leases in Alaska.
Item 1: Classification of Service wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water
supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other
spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple completion), so state in
item 16, and in item 24 show the producing intervals for the interval reported in item 27. Submit a separate form for each
additional interval to be separately produced, showing the data pertinent to such interval.
Item 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
Item 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location
of the cementing tool.

Well M-29ai Prep for CT Sidetrack
Well M-29 is scheduled for a through tubing sidetrack to be drilled with coiled
tubing. The following summarizes the procedure required to prep M-29 for
sidetracking and is submitted as an attachment to Form 10-403. A schematic
diagram of this proposed completion is attached.
1. A Tubing Integrity Test was successfully performed On 7/3/00.
2. The tubing will be drit~ed and a casing caliper will be run.
:3. E-line will be used to set a whipstock at approx. 9540' MD.
4. Service coil will be used to pump cement past the whipstock to abandon
existing perfs. Service coil will then mill the window. Service coil work is
expected to start about July 8.
5. Drilling coil is expected to move onto the well for sidetrack drilling about
July 11, 2000.

A HCL Treatment was performed to remove scale using
the following fluids:
SUMMARY
Well was swapped from PWI to MI injection. The well stacked out
at MI header pressure and plugged off. RU HB&R to pump Xylene
to clean-up the perfs and return the well to MI injection.
Injectivity improved during the job. However, Treating
Pressures and Shut-in Pressures are higher than should have
been expected during and following the job. This well still has
significant near-wellbore damage and may require subsequent
work.

CTU CEMENT SOUEEZE PROPOSAL
A coiled tubing cement squeeze of existing perforations is proposed as follows:
1. Perform pre-workover surveillance.
2. Rig up cTu and support equipment and pressure test BOP's to 4000 psi.
3. Perform perforation acid wash.
4. Fluid pack wellbore with produced water.
5. Pump cement and squeeze existing perforations.
6. Reverse out cement after squeeze is obtained. Wait 24 hours then test squeeze.
If squeeze fails, immediately resqueeze.
7. If squeeze is successful, reperforate the following intervals:
9807' - 9816' MD BKB
9980' - 9998' MD BKB
All depths are referenced to BHCS 1/29/89.
8. Return well to injection.
Depending on the results of this work, the following additional intervals may be
perforated:
9656' - 9789' MD BKB
9870' - 9912' MD BKB
9956' - 9980' MD BKB

all types of lands and leases in Alaska.
Item 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the
data pertinent to such interval.
Item 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
Item 23: Attached supplemental records for this well should show the details of any multiple stage cement-
ing and the location of the cementing tool.
Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water In-
jection, Gas Injection, Shut-in, Other-explain.
Item 28: If no cores taken, indicate "none".

PROPOSED DRILLING AND COMPLETION PROGRAM
WELL M-29 [41-12-11-121 PBU - WATER IN.lECTOR
Install and cement 80' of 20" conductor to surface.
Install 20" Diverter.
The Drilling fluid program will consist of an 8.5 - 9.5 ppg spud mud in the 17-
1/2" hole, a 9.0 - 9.7 ppg water base mud in the 12-1/4" hole and a 8.7 ppg water
base mud in the 8-1/2" hole.
Drill 17-1/2" hole to 2700' and cement 13-3/8" L-80 Buttress casing to surface.
Install blowout prevention equipment consisting of 5000 psi W.P. pipe rams,
blind rams, and annular preventer. Maximum possible bottomhole pressure is
4390 psi at 8800' TVDss based on original reservoir pressure. Maximum
possible surface pressure is 3330 psi based on a gas column to surface from
8600' TVDss and an oil column to 8800' TVDss.
DireCtionally drill 12-1/4" hole to the 9-5/8" casing point above the Sadlerochit
Sandstone. Run 9-5/8" 47# NT80-S NSCC casing to within 5' of 12-1/4" TD and
cement using Class G cement around the 9-5/8" shoe. Perform a mechanical
integrity test. The 13-3/8" x 9-5/8" annulus will be freeze protected with dry
crude.
Directionally drill 8-1/2" hole to TD. Run 7" liner from TD to 250' inside
9-5/8" casing shoe and cement using Class G cement.
Clean out to 7" float equipment. Change over to 8.7 ppg filtered NaCL
completion fluid. Test casing and lap to 3000 psi. Run cased hole logs,
Perforate well and mn a WD Isolation Packer.
Run 9-5/8" production packer on 7" tubing with a sliding sleeve. Stab into the
seal assembly, space out and set packer 50' above top of liner. A subsurface
safety valve with dual control lines will be placed 100' below base of
permafrost. Install and test Xmas tree. Freeze protect annulus and tubing.
Shut down rig operations for further perforating subsequent to rig release.
The well will be perforated in accordance with normal practices.

Revised M-Pad Well M-29
Close Approach Analysis
Richard:
We have examined the potential intersections of well M-29 with
the other M-Pad wells. The profile for M-29 was calculated from
parameters provided by you and the other wells were calculated
from gyro data provided by Standard.
As you requested, M-29's main KOP was moved 150 feet due west.
This was done with a KOP at 599, build 2.0/100 to 4.63 deg and
drop 1.0/100 back to vertical by 2800 TVD BKB.
The closest well to M-29 is M-05 which has a near point of 22
feet at 3850 TVD BKB. This is also the vicinity of the close
crossing with M-05 and will need to be closely watched. The near
points are summarized below:
Well Distance At TVD BKB
M-05 22 ft 3850
M-02 22 600
M-01 125 3900
With 7 deg lead
26' at 3738 TVD
87 3900
M-29 has crossings with M-05, M-02, M-17, and M-23. Ail except
M-05 exceed 450 feet of separation. M-05 crossing detail:
Well TVD BKB M-05 TVD M-29 is:
mm~m ~mm~mmm~
M-29 3762 3839 77 ft above
M-29 7 deg 3634 3727 93 ft above
TVD's have been adjusted to a Kelly bushing of 56 feet.
