INSTRUCTIONS General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Item 1: Classification of Service wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. Item 16 and 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Item 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). Item 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-in, Other-explain. Item 28: If no cores taken, indicate "none". Drilling & Wells Coiled Tubing Drilling Team Well 4-41 is scheduled for a through tubing sidetrack to be drilled with coiled tubing. The following summarizes the procedure required to prep 4-41 for drilling and is submitted as an attachment to Form 10-403. A schematic diagram of this proposed completion is attached. ]. The gas lift mandrels will be dummied off and a Mechanical Integrity Test is planned. 2. The well will be drifted to 2.8" id and a memory GR/CCL will be run with slickline. 3. Service coil will pump approx. 12bbls of cement to P&A the existing perforations. 4. The window will be milled off of a cement ramp with service coil. The KOP will be at approximately 9,676'md. 5. A sidetrack of approx. 1769 ft into lower Zone 1 will be drilled with CT. 6. The sidetrack will be completed with a cemented 2 3/8" liner that will overlap the existing 3 Y2" tubing to ,-,9480'md (TOL). The estimated Top of cement is planned for 188' above the window to the liner top. The coil tubing operations are expected to commence November 25. Drilling coil is expected to begin the sidetrack drilling on December 2, 2000. Thomas M. Excess non-hazardous fluids developed from the well operations and as necessary to con, trol the reserve pit fluid level will be pumped down the 7-5/8" x 10-3/4" annulus. The annulus will be left with a non-freezing fluid during any extended period. The annulus will be sealed with 300 sxs of cement followed by Arctic Pak. b. Minimum distances of wells within 200 ft. of the proposed well are: "DS 4-07: 100' @ 4200' MD/TVD "DS 4-08: 50' @ 2400' MD/TVD ~ DS 4-09: 126' @ 3600' MD/TVD c. The drilling fluid program and surface system will conform to the regulations set forth in 20AAC25.003. d. Maximum surface pressure is based on BHP and a full column of gas to TD (0.10 psi/ft ). e. Based on previous logs on DS#4, no potential productive zones are anticipated above 3500' TVD. f. No potential hazardous levels of H2S are anticipated in this well. · Co, AnCt~ora#e~' m~ni$$1en DS 4-41 MUD PROGRAM Depth Density ViS YP INT 10MIN FL OIL SOLIDS IVIJD (TVD) (PPG) GELS GELS cc/3omin (%) (%) TYPE SPUD 8.8 300 70 30+ 50+ 25 0 2 TO TO TO TO TO 10-3/4" 9.2 100 40 6 csg. pt.
