INSTRUCTIONS
General: This form is designed for submitting a complete and correct well completion repod and Icg on
all types of lands and leases in Alaska.
Item 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the
data pedinent to such interval.
Item 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
Item 23: Attached supplemental records for this well should show the details of any multiple stage cement-
ing and the location of the cementing tool.

DESCRIPTION OF WORK COMPLETED
PLUG & ABANDONMENT OPERATIONS
Well 5-15A was Plugged & Abandoned during Well 5-15B liner setting operations. A new 1473'
horizontal extension wellbore was drilled out the shoe of 5-15A to a new 5-15B target location.
The open perforations existing in 5-15A before drilling to 5-15B were located at:
10770 - 11070' MD Ref. Log: CET 8/19/93.

Drilling of the new wellbore to 5-15B started 3/21/96. Total depth was reached at 12550' MD,
and blank 2-7/8" liner was run & cemented across the perforation interval (above) on 3/28/96.
30 bbls of Latex Acid Resistive Cement was pumped to set the liner at 9969 - 12500' MD, which
left an estimated top of cement behind the liner at 10059' MD.

Intended Procedure
This intended drilling procedure for well 5-15B is also being
submitted as an intended P&A procedure for 5-15A. Well 5-15A
will be P&A'ed by isolating the producing perfs behind a cemented
liner. Therefore, 5-15A will not be P&A'ed until the drillout is
finished and 5-15B has been completed with a cemented liner.
Intended Procedllre:
Mill existing 7" float collar/shoe and 10' new formation.
Perform FIT.
Directionally drill a 3-3/4" hole to the target sands. Drill
approximately 950' of horizontal well in the Romeo
sands. Obtain GR and directional information from MWD.
Run 2-7/8" FL4S blank liner from TD to a minimum of
100' above the existing perforations in the 7" liner.
Cement liner in place bringing top of cement up to a
minimum of 100' above the existing perforations in
the 7" liner, isolating/abandoning 5-15A perforations.
Perforate selected intervals.
· Return well to production.

CT Sidetrack and Completion
Intended Procedure
NOTE: This work will be performed following the rig sidetrack of this
well as per State Permit to Drill # 93-97.
Drill approximately 800' MD of formation out of the
bottom of the 7" liner. Run pre-perforated 2-7/8" liner
in the open hole as necessary.

Install GLV's and lift off well to test the integrity of the liner cement
job. If remedial cementing is required, it will be performed with
CTU prior to CTU drfllout.
Rig up CTU, manifold, lines and Trico skid. Nipple up BOP's and
test.
CTU directionally drill +800' of horizontal section with MWD
logging for GR and directional survey. Gas lift will be used to
achieve + 1000 psi underbalance while d~lling.
Run 2 7/8" preperforated and plugged liner below a 2 7/8" by 4
1/2" packer. Set packer in the 4 1/2" tubing tail.
POOH and RD CTU.
After rigging down CTU, bullhead acid through check valves on
bottom of the preperforated liner to dissolve perforation plugs.
7. Put well on production. Run SSSV.

Monday July 12, 1993: I contacted Unocal representative Bill Meyers this date to set up an
inspection of Platform Bruce production equipment and to witness the SVS test on their Gas
well, which is the only well on the platform that has a SVS installed. I was informed by Bill that
Rig 150 was nippling up the BOP stack on 13 3~8" csg on well 42-2 and would be testing later
this date.
Tuesday July 13, 1993: Due to problems with the 13 3/8" csg head the BOPE test was
delayed until 3:30 p.m. this date. While waiting on the test to commence I conducted an
inspection of the production equipment, the drilling rig onboard the platform and witnessed the
SVS test on their gas well. With the exception of the three failures indicated on the attached
AOGCC BOPE Test Report, I found all equipment in good repair and in compliance. The
drilling and production crews were knowledgeable about their equipment and the AOGCC test
procedures.
As the attached AOGCC BOPE Test Report indicates/the Inside Kill Line Manual Valve, the Port
Hydraulic Choke Isolation Valve failed, the Gas Detectors were out of calibration and there was
no Visual High/Low warning lights installed. The Kill Line Manual Valve was repaired and
retested this date, parts had to be order for the Choke Manifold Valve failure as well as the Red
and Amber Strobe Lights for the High/Low Visual Alarm. Because there was an Audible Alarm
connected to the Low Level Gas Alarm which sounded when 20% LEL was detected and since
there was another block valve upstream of the Port Hydraulic Choke and two block valves
downstream I allowed Marty Lynch to continue with drilling operations and instructed him to
contact Blair or myself when repairs had been completed. I recommend we conduct another
inspection in the near future to ensure compliance of these non compliance items.
Tuesday July 20,1993: I contacted Unocal representative Mike Freedmont and was informed
that the Gas Detection System was repaired and that on 7/21/93 the Choke Manifold Valve
failure would be corrected.
IN SUMMARY, witnessed BOPE test on Unocal Bruce Rig 150 Well 42-2, Test time 5 hrs., 20
valves, 3 failures.

Objective:
DS5-15 has been selected for a redrill as it has a confirmed 20' channel up
behind the production liner and a failed production packer that would
otherwise require an extensive workover.
The primary objective of the DS 5-15A redrill is to return the well to
production by drilling a new hole to a location approximately 900' NE of the
current BHL. Targeting Sadlerochit Zone 1, Lower Romeo sands, this
redrill will improve reservoir performance through maximum stand-off from
gas in a thin light oil column. Hole angle will reach 91 ° at TD to allow
coiled tubing to drillout a horizontal section after the rig moves off location.
The horizontal extension will be drilled with coiled tubing and is not within
the scope of this request.
Mud Program:
A 9.5 - 10.00 ppg Biozan polymer/KC1 water based mud will be used.

starting at +/- 8700' MD, Prior to starting section cut, spot an 18 ppg pill
from PBTD to the base of the proposed section.
RIH with open-ended DP. Mix and pump a minimum of 165 ft3 of cement
to spot balanced kick-off plug from 100' below the base of the section to
200' above the top of the section.
Run in with an 8-1/2" steerable drilling assembly w/MWD and dress the
cement to 5' below the section top. Kickoff and directionally drill to the
target at 8775' TVDbkb as per the attached directional plan. Obtain GR
Icg from top of Sadlerochit formation to top of Zone 2. Obtain GR / /
NeutrQn / Density from the top of Zone 2 to the 7" casing point.
Run 7" 29# L80 NSOC standard liner assembly, with liner top packer, to
TD with +/- 150' lap.
Cement liner with 458 sx class G cement.
Re-test BOP's as needed.
Run in hole with 6" bit.
Clean out shoe joint and drill approximately 10' Qf 6" openhole.
POOH and pick up a 7" casing scraper spaced 30' from bit. RIH to TD
and change over to clean inhibited seawater.
Retest 9-5/8" wellhead packoff and replace as needed.
RIH with 4-1/2" 12.6# completion string w/7" packer. Space out GLM's
and jewelry as indicated in the "post rig diagram". Packer should be
placed 500' above the 7" shoe with the tailpipe 50' above the shoe.
Maintain a minimum ID of 3.813" in the completion...Do not run an XN
Nipple. Drop ball and set the packer.
Test down tubing to 4000 psi. Bleed tubing pressure down to 1000 psi
and hold. Test tubing x casing annulus to 2000 psi for 5 minutes to ensure
packer is set. (The reason for not testing to 3500 psi is to avoid the
possibility of shearing out the Iockdown screws that hold the tubing hanger
in place. After the tree is nippled up, then test the backside to 3500 psi for
30 minutes while holding 1000 psi on the tubing to avoid shearing the RP
valve.)
Set BPV. ND BOP's. NU the adapter flange and 5-1/8" 5M tree. Install
tree test plug and test to 5000 psi with diesel. Pull tree test plug and
BPV.
Pressure up down the tubing to 1000 psi. Test tubing x casing annulus to
.3500 psi for 30 minutes.

No depth adjustments were made because this TDTP log
logged 26-APR-94 has been determined to be the base GR
for this well. Note that because of numerous telemetry
tool response problems, this processed EDIT is presen
ted somewhat unconventionally. In order to come up
with at least one good Base pumping pass, the best
portions of two of the Base passes was used to create
a spliced data set that was used for an 'averaged' set.
