GENERAL-' This form is designed for submitting a complete and correct well completion report and log on all types of lands and
leases in Alaska.
ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water
supply for injection, observation, injection for in-situ combustion.
ITEM 5.' Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces
on this form and in any attachments.
ITEM 16 AND 24.' If this well is completed for separate production from more than one interval (multiple completion), so state in item
16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional

PRE-SIDETRACK CEMENT PLUG
01/12/98
A Cement Plugback was performed on the interval using the
following fluids:
SUMMARY
Move in rig up DS CTU #2. Blow out preventer test. Rig up and test surface
lines. Pick up bottom hole assembly #1 (CS fishing assembly). Run in hole.
Latch XN plug at 11514', pulled out of hole. Recovered fish and lay down. Pick
up jetting nozzle. Run in hole, pump 578 bbls seawater to kill well. Reciprocate
and jet with 33 bbls diesel/Xylene mix followed by seawater from 11688' to
12054'. Tag TD at 12054'. Pump mud and Diesel/Xylene mix into perforations.
Pull out of hole. Freeze protect coil and backside. Pick up cementing nozzle.
Run in hole with cementing nozzle. Pump 20 bbls. Class 'G' cement from TD at
12,050' to tubing, tail at 11,528'. Apply 1000 psi squeeze pressure. Approx.
one bbl squeezed. Jet contaminated cement with biozan down to 11,743'.
circulate contaminated out the long way while holding positive pressure on
squeeze. Pulled out of hole. Freeze protect tubing to 2000' with diesel. PT
squeeze to 1000 psig for 5 min. - good. Left well shut in with 400 psi on well.

Objective
The objective of the pre-rig work is to ensure tubing and casing integrity for the rig TTRD procedure, P&A the
existing perforations, set a whipstock, mill a casing window, and freeze protect the well in preparation for a
Through Tubing Rotary Drilling Horizontal Sidetrack.

Pre-Rig Work:
NOTE: If any problems with the 9-5/8" packoff are observed during pre-rig work, contact the wellhead
manufacturer to initiate repairs. If repairs are not possible, the TTRD sidetrack may be halted, and an
alternative sidetrack method used.
This well is an injector, and the existing perforations will be thermally fractured. This means that the
perforations will likely drink most or all of a standard cement plug. For this reason, it is recommended
that a 20ppb aggregate cement plug is pumped until some squeeze pressure is obtained.
1. If tubing and annulus have not recently (<1 month) been tested R/U wireline and retrieve SSSV. Set tubing
plug in SWN nipple. Perform MIT on tubing and annulus to 4,000psi. Retrieve tubing plug and POOH.

R/U slickline, and drift tubing to plug back TD at 12,059' (elm) to ensure access to lower perforations. Drift
tubing to SWN nipple at 11,781' with drift to ensure whipstock (3.635" OD) pass through of minimum 3.8"

R/U CT unit and RIH with cementing BHA. Jet liner across the proposed cement interval in order to
remove any debris (consider using "schmoo-be-gone"). Spot a balanced cement plug w/20ppb aggregate
(crushed brick) from base of the well to -11,500' (open perforations from 11,696' to 11,912'), displacing
with diesel against a closed choke until 1,000psi squeeze pressure is seen. Run back in hole to _+11,743',
circulating out excess cement with contaminant to obtain the required TOC at _+11,743'. POOH. Let the
plug set up, then tag with slickline.
NOTE: If necessary to prevent loss of cement, circulate well to a diesel cushion sufficient to reduce the
hydrostatic overbalance on the cement column.
4. Pick up underreamer sized for 7" 26# casing, and underream interval from 11,600' to 11,743' to ensure
good casing for whipstock to set in. Tag TOC as required for AOGCC representative and POOH.
NOTE: The under-reamer will be working across a perforated interval and hence care must be taken not
to damage or lose any parts of the under-reamer arms

R/U electric line. Set Baker through tubing whipstock oriented highside with top of whipstock at 11,695' set
for a kickoff to exit the casing at 11,698' (the base of the whipstock should be at approximately 11,713').
This will put the top of the whipstock 9' MD below the base of the Kingak formation which is known to be
troublesome. Please have a Baker representative on site when setting the whipstock. R/D electric line.

M/U 4.55" OD milling assembly and RIH. Mill out SWN nipple at 11,514' to 4.55". RIH, tag whipstock/liner
and mill out/dress window to 4.55". Do not mill out more formation than is needed to bury the window
dressing mill.
7. Circulate kill weight fluid and freeze protect with diesel (estimated BHP at 8800'TVDSS is 3,500psi -
7.6ppg EMW) to depth of ___2,200'.
8. Set and test BPV. Remove wellhouse and flowlines, level pad, dig out cellar
Summary Ri.cl Procedure
1. Prior to rig move, test 9-5/8" packoff to 5,000psi, and function lock down screws (FMC wellhead
representative to do this). If any problems with the 9-5/8" packoff are observed, contact the wellhead
manufacturer to initiate repairs. If repairs are not possible, the TTRD sidetrack may be halted, and an
alternative sidetrack method used.
2. MIRU. Pull BPV and set/test TWCV. ND tree, NU BOP's and test same. Pull TWCV.
3. RIH with 4.5" drilling assembly (circulate out freeze protection) and tag TD of new hole. Displace well to
Quikdriln drilling fluid, and drill 4.5" hole section to TD of 17,565' as per directional plan.
4. RIH with DP(:; logs and make logging run across open hole section from window to TD.
5. M/U and run 6,135' of 3-1/2" solid L80 ST-L liner to hang off depth in 7" tubing of +11,430'. Cement liner in
place with inner string cementing assembly, and set liner hanger/packer. Circulate out excess cement, and
displace well to clean seawater. POOH.
6. RIH with DPC CBL log and make logging pass over the liner to determine condition of cement job. POOH.
7. RIH with DPC perforating guns and perforate 50' of liner.
8. RIH with DP and circulate freeze protection to +2,200'MD.
9. Set and test TWCV, ND BOP, NU tree. Pull TWCV and set/test BPV. RDMO.

Monday, November 22,. 1993: I traveled to BPX's R Pad, well 32, this date
and witnessed the BOPE.test on Nabors rig 28 E. As the attached BOPE Test
Report shows there were no failures.
The ram type preventers were changed out on the BOP stack on this hole
because of wear on the'old ones, which will be sent back to the rebuild shop
for repairs.
The rig was in good shape and the location was clean and all equipment was
arranged neatly for easy access to the rig and surrounding, area.
Summary: I witnessed the BOPE test on Nabors rig 28E- no failures.

Thursday November 18, 1993: I traveled to BPX PBU well R- 32 to witness a
diverter test on Nabors 28E
Nabors has 2 hydraulic knife valves on their vent lines next to the diverter and
also a manual knife valve on 1line outside the rig sub-base. Nabors toolpusher
Mike Downey indicated to me that they were going to be changing the manual
valve to a hydraulic knife valve and placing it inside the rig sub-base so it
would stay warm.

The material and equipment on the pad was well laid out and plenty of room
was made for any emergency services.
Summary: I witnessed the diverter test on Nabors 28E--no failures

Dear Mr. Maskell:
Enclosed is the approved application for permit to drill the above
referenced well.
The permit to drill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and does not
authorize conducting drilling operations until all other required
permitting determinations are made.
The blowout prevention equipment (BOPE) must be tested in accordance
with 20 AAC 25.035; and the mechanical integrity (MI) of the injection
wells must be demonstrated under 20 AAC 25.412 and 20 25.030(g)(3).
Sufficient notice (approximately 24 hours) of the MI test before
operation, and of the BOPE test performed before drilling below the
surface casing shoe, must be given so that a representative of the
Commission may witness the tests. Notice may be given by contacting the
Commission petroleum field inspector on the North Slope pager at
659-3607.
