INSTRUCTIONS GENERAL-' This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. ITEM 1.' Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. ITEM 5-' Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Form 10-407 Rev. 07-01-80 R-25 DESCRIPTION OF WORK COMPLETED PRE-SIDETRACK CEMENT PLUG 12/17/97 A Cement Plugback was performed on the interval using the following fluids: SUMMARY 12/15/97: Rigged up coil and squeeze unit after snow removal. Shut down. 12/16/97: Bleed well down R-34 from 2200 psi to 300 psi. Picked up 7" riser spool to lubricate 5-1/2" XX plug. Pick up fishing assembly and run in hole. Attempted to latch plug but was unsuccessful until washing down to 9967' cement and then started jarring. Jarred 11 times and plug pulled free. Freeze protected surface lines and pulled plug to surface. Layed down riser spool and rig down for night. 12/17/97: Rigged up squeezing nozzle and rig in hole pumping water down coil at 2.0 bpm and closed chokes with 800 whp. Started mixing cement while open chokes holding 500 whp. Tagged TD at 10518' and jetted up 10,000 cemented. Staged 5 bbls water followed by 8 bbls cement w/aggregate, 9bbls cement, and spaced with water. Layed cement plug from TD to 10072' while holding 300 whp. Brought squeeze pressure up to 900 psi where it broke and backed down to 500 getting bbl for bbl returns. Then walked squeeze up to 2500 psi and got bbl for bbl returns. Shutdown and monitored and squeeze bled off 20 psi in 5 min. Switched to reverse out cement and reversed hole clean to 10148' cement.. Tried jetting deeper without success so pull out of hole and rig down for night. 12/18/97: Rigged up undereamer. PT cement plug to 1500 psi, monitor pressure for state test. Pressure bled off +/- 10 psi in 30 min. Run in hole and tag cement node at 10,098', hard cement at 10,154'. Reaming cement at 15 fph to 10,212'. Stopped reaming and circulate at min rate waiting for vac truck. Ream down to 10,245' at 15 fph. Waiting on water (shut down at water plant). Decide pooh and freeze protect. Resume reaming operation 12/19/97. 12/19/97: Rigged up undereamer. Rig in hole and tag hard cement at 10,253'. Ream cement at 15 fph w/4000 lb on undereamer. Switch to pump Biozan sweep after 50' reamed. Could not get pump prime, switch back to water and resume reaming. Ream down to 10,320' Trip up to original TOC and run back to bottom, hole open. Pump 55 bbl Biozan sweep for hole cleaning. R-25, Pre-Sidetrack Ceme,,, Job Page 2 12/20/97: Drop ball 10 bbls into Biozan sweep. Open circ sub and begin pull out of hole at 80% of fluid annular velocity. Pump diesel to freeze protect well from 2200 ft. to surface. Finish pull out of hole. Begin rig down. Move off location. Critical Well Information 7/5-1/2", 26/17# L80 Tubing 7", 26#, L80, Liner I.D. = 6.276"/4.892" I.D. = Capacity- 0.0232 bbl/ft Capacity- SSSV Landing Nipple: 2,112' MD PBTD (7"): Minimum ID (4.455" I.D.): 9,972' MD Perforations: Proposed TOC: _+10,298' MD 6.276" 0.0383 bbl/ft 11,884' MD 10,259'-10,309' MD 10,415'-10,505' MD 10,515'-10,575' MD Maximum documented hole inclination is 39° at 4,915' MD. Angle at the proposed KOP inside the 7" liner (_+10,255' MD) is _+36°. The well has a 10' cement cap above 20/40 carbolite from PBTD to 10,522' which covers some of the lower set of perforations Notify an AOGCC representative prior to commencing P&A operations. Pre-Riq Work: NOTE: If any problems with the 9-5/8" packoff are observed during pre-rig work, contact the wellhead manufacturer to initiate repairs. If repairs are not possible, the TTRD sidetrack will be halted, and an alternative sidetrack method used. This well is an injector, and the existing perforations will be thermally fractured. This means that the perforations will likely drink most or all of a standard cement plug. For this reason, it is recommended that a 20ppb aggregate cement plug is pumped until some squeeze pressure is obtained. 1. If tubing and annulus have not recently (<1 month) been tested R/U wireline and retrieve SSSV. Set tubing plug in SWN nipple. Perform MIT on tubing and annulus to 4,000psi. Retrieve tubing plug and POOH. 2. RIH and drift tubing to plug back TD with drift to ensure whipstock (3.635" OD) pass through of minimum 3.8" OD. . R/U CT unit and RIH to PBTD at 10,522' (elm) with cementing BHA. Spot a balanced cement plug w/20ppb aggregate (crushed brick) from base of the well to 10,200' (open perforations from 10,259' to 10,522'), pumping against a closed choke until 1,000psi squeeze pressure is seen. Attempt to squeeze 5bbls of cement to perforations. Run back in hole to _+10,298', circulating out excess cement with contaminant to obtain the required TOC at _+10,298'. POOH. NOTE: If necessary to prevent loss of cement, circulate well to a diesel cushion sufficient to remove hydrostatic overbalance from the cement column. 4. Pick up underreamer sized for 7" 26# casing, and underream interval from 10,222' to 10,298' to ensure good casing for whipstock to set in. Tag TOC as required for AOGCC representative and POOH. NOTE: The under-reamer will be working across a perforated interval and hence care must be taken not to damage or lose any parts of the under-reamer arms 5. R/U electric line. Set Baker through tubing whipstock oriented highside with top of whipstock at 10,252'. This will put the top of the whipstock 3' MD below the base of the Kingak formation which is known to be R-25i Sundry P&A Proceaure Page 2/2 troublesome. Do not set the top of the whipstock any higher than 10,252'. Please have a Baker representative on site when setting the whipstock. R/D electric line. If cement plu.qback or whipstock settinq problems are encountered~ please contact SSD Ops Drilling Engineer immediately for possible alternative procedures. . M/U 4.5" OD milling assembly and RIH. Mill out SWN nipple at 9,972' to 4.5". RIH, tag whipstock/liner and mill out/dress window to 4.5". Do not mill out more formation than is needed to bury the window dressing mill. 7. If any problems with the 9-5/8" packoff are observed, contact the wellhead manufacturer to initiate repairs. If repairs are not possible, the TTRD sidetrack will be halted, and an alternative sidetrack method used. 2. MIRU. Pull BPV and set/test TWCV. ND tree, NU BOP's and test same. Pull TWCV. 3. RIH with 4.5" drilling assembly (circulate out freeze protection) and tag TD of new hole. Displace well to Quikdriln drilling fluid, and drill 4.5" hole section to TD of 13,081' as per directional plan. 4. I received no notice of this test, and only by chance happened along at the time they . w~. testing John Solario, BP rep, in the past has given me notice of upcoming BOP tests; and I felt that it. was not appropriate to require the test be started over this time. All BOPE equipment tests were observed with the exception of the choke manifold and H~RC ~ 'valves. There were no failures noted, and I found housekeeping at 'this rig, inside and out, to be a top priority. In smnmary, I witnessed tim weekly BOP test on Nabor's Rig 28-E. The Drilling fluid program will consist of an 8.5 - 9.5 ppg spud mud in the 16" hole, a 9.0 - 9.7 ppg water base mud in the 12-1/4" hole and an 8.6 ppg water based mud in the 8-1/2" hole. Drill 16" hole to 3195'. Run and cement 13-3/8", 68 #, L-80 Buttress casing to surface. Install blowout prevention equipment consisting of 5000 psi W.P. pipe rams, blind rams, and annular preventer. Maximum possible surface pressure is 3330 psi based on a gas column to surface from 8600' TVDss and an oil column to 8800' TVDss.
