Item 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the
data pertinent to such interval.
Item 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
Item 23: Attached supplemental records for this well should show the details of any multiple stage cement-
ing and the location of the cementing tool.

PROCEDURE
1. Run in the hole with metal muncher mill assembly. Mill the 9-5/8" casing to + 3625'.
2. Run in the hole with 13-3/8" R'I-rS. Set R'I'TS at 3450' and pressure test the 13-3/8" casing to 2500
psi for 30 minutes. Release R-I-rS and POOH.
3. Run in the hole with 5" drill pipe and 1000' of 3-1/2" stinger to 4100'. Circulate and condition mud.
4. Mixand pump 800 sx Class "G" + 1%S-1 +0.5% D79+0.2%D46at 15.8ppg. Spot cement as
balanced plug from 4100' to 3300'. Pull out of the hole.
5. Run in the hole with retrievable bridge plug and set same at 2000'. Run in the hole with RTTS and
storm packer, set the R'I-rS (2) joints below casing hanger.
6. Nipple down BOPs. Inspect casing head and replace as necessary. Nipple up BOPs and test same.
7. Run in the hole and retrieve RTTS and storm valve. Run in the hole and retrieve RBP at 2000'.
8. Run in the hole and tag cement. Test cement and casing to 2500 psi. Clean out cement to 3500'.
Spot additional cement plugs as necessary to establish a P & A / kickoff plug.
9. Pull out of the hole to begin the PWDW 1-2A redrill operations.

4. RECORD ON THE IADC REPORT. THE ACCEPTABLE LOWER LIMIT
IS 45 SECONDS CLOSING TIME AND 1200 PSI REMAINING
PRESSURE.
BOP STA(~K TEST
1. FILL BOP STACK AND MANIFOLD WITH ARCTIC DIESEL IN WINTER OR
SEAWATER DURING SUMMER MONTHS.
2. CHECK THAT ALL HOLD-DOWN SCREWS ARE FULLY RETRACTED.
PREDETERMINE DEPTH THAT TEST PLUG WILL SEAT AND SEAT
IN WELLHEAD. RUN IN LOCK-DOWN SCREWS IN HEAD.
3. CLOSE ANNULAR PREVENTER AND CHOKES AND BYPASS VALVES
ON THE MANIFOLD.
4. TEST ALL COMPONENTS TO 250 PSI AND HOLD FOR 10 MINUTES.
5. INCREASE PRESSURE TO 2500 PSI AND HOLD FOR 10 MINUTES.
BLEED TO ZERO PSI.
6. OPEN ANNULAR PREVENTER AND CLOSE TOP SET OF PiPE RAMS.
7. INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 10 MINUTES.
8. CLOSE VALVES DIRECTLY UPSTREAM OF CHOKES. BLEED DOWNSTREAM
PRESSURE TO ZERO PSI TO TEST VALVES. TEST REMAINING UNTESTED
MANIFOLD VALVES, IF ANY, WITH 5000 PSI UPSTREAM OF VALVE AND
WITH ZERO PSI DOWNSTREAM. BLEED SYSTEM TO ZERO PSI.
9. OPEN PIPE RAMS. BACKOFF RUNNING JT AND PULL OUT OF HOLE.
CLOSE BLIND RAMS AND TEST TO 5000 PSI FOR 10 MINUTES. BLEED
TO ZERO PSI.
10. OPEN BLIND RAMS AND RECOVER TEST PLUG. MAKE SURE MANIFOLD AND
LINES ARE FULL OF ARCTIC DIESEL AND ALL VALVES ARE SET IN THE
DRILLING POSITION.
11. TEST STANDPIPE VALVES TO 5000 PSI.
12. TEST KELLY COCKS AND INSIDE BOP TO 5000 PSI WITH KOOMEY PUMP.

straight hole.
CET/CBT both show bonding @ 3500'-_+3700'. This is very unlikely. Cement
volume was calculated by using 4 arm Caliper of hole +15% excess, to bring cement
above Hoss tool @ 5000' cement should be +_270' above Hoss tool.

OBJECTIVE:
The primary objective for this workover is to establish isolation behind the 9-5/8" casing. It is
planned to perforate the well from 5050' - 5060', 600' below the existing SV1 perforations at
4130'- 4220'; 4406' - 4430' and the old squeeze perfs at 4448' - 4450'. A baseline Temperature
/ GR log will be run prior to perforating. Following perforating, 150 degree produced water will
be injected into the lower perforations for 24 hours. Another Temperature / GR log will then
be run to detect any channeling to the old perforations. If channeling is found, the lower
perforations will be squeezed with cement and the injection and the logging procedures
repeated. A maximum of two cement squeezes will possibly be performed on the lower
perforations. The injection and logging procedures will be performed after each squeeze.
Following the work on the lower perforations, the old squeeze perforations at 4448' - 4450'
will be isolated from the lower perforations and the injection and logging procedures will be
repeated. If isolation can not be achieved at any of these points, the well will be plug and
abandoned. If isolation can be established, one last cement squeeze will be performed on the
upper perforations at 4130'- 4220'. The well will then be completed with a 5-1/2" tubing
straddle assembly covering the interval from 4000' - 4500' and 7" tubing to surface. The
procedure below outlines the three injection /logging tests and two cement squeezes at the
lower perforations, the injection tests at the old squeeze perforations, one cement squeeze
at the upper perforations, and completing the well with a straddle assembly.
PROCEDURE:
1. MIRU Nabors 2ES.
2. Kill well if necessary. N/D tree. N/U BOPs.
3. Pull and lay down tubing.
4. RIH with mill and PRT. Mill and pull AVA packer at 3200'.

RIH with 8-1/2" bit and 5" drill pipe. Clean out to PBTD. Circulate fresh 9.1 ppg seawater around.

RIH with RTTS to 4100'. Pressure test casing to 3000 psi for 10 minutes. RIH to 4440' and set
R-FT'S. Pressure test old squeeze perfs at 4448' - 4450' to 1000 psi. RIH to PBTD.

8. POOH to 5035'. Set RTTS.

35. Spot cement and POOH to above TOC. Perform bradenhead squeeze. POOH.
36. Wait on cement for 18 hours.
37. RIH with bit and clean out to the EZSV at 4250'. Test squeeze to 1500 psi. Clean out to PBTD.

42. Run 7" completion assembly with SSSV. Displace well to seawater and freeze protect. Sting into
upper packer and land tubing.
43. Test tubing to 3000 psi. Test packer and casing to 3000 psi. Pressure tests are to be witnessed by
the AOGCC.
44. N/D BOP, N/U tree. Test tree to 5000 psi. Pressure down the tubing to shear the shear out sub.
45. Release rig. Turn over well to Production.

ON THE MANIFOLD.
4. TEST. ALL COMPONENTS TO 250 PSI AND HOLD FOR 10 MINUTES.
5. INCREASE PRESSURE TO 2500 PSI AND HOLD FOR 10 MINUTES.
BLEED TO ZERO PSI.
6. OPEN ANNULAR PREVENTER AND CLOSE TOP SET OF PIPE RAMS.
7. INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 10 MINUTES.
8. CLOSE VALVES DIRECTLY UPSTREAM OF CHOKES. BLEED DOWNSTREAM
PRESSURE TO ZERO PSI TO TEST VALVES. TEST REMAINING UNTESTED
MANIFOLD VALVES, IF ANY, WITH 5000 PSI UPSTREAM OF VALVE AND

Per Rule 7 Requirements of Area Injection Order No. 4
DISCUSSION: Notified Lonnie that the 13-3/8" and surface were in communication
with some pressured interval which responds to injection. I told him our plans were
to do an injectivity test down the 9-5/8" x 13-3/8" annulus to determine if we
could arctic pack the 13-3/8" annulus. Also we are evaluating the possibility of
squeezing the well.

Friday June 29, 1990: I called Lonnie Smith with AOGCC and he
ihfo~ed me ~hai~the injection well AAI PRU PW DW 1-2, had a problem
and to check it out. I did. The well is now disconnected from the
injection source and is shut down. Pressures on well are zero. I
checked the pad around the well. You could see where water had
flooded on the pad but no hydrocarbons were present on or in the
gravel. Production took over well after I witnessed and passed mech
integrity tests. Two were performed and passed. This was not where
the problem was. The problem seems to be a bad cement job on 9 5/8
casing. When injection was started, it came up around 9 5/8 and then
up around and away from the 13 3/8 and 20" and outside the cellar to
the surface. I was told by Steve Massey and James Fausett that this
was the drilling department's problem and should never have been
turned over to production to inject when they knew there was a
problem with the cement bonding to the 9 5/8 casing.
In summary, the problem was not the MIT tests, it was the long string
of 9 5/8 incorrectly cemented in the first place.

Workovcr Objective : Repair cement channel.
1. Make a sliekline tag to verify PBTD.
2. Perform a coiled tubing unit cement squeeze with 150 bbls of cement to isolate the
perforations from 4130'-4450'.
3. Jet out the contaminated cement.
4. Log with gamma ray to determine top of cement.
5. Underream cement across perforations.
6. Reperforate as follows:

7. Bring well on injection.
Other Well Information
Current status: Shut in.

OPERATIONS SUMMARY
1. MIRU ddlling rig. NU diverter and function test same.
2. Spud well and drill 17-1/2" hole to" 3000' TVD /MD.
3. Set 13-3/8" casing and cement same. Nipple up BOPE and test to 5000 psi. Test casing to 2500 psi
for 30 minutes.
4. Pick up 12-1/4" BHA and GIH. Drill 10 ft of new hole and perform leakoff test.
5. Ddll a straight 12-1/4" hole to 6600' TVD/MD.
6. Run wireline logs as programmed.
7. Run 9-5/8" casing and displace cement with brine.
8. Test casing to 3000 psi for 30 minutes.
9. Release drilling rig. Run cased hole logs 7 days after rig release.
10. MI and RU completion rig. Run 7" tubing and packer assembly. Test tubing casing annulus to 3000
psi for 30minutes. Notify the AOGCC so a representative can witness the casing integrity test.
11. N/U 7" tree and test to 3000 psi. Release completion rig.
12. Rig up wireline unit and perforate.
NOTE;
a. The 13-3/8" x 9-5/8" annulus will be left with a non-freezing fluid during any extended period. The annulus
will be sealed with 300 sx of cement followed by Artic Pack.
b. No existing wells are within 200 ft. of the proposed well.
c. The drilling fluid program and surface system will conform to the regulations set forth in 20AAC25.033.
d. Maximum surface pressure is based on BHP and a full column of gas to TD (0.10 psi/fi).
e. Based on the logs from PWDW 1-1, no potential productive zones are anticipated above 3000' TVD.
f. No potential hazardous levels of H2S are anticipated in this well.

1. FILL BOP STACK AND MANIFOLD WITH A NON-FREEZING FLUID.
2. CHECK THAT ALL HOLD DOWN SCREWS ARE FULLY RETRACTED.
MARK THE RUNNING JOINT WITH PREDETERMINED PLUG SEATING
DEPTH. RUN THE TEST PLUG ON DRILL PIPE AND SEAT IN WELL-
HEAD.
3. CLOSE ANNULAR PREVENTER AND UPSTREAM VALVES.
4. FUNCTION TEST CHOKES.
5. IN EACH TEST, FIRST HOLD 250 PSI FOR 3 MINUTES PRIOR TO THE
HIGH PPRERSSURE TEST.
6. INCREASE PRESSURE ON ANNULAR TO 3000 PSI AND HOLD FOR 3
MINUTES. BLEED PRESSURE TO ZERO.
7. OPEN ANNULAR PREVENTER AND CLOSE TOP PIPE RAMS.
INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 3 MINUTES.
BLEED PRESSURE TO ZERO.
8. CLOSE VALVES DIRECTLY UPSTREAM OF THE CHOKES AND OPEN
CHOKES. INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 3
MINUTES. BLEED PRESSURE TO ZERO.
9. OPEN TOP SET OF PIPE RAMS. CLOSE BOTTOM PIPE RAMS.
INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 3 MINUTES.
BLEED PRESSURE TO ZERO.
10. TEST REMAINING MANIFOLD VALVES (IF ANY) WITH 5000 PSI
UPSTREAM OF THE VALVE AND ZERO PRESSURE DOWNSTREAM.
11. BLEED PRESSURE TO ZERO. OPEN BOTTOM PIPE RAMS.
12. BACK OFF RUNNING JOINT AND POH. CLOSE BLIND RAMS AND
TEST TO 5000 PSI FOR 3 MINUTES.
13. BLEED OFF ALL PRESSURE. OPEN BLIND RAMS. FILL MANIFOLD

17. PERFORM COMPLETE BOP TEST WEEKLY AND FUNCTION TEST
DAILY.
