion induced holes in the tubing located at 9,692' MD and 9,734' MD. The well was gas lifting normally from both the orifice and the tubing leaks at the time of the survey. Well A-32's last representative well test was 9/12/93 at 610 BOPD, 89%WC and 1993 GOR prior to being shut-in. It is requested that a variance be granted to return the well to production for a maximum period of 15 days. If this test period confirms production can be sustained in commercial quantities the well will be repaired and returned to full- time production. The following is proposed; 1. Receive variance from the State. 2. Return A-32 to gas lifted production for test period of 15 days. 3. Install tubing patch. 4. Test the tubing (MIT) and annulus for communication.
