in Alaska.
ITEM I: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply
for injection, observation, injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on
this form and in any attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item
16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional
interval to be separately produced, showing the data pertinent to such interval.
ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the
cementing tool.

Well S-25i Prep for CT Sidetrack
Well S-25i is scheduled for a through tubing sidetrack to be drilled with
coiled tubing. The following summarizes the procedure required to
prepare S-25i for sidetracking and is submitted as an attachment to
Form 10-403. Schematic diagrams of the proposed cement squeeze
and sidetrack completion are attached.
1. A mechanical integrity test has been successfully performed.
2. A Schmoo-Be-Gone treatment to remove mung from the wellbore
has been performed.
2. Tubing and liner have been drifted with a 3.65" OD drift.
4. Kinley caliper has been run over the proposed whipstock interval to
confirm tubing condition.
5. A mechanical ~will be set on e-line at approx. 9,145',
within the existing Sag R. perforations. --
6. Service coil will be used to downsqueeze cement to abandon
existing perforations in the 7" liner. Approx. 20 bbls cement w/
aggregate will be used. A__c_q.tZC,~ze pressure of 1500 psi will be
achieved and witnessed by AOGCC..cement w~ then De Washed
ou-Cf-ff'O'm-t~ fbce of ~ne wn~pstocl~i
7. Service coil will mill the window utilizing a flowline to the GC for all
fluid and cuttings returns. Prior to milling the window the service
coil BOPE will be fully tested with AOGCC witness.

PROPOSED DRILLING AND COMPLETION PROGRAM
WELL S-25 [24-26-12-12] PBU
AFE No. 125259
Install and cement 80' of 20" conductor to surface.
The drilling fluid program will consist of an 8.5 - 9.5 ppg spud mud in the 17-
1/2" hole, a 9.0 - 9.7 ppg water base mud in the 12-1/4" hole and a 9.0 ppg water
base mud in the 8-1/2" hole.
Drill 17-1/2" hole to 2700' TVD(BKB) and cement 13-3/8" L-80 Buttress casing to
surface.
Install blowout prevention equipment consisting of 5000 psi WoP. pipe rams,
blind rams, and annular preventer. Maximum possible bottomhole pressure is
3710 psi at 8800' TVD(SS) based on original reservoir pressure. Maximum
possible surface pressure is 3330 psi based on a gas column to surface from
8600' TVD(SS) and an oil column to 8800' TVD(SS).
Directional drill 12-1/4" hole to the 9-5/8" casing point above the Sag River
Sandstone. Run 9-5/8" 47# L-80 NSCC casing to within 5' of 12-1/4" TD and
cement with Class G cement around the 9-5/8" shoe. The annulus will be freeze
protected by dry crude.
Directional drill 8-1/2" hole to TD. Run 7" liner from TD to 250' inside
9-5/8" casing shoe and cement using Class G cement.
Clean out to 7" float equipment. Change over to NaC1 completion fluid. Test
casing and lap to 3000 psi.
Run 9-5/8" production packer on tubing. Set packer 50' above top of liner. A
subsurface safety valve with dual control lines will be placed 100' below base
of permafrost. Install and test Xmas tree with diesel. Displace tubing to diesel.
Set and test packer.
Shut down rig operations for perforating subsequent to rig release.
will be perforated in accordance with normal practices.
The well
Fluids incidental to the drilling of a well will be pumped down the 9-5/8" x 13-
3/8" annulus of the well.
Tubing Size: 4-1/2", 12.6# L-80 TDS TBG with 5 GLM's.
