leases in Alaska.
ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water
supply for injection, observation, injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces
on this form and in any attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item
16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional
interval to be separately produced, showing the data pertinent to such interval.

DESCRIPTION OF WORK COMPLETED
PRE-SIDETRACK CEMENT PLUG
10/13/98
A Cement Plugback was performed on the interval using the
following fluids:
SUMMARY
Moved in rig up DS CTU #2. Well is shut in and FP. Rig in hole w/2" combo
nozzle. Go through GL station #3, see no over pull through it today. Rig in hole
through seal assembly to 10630'. Have ratty PU of 29k. Pull up to circulate
more 2% Kcl water to see if we can get cleaner PU. Shut in wing and perform
short injectivity test. Same as acid job done previously, 0.5 bpm at 2445 psig.
DS cementers MIRU. Circulate bull head 10 bbls 12/10 HCL/Xylene. See a
,-,200 psig break. Injectivity is 0.5 bpm 1733 psig. Mix and pump 11.8 bbls of
15.8 ppg class G cement w/5 bbl fresh water spacer ahead. Circulate down and
then bull head cement w/nozzle positioned at 10580'. Get possibly 1/2-1 bbls
cement behind pipe. Squeeze to 3500 psig. Lost 70 psig in 6 min. Rig down
moved off DS pump and pulled out of hole replacing voidage. FP tubing and
FL. Leave 500 psig on IA and 150 psig on tubing. Rig down moved off DS CTU
#2. Estimate. TOC at ~10765'. IA and Tubing are still in communication after
abandonment plug in place.

Extended Horizontal Sidetrack Well (W - 19A)
Nabors 18 E will move to W - 19A on/about October 20,1998 following the
completion of Well W - 12A. The drilling objective of well W - 19A is to mill through
9 5/8" csg via Whipstock, directionally drill a 8 1/2" hole to Top of Sag River. Run,
cement and test a 7" liner. Continue directionally drill a 6" hole to 1 target, Zone 4
( 15113' MD, 6" section ). Run, cement and test a 4 W', L-80 liner, perforate the
productive zones, then complete the well with 4 ~", L-80 tbg.
Objective:
The objective of the pre-rig work is to ascertain the condition of the 3 Y2" tubing presently in the
hole; P & A any open perforations; chemically cut the 3 ~" tubing; test the 9 5/8" pack off.
Pre-Rig Operations:
NO
1.
WORK MAY BEGIN PRIOR TO AN APPROVED SUNDRY PERMIT.
RU slickline and drift 3 ~" tbg. If cannot get coil tbg to production packer ( due to I.D
restriction @ 10602' ), notify Operations Drilling Engineer.
::2. P & A Open perforation (11,252' - 11,320').With 10 bbls of class "G" cement, Top of Cement
MUST NOT be above 10.,900' (Minimum per AOGCC = 11,1 52' or 100' above uppermost
maraseal squeezed perforation ). Tag and test cement plugback. Notify AOGCC of tag &
test.
3. Pressure test 9 5/8" to 3,000 psi. Report results to ODE in Anchorage.
4. RU electric line and make a chemical cut of tubing in the second full joint above production
packer _+ 10,530' MD. NOTE: Cut depth should be very close to 10530',as the stub will
determine final Kick Off point. Circulate well to seawater through the cut. Freeze protect to
_+ 2,200' MD.
:5. Pressure test the 9 5/8" packoff. Carefully cycles LDS.
(~. Set a BPV in preparation of the rig sidetrack.
Rig Sidetrack Procedure
1. MIRU. Circulate out freeze protection. ND tree, NU and test BOPE. Circulate well clean.
2. Pull the 3 ~" tubing completion. Tubing cut at _+ 10,530' MD.
3. Replace the leaking 9 5/8" packoff if necessary.
4. RIH w/GR/JB to top of cut tubing.
.5. Set 9 5/8"whipstock on top of cut tubing and mill window.
(~. Kick-off and directionally drill 8 1/2" hole to 10,958'.
?. Run and cement 7" ,26#, L-80 liner.
8. Directionally drill the Zone 4 horizontal section to TD + 15,113' MD.
0. DPC log at horizontal (_+ 11,600' ) log suite are; Platform Express and Sonic, RFT.
10. Run and cement 4 ½", L-80 solid production liner.

above referenced well.
The permit to drill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and
does not authorize conducting drilling operations until all other
required permitting determinations are made.
To aid us in scheduling field work, please notify this office 24
hours prior to commencing installation of the blowout prevention
equipment so that a representative of the Commission may be
present to witness testing of the equipment before the surface
casing shoe is drilled. Where a diverter system is required,
please also notify-this office 24 hours prior to commencing
equipment installation so that the Commission may witness testing
before drilling below the shoe of the conductor pipe.

PROPOSED DRILLING AND COMPLETION PROGRAM
WELL W-19 [21-16-11-12] PBU
Install and cement 80' of 20" conductor to surface.
The Drilling fluid program will consist of an 8.5-9.5 ppg spud mud in the 17-
1/2" hole, a 9.0 - 9.7 ppg water base mud in the 12-1/4" hole and a 9.5 ppg water
base mud in the 8-1/2" hole.
Drill 17-1/2" hole to 2700' MD and cement 13-3/8" L-80 Buttress casing to
surface.
Install blowout prevention equipment consisting of 5000 psi W.P. pipe rams,
blind rams, and annular preventer. Maximum possible bottomhole pressure is
4390 psi at 8800' TVDss based on original reservoir pressure. Maximum
possible surface pressure is 3330 psi based on a gas column to surface from
8600' TVDss and an oil column to 8800' TVDss.
Directionally drill 12-1/4" 'hole to the 9-5/8" casing point 10' below the Sag
River Sandstone. Run 9-5/8" 47# NT-80S NSCC casing to within 5' of 12-1/4" TD
and cement using Class G cement around the 9-5/8" shoe.
Drill 8-1/2" hole to TD. Run 5 1/2" liner from TD to 250' inside 9-5/8" casing
shoe and cement using Class G cement.
Clean out to 5 1/2" float equipment. Change over to 8.9 ppg. filtered NaC1
completion fluid. Test casing and lap to 3000 psi. Run CET/CBT/CCL-GR and
GYRO. Perforate with wireline.
Run 9-5/8" production packer on 3.5" tubing. Set packer 50' above top of liner.
A subsurface safety valve with dual control lines will be placed 200' below base
of permafrost. Install and test Xmas tree. Displace tubing to diesel. Set and test
packer.
Fluids incidental to the drilling of a well will be pumped down the 9-5/8" x
13-3/8" annulus of the well.
Tubing Size: 3-1/2", 9.2# L-80 TDS TBG with GLM's.
Tailpipe Size: 3-1/2" Tailpipe
