INSTRUCTIONS
GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska.
ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation,
injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any
attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the
producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data
pertinent to such interval.
ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool.
ITEM 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, Other-explain.
ITEM 28: If no cores taken, indicate 'none'.

Re: CTD Sidetrack of Niakuk well NK-62
/
Niakuk well NK-62 is scheduled for a through tubing sidetrack with the Nordic 1 cgil. xtrjlling rig. The sidetrack,
NK-62A, will kick off from a window milled in the DFS sand and will target tl~B~B~and. All existing perfs
will be cement squeezed and the new sidetrack will be completed with a fully c~t~ted liner. -'~'
The following describes work planned to prep for sidetracking and is submitted as an attachment to Form 10-
403. A schematic diagram of the proposed sidetrack is attached for reference.
1. Install dummy gas lift valves. Drift and tag TD.
2. MIT tubing, IA, OA./'
3. Run caliper log in 7" liner and in 4-1/2" tubing.
4. Install a mechanical whipstock for an 11840' MD KOP.
5. Either a service coil tub~g'ffn-i't")ar the CTD rig will squeeze 15.8 ppg cement plug across all existing
perfs. A minimum)~20 bb.!s,6f 15.8 ppg cement will be used. Cement will be washed down to the
whipstock tray. ~ ..........
Steps 2-4 will begin as soon as possible with the goal of having the whipstock set by April 5th. CTD is
scheduled to mill the window and drill the sidetrack on April 10, 2003.

Aoril 16, 200.1: I traveled to the Niakuk Field to witness Safety Valve System
tests on BPX's oil producing wells. Well house inspections were also
conducted.
The testing went well with Ron White as the BPX representative in charge of
testing, 1.2 wells were tested with two failures witnessed, The pilot on NK-12A
tripped below 50% of the separator pressure and was replaced and passed a
retest, The SSV on NK-62 failed to hold D,P. and was stroked and passed a
'retest. I inspected 12 well houses and all were in order with no exceptions noted,
The AOGCC test forms am attached for reference.
S, UM,M. ARy,,.;. I witnessed SVS tests on 12 oil producing wells in BPX's Niakuk
Field.
t 2 wells tested, 36 components, 2 failures witnessed, 5.5% failure rate.
12 well houses inspected, ho.exceptions noted. ..

ITEM 5.' Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces
on this form and in any attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item
16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional
interval to be separately produced, showing the data pertinent to such interval.
ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of
the cementing tool.
ITEM 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In,
Other-explain.

Dear Mr. Shultz:
Enclosed is the approved application for permit to drill the above referenced well.
The permit to drill does not exempt you from obtaining additional permits required by law from
other governmental agencies, and does not authorize conducting drilling operations until all other
required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25.035.
Sufficient notice (approximately 24 hours) of the BOPE test performed before drilling below the
surface easing shoe must be given so that a representative of the Commission may witness the
test. Notice may be given by contacting the Commission petroleum field inspector on the North
Slope pager at 659-3607.

Reservoir Pressure
This well will penetrate a portion of the reservoir supported by water injection from NK- 16. The
last fall-off test indicated a reservoir pressure gradient of 9.8 ppg EMW. The maximum
anticipated surface pressure is 4380 psi; based on a bottom hole pressure of 4865 psi at 9550' tvd
and dry gas to surface.
Close Approaches
NK-16 and NK-17 are adjacent (at the surface) water injection wells to NK-62 and will be shut-
in until surface casing is set.
Well Control:
Well control equipment consisting of 5000 psi WP pipe rams, blind rams, and annular preventer
will be installed and is capable of handling maximum potential surface pressures.
Diverter, BOPE and mud fluid system schematics on file with AOGCC.
