FOR SIDETRACK CEMENT PLUG
2/17/96
A Cement Plug Lay was peformed on the interval using the following
fluids:
SUMMARY
RIH w/1.75" coil and 3.5" nozzle. Loaded well with 200 bbls SW. Tagged TD.
PU at 13687'. Layed in a 13 bbls cement plug from TD. Built 1000 psi max
squeeze pressure. POOH maintain -800 psi on well. Pumped a foam pig to
clean coil due to reverse out yesterday and cement job today. Freeze protected
coil and freeeze protected wellbore to 100' only. Ready for pressure test and E-
line tag. Left well with 770 psi WHP.Flowline is not freeze protected.
Estimated Cement top: 13,370'.

Plug and Abandonment
Dummy the GLMs and pull the 4-1/2" SSSV. Valve is set at 2219' MD. Test the 9-5/8"
mandrel packoff to 5000 psi.
Notify an AOGCC representative prior to commencing plug-back operations. RU CT unit.
RIH with coiled tubing to 13656' MD - last official tag of PBTD (bottom perforation at 13620'
MD). Lay a cement plug from 13656' to 13400' MD. This will leave cement about 100'
above the top perforation (top perforation at 13500' MD). Planned kickoff depth for sidetrack
is 13265' MD.
Total volume of cement should be 10 bbls.
With an AOGCC representative on location to witness the operation, R/U E-line and tag the
top of the cement to verify cement is at least 100' MD above perfs.
RIH and cut the 4-1/2" 13Cr-80 tubing with a chemical cutter in the middle of the first joint of
tubing above the 7" packer at approximately 13128' MD.
Displace the tubing and wellbore to 8.5 ppg seawater.
Freeze protect top 2200' MD with diesel.
Bleed tubing and annuli pressures to zero.
Set BPV in the tubing hanger and test from below to 1000 psi. Secure well.
Remove the well house and flowlines. If necessary, level the pad and dig out the cellar.
Please provide total well preparation cost on the handover form.

11/13/94 A Mud Acid Treatment was performed to remove scale using the
following fluids:
SUMMARY
November 11, 1994
MI & RU Nowcam 4905 & Trico FPU. Pumped Styrofoam plug and cleaned
out CT. RU Packer assembly. Pressure test. Pumped 54 bbl of FSW down
the Wellbore. At 10 bbl pumped, the WHP increased rapidly to 2900 psig,
slowed down the pump rate 1.5 bpm and the WHP = 2400 and climbing.
Pumped a total of 54 bbl FSW and SD. Bleed WHP down to 0 psig in 1 min.
Wellbore is full from the initial CTU workover. Set counters and TIH
pumping FSW at 0.5 bpm. set baker thru tbg. spot & treat packer, pump
1000 gal 85% xylene 15% methanol ,3500gal 15% hcl / 10% xylene, 3500
gal.9:l mud acid. ,5000 gal. 2% NH4CL H20 overflush,200 gal lppg divert 1
November 12, 1994
Pumped 2nd stage as per program at 1.25 bpm at 1960 psig.
Flushed with 33 bbl of FSW Final Qp =1.17 bpm at 1910 psig. WHP
SD, unset packer, POOH and FP the CT with 40 bbl of MEOH. OOH,
Recovered all of the packer element. Called PO to put well on
production.

Wednesday, October 6, 1993 ! witnessed the initial BOP test on Doyon rig #14 in the Prudhoe
Bay field.
There were two failures observed¢ these being the manual choke valve on the stack and the
lower Kelly valve. Both were replaced and tested "OK." I found the crew's testing procedure to
be quick but thorough and showing evidence of regularity.
The rig was quite clean as was the area surrounding it. Access to the far side of the rig was
tight but adequate.
SUMMARY; I witnessed the initial BOP test on Doyon rig #14 in the Prudhoe Bay field with two
failures observed these being the manual choke raj,ye on the stack and, the lower Kelly valve .....
Test time 2.5 hours.

referenced well.
The permit to redrill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and does not
authorize conducting drilling operations until all other required
permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with
20 AAC 25 035. Sufficient notice (approximately 24 hours) must be given
to allow a representative of the Commission to witness a test of BOPE
installed prior to drilling new hole. Notice may be given by contacting
the Commission petroleum field inspector on the North Slope pager at
659-3607.

NOTE: There is no completion string in this well. According to records the well
has a tubing hanger with 1 joint of tubing below the hanger.

rig floor and LD same. PU BOPE test plug. Test BOPs to 5000 psi.

RIH with a 9-5/8" section mill and cut a 50' section in the 9-5/8" casing starting at
+/- 1. 2,050' MD. Prior to starting section cut, spot an 18 ppg pill from PBTD to the
base of the proposed section.
RIH with open-ended DP. Mix and pump a minimum of 167 sacks of cement to spot
balanced kick-off plug from 100' below the base of the section to 200' above the top of
the section.
RIH with 8-1/2" steerable drilling assembly and dress off cement to .5' below section.
Kickoff well and directionally drill to align well with target.
Drill with an 8-1/2" packed hole assembly to total depth of 13.853' MD/9.149' TVD
which is +150' MD below OOWC.
RU Atlas wireline and Icg with the GR/SP/DIL/BHCS/CNL. Make wiper trip depending
on hole conditions and logging time.
NOTE: If it is felt that wireline logs will not be able to go down SPERRY SUN is to
be prepared to run a MAD pass.
Run 7", 29#, 13CR-80 X LBO, NSCC standard liner assembly to TD with liner top
packer and +/- 150' lap.
NOTE: Run 13CR from TD to + 13,000' MD. Run L-80 from 13,000' md to
150' inside the 9-5/8" at +11,900' md.

pressure down to 1000 psi and hold. Test tubing x casing annulus to :~500 psi.
Shear RP valve and freeze p}otect, set BPV. Release rig.
