Arco PBU DS 4 well 34
Prudhoe Oil Pool
Monday March 7,1994: I traveled to Arco PBU DS 4 well 34 to observe an
oil spill caused from a 1/4" diameter hole in well 34's flow line.
I spoke to Roy Wells Arco FS-2 drill site supervisor about the spill. He informed
me that the leak was discovered at about 11:52 AM. The spill was reported to
be approximately 150 bbls., 7 bbls. crude, 137 bbls. produced water, 150mscf
gas, the well has a 93% water cut and was flowing at 260 psi. The line
diameter was 6" and approximately 1/2 mile long, the leak occurred about 600'
from the well in a straight section of line 4" from a weld.
The safety valve system was in the normal operating position, and the sub
surface safety valve was in the well and fully operational. It is fairly evident the
line leak was small enough not to cause a pressure drop that would activate
the low pressure sensor.

BACKGROUND:
DS
4-34 Abandonment
DS 4-34 was initially spudded on 9/27/91. The well was kicked off at 500'
md/tvd with inclination angle being built to 56°-. The 10-3/4" casing was run and
cemented without incident at 3513' TVD/3970' MD. The intermediate 9-7/8" hole
section was then drilled to casing point at 13618' md. Intermediate 7-5/8" casing
was then run and cemented with an MTC cretaceous lead slurry and "G" tail
system. The intermediate 7-5/8" was set with emergency slips to reduce
potential rathole due to the high inclination of the wellbore and its related
sensitivity to shale instability. A 6-3/4" production hole was then ddlled to TD in
the well at 13,840'MD/8868' TVD. Open hole Icg evalution of the production
hole did not indicate producable oil column. A gyroscopic survey was run to
confirm the MWD directional coordinates - the results of which validated the
MWD. DS 4-34 was then temporarily abandoned by positioning an EZSV at
13543' MD and squeezing 8.5 bbls below- an additional 11 bbls of cement
was placed on top of the EZSV. The cement top was tagged at 13035' md and
10,000 Cf of stringweight was slacked off to confirm cement integrity ( witnessed
by AOGCC inspector Doug Amos). The well was then displaced to diesel from
2180' md to surface, an FMC dryhole tree was installed & tested and the 10-3/4"
x 7-5/8" annulus was freeze protected with 120 bbls of diesel. Nabors 28-E
released the rig 0900 hrs 10/20/91.
The re-drill of well DS 4-34 now designated DS 4-34 A required the placement
of an additional EZSV postioned at 11340'md (date of installation: 1-18-92). In
addition a cement plug will be placed in the well starting 100' md below the
proposed milled section ( milled section to extend from 10960' md to
11040'md), estimated at 11,140'md. A 5-1/2" production liner will be tied back to
the intermediate 7-5/8" casing with approximately 300' of lap. A "G" class tail
cement will be used for the primary cementing of the 5-1/2" 13 Cr liner. The
calculated displacement of the "G "tail slurry will bring the cement top to 200'
above the 5-1/2" liner top packer in the 3-1/2" x 7-5/8" annulus. This cement will
then be drilled up or reversed of the top of the liner top to provide access to the
5-1/2" liner to run the completion.

OBJECTIVE:
A temporay suspension procedure is to be developed for DS 4-34 to effect an operational
shutdown on the subject well per article 20 AAC25.~ of the Alaska Oil and Gas
Commision. , ~','~ ~,.o
WELL INFORMATION:
A temporary suspension of DS 4-34 was decided as the appropriate course of action for
the well after open hole logging information indicated that the Sadlerochit Lower Romeo
Zones (lA & lB ) were absent in this target location. A re-drill of DS 4-34 is being
proposed that would include the ~ of the intermediate hole to an alternate
target location.

1. M/U Halliburton EZSV to 3-1/2" DP
2. RIH to 13543'md ( 75' above 7-5/8" shoe)
3. Set EZSV @ 13543'md & establish injection. Note that fracture gradient is
estimated at 14.5 to 15.0 ppg mud wt. equivalent. Predicted surface pressure to
establish injection is estimated at 2300 - 2400 psig.
4. Start batch mix of 23 bbls (111.5 sxs ) of Dowell Schlumberger class G slurry.
Note to use existing mix fluid on location.
5. Sting out of EZSV.
6. Pump 18 bbls of class G cement.
7. Drop drill pipe wiper plug, follow with 65 bbls of rig mud.
8. Sting back into EZSV ( cement bottom in drill pipe -- 6.1 bbls/923 ft. short of end
of DP )
9. Pump 19 rig mud (6 bbls mud + 13 bbls of G cement ) to fill up open hole volume
Sting out of EZSV. Note: 5 bbls of cement in 3-1/2" drill pipe.

39 Note that derrick will need to be laid down for this rig move due to the pad level
elevation change between DS 4-34 and DS 4-39.

October,,,1.3, 1991 I made a rig/BOPE inspection on Nabor's Rig
28-E Drilling Well DS 4-34 for Arco in the Prudhoe Bay Unit.
I found the rig to be in good shape and all BOPE equipment in
place and operating. The surrounding location was neat and
orderly,
It should be noted that Arco has designated this drillsite as
a H2S location and therefore the quarters and Co. Man's office
are not on location. These are located on the adjoining
Drillsite #DS-11
In summary~ the rig/BOPE inspection on Nabor's Rig 28-E
revealed no non compliances and the rig should be in good
shape for drilling,

5. Pick up 9-7/8" BHA and RIH. Test casing to 3000 psi for 30 minutes. Ddl110' of new hole and
perform leakoff test.
6. Ddll 9-7/8" hole to 8734"1'VD, 13251 'MD,( 10' TVD above Sadlerochit top).
7. Set 7-5/8" Casing and cement same.
8. Pick up 6-3/4" BHA ,GIH. Test casing to 3000 psi.
9. Drill 10' of new hole and perform EMW test.
10. Drill 6-3/4" hole to TD @ 8861' TVD, 13464'MD, condition hole to run OH logs
11. Run wireline logs as programmed.
12. Set 5-1/2" liner and cement same.
13. Clean out the liner and test liner to 3000 psi for 30 minutes. Circulate well to brine. N/U Dry hole tree
and test to 5000 psi. Release Ddlling Rig.
14. Run cased hole logs.
15. Move in with Completion rig and run 4-1/2" tubing and packer assembly with GLMs,and SSSV. Test
tubing and casing to 3000 psi. Freeze protect with diesel.
16. N/U tree and test to 5000 psi. Release Completion rig.

NOTE;
Excess non-hazardous fluids developed from the well operations and as necessary to control the reserve
pit fluid level will be pumped down the 7-5/8" x 10-3/4" annulus. The annulus will be left with a
non-freezing fluid during any extended pedod. The annulus will be sealed with 300 sx of cement followed
by Arctic Pack.
b. Minimum distances of wells within 200 ft. of the proposed well am:
o DS 4-20 '59.06' @ 600' MD / 600q'VD
c. The drilling fluid program and surface system will conform to the regulations set forth in 20AAC25.033.
d. Maximum surface pressure is based on BHP and a full column of gas to TD (0.10 psi/ft).
e. Based on previous logs on DS 4, no potential productive zones am antidpated above 3500' TVD.
f. No potential hazardous levels of H2S am antidpated in this well.

1. CHECK AND FILL ACCUMULATOR RESERVIOR TO THE PROPER
LEVEL WITH HYDRAULIC FLUID.
2. ASSURE THAT THE ACCUMULATOR PRESSURE IS 3000 PSI WITH
1500 PSI DOWNSTREAM OF THE REGULATOR.
3. OBSERVE TIME, THEN CLOSE ALL UNITS SIMULTANEOUSLY AND
RECORD THE TIME AND PRESSURE REMAINING AFTER ALL UNITS
ARE CLOSED WITH THE CHARGING PUMP OFF.
4. RECORD ON THE IADC REPORT. THE ACCEPTABLE LOWER LIMIT
IS 45 SECONDS CLOSING TIME AND 1200 PSI REMAINING PRES-
SURE.
13-5/8" STACK TEST
1. FILL BOP STACK AND MANIFOLD WITH A NON-FREEZING FLUID.
2. CHECK THAT ALL HOLD DOWN SCREWS ARE FULLY RETRACTED.
MARK THE RUNNING JOINT WITH PREDETERMINED PLUG SEATING
DEPTH. RUN THE TEST PLUG ON DRILL PIPE AND SEAT IN WELL-
HEAD.
3. CLOSE ANNULAR PREVENTER AND UPSTREAM VALVES.
4. FUNCTION TEST CHOKES.
5. IN EACH TEST, FIRST HOLD 250 PSI FOR 3 MINUTES PRIOR TO THE
HIGH PPRERSSURE TEST.
6. INCREASE PRESSURE ON ANNULAR TO 3000 PSI AND HOLD FOR 3
MINUTES. BLEED PRESSURE TO ZERO.
7. OPEN ANNULAR PREVENTER AND CLOSE TOP PIPE RAMS.
INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 3 MINUTES.
BLEED PRESSURE TO ZERO.
8. CLOSE VALVES DIRECTLY UPSTREAM OF THE CHOKES AND OPEN
CHOKES. INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 3
MINUTES. BLEED PRESSURE TO ZERO.
9. OPEN TOP SET OF PIPE RAMS. CLOSE BOTTOM PIPE RAMS.
INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 3 MINUTES.
BLEED PRESSURE TO ZERO.
10. TEST REMAINING MANIFOLD VALVES (IF ANY) WITH 5000 PSI
UPSTREAM OF THE VALVE AND ZERO PRESSURE DOWNSTREAM.
11. BLEED PRESSURE TO ZERO. OPEN BOTTOM PIPE RAMS.
12. BACK OFF RUNNING JOINT AND POH. CLOSE BLIND RAMS AND
TEST TO 5000 PSI FOR 3 MINUTES.
13. BLEED OFF ALL PRESSURE. OPEN BLIND RAMS. FILL MANIFOLD
AND LINES WITH NON-FREEZING FLUID. SET ALL VALVES IN THE
DRILLING POSITION.
14. TEST STANDPIPE VALVES TO 5000 PSI.
15. CHECK DEGASSER OPERATION.
16. RECORD BOP TEST INFORMATION ON THE BOP TEST FORM SIGN
AND SEND TO DRILLING SUPERVISOR.
