Bullheaded kill the well with 365 bbls FSW. Well is a drinker. RIH and cool sump with 86 bbls.
FSW. Laid in 2 bbl "G", 18 bbls fiber cement and 1 bbl "G". POOH to safety of 9600. Pumped a
total of 31 bbls down b/s and tubing before fluid at surface. Based on a static of 3375 psi this
would equate to a fluid level of,--1300 or 16 bbls in ct/tubing annulus indicating well drank cement
to 10840'. Jetted 50 bbl. biozan to 10735 CTD and POOH. Saw cement tins in tank with 180
bbls of jetting SW. There were indications from pump pressure that some of the fiber cement
was left in coil.

Well D-26 Prep for CT Sidetrack
Well D-26 is scheduled for a through tubing sidetrack to be drilled with
coiled tubing. The following summarizes the procedure required to
prepare D-26 for sidetracking and is submitted as an attachment to
Form 10-403.
1. Perform tubing integrity test.
2. Drift tubing and liner with a 3.7" OD, 20 ft long drift.
3. Run Kinley caliper over the proposed whipstock interval to confirm
liner condition.
4. RU service coil and abandon existing perforations with approx. 20
bbls. of fiber cement in preparation for cement whipstock..
Squeeze to 2000 psi. Pressure test with AOGCC witness.
5. RU slick line and tag cement with AOGCC witness.
6. Mill window in liner with drilling coil.

STIMULATION OPERATIONS
A CTU selective HCI treatment was performed October 7, 1990 on Well D-26.
Pressure tested to 4000 psi to start acid job. Pumped 13 BBLS of diesel with mutual
solvent. Pumped 25 BBLS of 15% HCL and 8 BBLS NH4CL with Divert 1. The well
was returned to production immediately following the treatment; then tested.

Procedure Outline:
1. MIRU workover rig. Accepted rig at 1500 hrs on 3-31-89.
2. Pulled BPV. Circulated diesel from well. Installed two-way
check valve.
3. ND tree. NU & tested BOP's to 5000 psi.
4. Pulled 4-1/2" tubing. Changed rams to 3-1/2". Tested BOP's.
5. RIH w/overshot release and recovered the 9-5/'8" Baker
retrievable packer with 4-1/2" tubing.
6. RIH with 6" overshot. Recovered cutoff and 'K' anchor from 7"
packer. RIH 9-5/8" casing scraper. Reverse circulated.
7. Made 7" scraper run to top of fish. Replaced 9-5/8" packoff &
tested to 5000 psi.
8. RIH with 7" RTTS and dummy latch. RIH and tested 9-5/8"
casing and 7" liner lap to 2500 psi. Stabbed dummy into latch
and circulated. POH.
9. Made 8.5" gauge ring/junk basket run.
10. Ran 4-1/2" straddle completion with GLM's.
1 1. Latched into 7" packer.
12. RU wireline and set plug in the tailpipe.
13. Set 9-5/8" packer.
14. Sheared SBR. Determined space out and displaced annulus to
inhibited brine and landed tubing.
15. Tested SBR. Set 2-way check. ND BOP's. NU & tested tree to
5000 psi.
16. Pressured up annulus to test packer and sheared valve in top
GLM.
17. Pumped 135 bbls diesel down the annulus and "U" tubed diesel
to tubing.
18. Set BPV.
19. Pumped down cellar. Released rig @ 1900 hfs on 4/7/89.

Gentlemen:
Enclosed, in triplicate, are Application for Sundry
Approvals (Form 10-403) requesting approval to
continue producing Well D-26 until a scheduled rig
workover to repair a 9-5/8" packer/PBR leak can be
completed during April, 1989. As a temporary
measure, the inner annulus has been liquid filled to
place an overbalanced hydrostatic pressure
differential on the tubing.

A lubricator will be installed and tested to 3500 psi for all well
work. A SSSV will be set and tested in the ball valve nipple at
2036' at the completion of operations.

Dear Mr. Allan:
Enclosed is the approved application for permit to drill the
above referenced well.
The permit to drill does not indennlify you from the probable need
to obtain additional permits required by law from other
governmental agencies prior to commencing operations at the well
site.
To aid us in scheduling field work, please notify this office 24
hours prior to commencing installation of the blowout prevention
equipment so that a representative of the Commission may be
present to witness testing of the equipment before the surface
casing shoe is drilled. ~.Paere a diverter system is required,
please also notify this office 24 hours prior to commencing
equipment installation so that the Commission may witness testing
before drilling below the shoe of the conductor pipe.

Install and cement 80' of 20" x 30" insulated conductor to surface.
Drill 17-1/2" hole to 2700' MD and cement 12-3/8" 72# L-80 Buttress
casing to surface,
Install blowout prevention equipment consisting of 5000 psi W~P~
pipe rams, blind rams, and annular preventer. Maximum possible
bottomhole pressure is 4390 psi at 8800' TVDss based on original
reservoir pressure. Maximum possible surface pressure is 3300 psi
based on a gas column to surface from 8600' TVDss and an oil column
to 8800' TVDss~
Directionally drill 12-1/4" 'hole to the 9-5/8" casing point above
the Sadlerochit Sandstone~ Run 9-5/8" 47# L-80 Buttress casing to
within 5' of 12-1/4" TD and cement in two stages: First stage using
Class G cement around the 9-5/8" shoe~ The second stage to be
downsqueezed in the 13-3/8" x 9-5/8" annulus followed by nonfreezing
fluid~
Directionally drill 8-1/2" hole to TD~ Run 7" liner from TD to 250'
inside 9-5/8" casing shoe and cement using Class G cement with 5%
KCL~
Clean out t° 7" float equipmentJ. Change over to 9~6 ppg. NaCl
completion fluid~ TeSt casing and lap to 3000 psi"
Run 9-5/8" production packer on internal plastic coated tubing with
gas lift mandrels and a sliding sleeve. Set packer above top of
liner. A subsurface safety valve with dual control lines will be
placed 50' below base of permafrost. Install and test Xmas tree.
Displace tubing to diesel. Set.and test packer.
Suspend well for perforating subsequent to rig release.
