Subject: Corrosion Inhibitor Treatments of GPMA L5
Dear Mr. Maunder,
Enclosed please find multiple copies of a spreadsheet with a list of wells from GPMA L5
that were treated with corrosion inhibitor in the surface casing by conductor annulus.
The corrosion inhibitor is engineered to prevent water from entering the annular space
and causing external corrosion that could result in a surface casing leak to atmosphere.
The attached spreadsheet represents the well name, API and PTD numbers, top of
cement depth prior to filling and volumes of corrosion inhibitor used in each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10 -404,
Report of Sundry Operations.

General: This form is designed for submitting a complete and correct well completion report and Icg on all
types of lands and leases in Alaska.
Item 1: Classification of Service Well: Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16 and in item 24 show the producing intervals for only the inverval reported
in item 27. Submit a separate form for each additional interval to be separately produced showing the data
pertinent to such interval.

...Thrusday, September 22, 1994: I traveled this date to ARCO's Lisburne well L5-19
after being contacted by Eddie Vaughn, ARCO rep., informing me that he was
installing the P & A marker plate.
The crew was sawing off the casing when I arrived. After the casing was sawed off
and removed we saw that the 9.625" casing and the 20" and the 13.375" was full of
hard cement, but the 13.375" and the 9.625" annulas was not full of cement. There
was approximately 15' of dead crude between the casings. This was removed by a
vacuum truck and refilled with cement. The marker plate was welded on with ali of
the correct information bead welded on it. The marker plate was 3' + below tundra
level.
Summary: i witnessed the filling of the 13.375" X 9.625" with cement and the
installation of the P&A marker plate on ARCO's L5-19.

Monday, August 29, 1994: I traveled to Arco's Lisburne L5-19 well to inspect the
well condition and status.
The well head is still installed on the casing, the well house is removed and no flow
lines are connected: The pad and surrounding area was clean and free of debris.
! contacted Arco representative Chris Alonzo and talked with him about their plans
for this well. The well has never been on production and is plugged to surface. He
informed me that before freeze-up, the tree would be removed, the casing cut off
and an abandonment plate welded to the casing stub.
Chris will contact the inspection staff prior to commencing the final abandonment
work on this well.
Summary: I conducted a location inspection on Arco's Lisburne L5-19 well. Final
work should be completed before freeze-up this year.

Dear Ms. Griffin:
Please find enclosed an Application for Sundry Approval to abandon Lisburne well
L5-19. Enclosed you will also find pertinent maps of the location, a wellbore
diagram and a procedure outline for the operation.
L5-19 currently has a 299 ft cement plug on top of 5" drillpipe that was stuck during
the drilling of the 12-1/4" hole at 16,142' (top of fish at 15,998'). Another cement
plug is set from 15,050' - 14,788', when the rathole could not be cleaned out past
15,126'. Two cement plugs are also set across the 9-5/8" shoe. A 9-5/8" bridge
plug is set at 350'. The fluids currently in the wellbore are 11.7 ppg mud from
14,180'- 2200' and diesel from surface to 2200'.
The remainder of the abandonment work is detailed in the attached Outline
Procedure.

Notify an AOGCC representative prior to commencing operations.
Remove ice and fill from cellar to allow access to all wellhead equipment.
Note wellhead and annulus pressures and bleed to zero if necessary.
Rig up to pump through the casing head. Establish injection between the 13-
3/8" and 9-5/8". Pump 300' of cement into the annulus. Leave the TOC at 11'
If an injection rate cannot be established, notify PBD. We will probably need
to cut and pull the 9-5/8" casing at 175' and use one cement plug to fill the 9-
5/8" and 13-3/8" from 350'- 11'
Pull the BPV. Rig up coiled tubing unit. Run in hole to EZSV bridge plug at
350'. Spot an Arctic Set cement plug from 350' to 11'. WOC overnight.
Dig out around the cellar to 5' below ground level. Cut off all casing strings at
least 3' below ground level. Send the wellheads to Drill Site Maintenance.
The gravel pad should be 6' thick.
Install a 4" x 10' post with the following information bead welded to it. If a 4"
post is not readily available use a 4-1/2" x 10' pup joint. The post may be
installed by cementing it into the top of the well or by welding it to a plate and
welding the plate to the well. This procedure should be witnessed by
AOGCC.

Our records show that well number L5-19 was approved for operati.on
shutdown as o.f August 28, 1990. According to 20 AAC 25.072(d), "If
well operations are not resumed within 12-months, the operator shall
immediately proceed to abandon the well." It is your responsibility
to monitor the status' of your wells for compliance with Commission
Regulations.
The Commission has considered your request for an additional three
months to determine if this well fits into your plans for closing out
reserve pits. We certainly hope that you can come to a decision and
plan of action sooner than that, but we approve your requested date of
November 1, 1992.
You are Welcome to discuss your progress in this matter at any time with

so newly developed or so limited or specialized
in application that they are not commonly run
on petroleum exploration or development wells;

(2) "velocity survey" means a survey, the pri-
mary purpose of which is to determine velocity
of seismic waves through formations penetrated
by a well by measuring travel times of seismic
pulses from or near the surface to a seismometer
lowered to various depths in the well. (Eft.

20 AAC 25.072. SHUTDOWN OF WELL OP-
ERATIONS. (a) If climate, operational or en-
vironmental concerns, prudency, or other cir-
cumstances prevent the continuation of the pro-
gram approved on the drilling permit, or for rig
substitution, the operator shall apply to the
commission for approval to shut down well op-
erations. The commission will, in its discretion,
approve the shutdown of well operations. The
letter of application must set out a full justifica-
tion for the shut down and be accompanied by
an Application for Sundry Approvals (Form
10-403) which sets out the proposed condi-
tion of the well bore upon shutdown of well op-
erations, approximate date when well opera-
tions will resume, and a proposed program for
securing the well during the period of shutdown.
(b) The operator shall file with the com-
mission, within 30 days after well shutdown,
a complete well record on a Report of Sundry
Well Operations (Form I0-404), including a
copy of the daily reports of well operations
required by 20 AAC 25.070(a)(1) and a copy
of all logs run in the well as required by 20 AAC
25.071(b)(5). The commission will, in its dis-
cretJon, waive the requirements of this subsec-
tion Jf well operations are to be resumed within
60 days.
(c) Shutdown of well operations does not
establish a completion, suspension, or abandon-
ment date for a well.
(d) If well operations are not resumed within
12 months, the operator shall immediately
proceed to abandon the well. (Eft. 4/2/86, Reg.
97)'

20 AAC 25.075. OTltER WELLS IN DESIG-
NATED AREAS. There are areas in the state
where drilling operations could unexpectedly
encounter oil, gas, or hazardous substances at
shallow depths. When the commission obtains
sufficient evidence to define a specific area and
the approximate depth range of the substances,
it will issue an order that will present the evi-
dence, define the area, and stipulate a drilling
depth. After the issuance of such an order, any
well drilled in the defined area, for any purpose,
that exceeds the stipulated depth will require
a drilling permit and may be subject to the other
requirements of this chapter. (Eft. 4/13/80,

Prudhoe Bay Drilling is studying the feasability of creating injector
wells by converting producing wells that can no longer be produced.
These wells would be used for disposal of drill solids from the
cuttings reserve pits at all North Slope Drill Sites. Our L5-19 well is
one of the wells that we are considering for conversion.
ARCO Alaska's Engineering Group has informed us that you would
like to see this well plugged as soon as possible. If you approve, we
would like an additional 3 months to determine if this well fits into
our plans for closing out reserve pits. We will provide you with a
decision and plan of action by November 1, 1992.

~be set above a fish in the hole. Mike said the top
of the fish was at 15,699 ft.- M.D. which was 210' below the top of
the Sadlerochit pay zone. Mike said verbal approval had been given by
Mike Minder to set a cement kick off plug in the open well bore above
the =~ .
S_aturday July _7, !790: I went to Pool rig 5 and spoke with Mr.
Weata!l concerning the cement plug set the previous day. Mike said he
pumped a 619 sk(ti0 bbls) of 17# Class G cement on the top of the fish
at 15,699' M.D.
Mike said a bit had been run in the hole and the TOC was tagged at
15055' M.D. Mike said the cement would be dressed off to 10' above
the top of the fish and the 9 5/8" casing would be run.

degrees 02 minutes West direction. Maintain hole angle and direction to 13-3/8" casing point at 6115'
MD / 4500' TVD.
3. Run and cement 13-3/8" casing to surface.

mudloggers and mudlog from 14612' MD / 8015' TVD.
Run and cement 9-5/8" casing.

7. Run open-hole logs. Run and cement 7" liner.
8. Run cased hole logs.
9. Run completion assembly and nipple up tree. Release drilling dg. Test well as required.
NOTE:
Excess non-hazardous fluids developed from well operations and as necessary to control fluid levels
of the mud pits, will be pumped down the 9-5/8" x 13-3/8" annulus of an offset well. The annulus will
be left with a non-freezing fluid dudng any extended interruption in the pumping process. The annulus
will be sealed with cement followed by arctic pack upon the completion of the pumping process.
The drilling fluid program and fluid surface system will conform to the regulations set forth in
20AAC25.033.
Maximum surface pressure is based upon a BHP of 4490 psig at 8695' TVD, less a 1500' 'I-VD oil
column, less a gas column to surface.

dictate the actual mud properties needed. Any major changes should

Do not weight up prematurely to control swelling shale in the
