INSTRUCTIONS
General: This form is designed for submitting a complete and correct well completion report and log on
all types of lands and leases in Alaska.
Item 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the
data pertinent to such interval.
Item 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).

PRE-SIDETRACK CEMENT PLUG & ABANDONMENT
8/30/96:
MIRU CTU & PT equipment. RIH w/CT & loaded the wellbore w/Hot '1% KCI Water.
Tagged landing collar at 10402' MD & jetted OOH w/Hot 1% KCI Water.
8/31/96:
RIH w/cementing nozzle to Plug & Abandon the open perforation intervals at:
9626 - 9736' MD
9760 - 9910' MD
9940 - 10058' MD
10106 - 10138' MD
10300 - 10362' MD
Ref. Log: CNL 6/17/94.
Pumped 2 bbls Neat Class G cement followed by 26 bbls Class G cement w/fibers to put estimated
TOC at 8950' MD. Squeezed cement to 500 psi; estimated no cement was placed behind pipe.
Contaminated cement w/biozan & jetted contaminated cement from wellbore down to 9300' MD.
POOH & shut in well w/500 psi, WOC. RIH w/milling BHA to mill out the 3.725" XN Nipple at
9276' MD to 3.8". Started milling XN Nipple.
9/1/96:
Completed milling through the XN Nipple. PT the cement plug to 2000 psi; OK. RIH to tag TOC
at 9322' MD, then milled a pilot hole to 9390' MD. POOH w/BHA. MU & RIH w/underreamer
& reamed out liner cement to 9415' MD. Pumped 32 bbl biozan pill & POOH w/BHA. RIH
w/cementing nozzle to TOC & jetted warm Fresh Water, followed by 2 bbls Neat Class G cement &
8 bbls Class G cement w/fibers. Squeezed cement to 1500 psi, then bled & circulated biozan to
contaminate cement. Jetted excess cement from wellbore to 9300' MD, then jetted OOH & WOC.
9/2/96:
WOC. RIH w/pilot mill & tagged TOC at 9308' MD.
Continued w/Coil Tubing Sidetrack Drilling operations to 5-10B (9/2/96).

The following information was derived from in house evaluations of well cuttings caught at the
Nabors 2ES rig site during the drilling of well 5-10A. The lithological evaluation was performed
for the Horizontal / Slimhole section.
The Engineer and Sample Catcher on location performed their duties on a twelve hour tour basis.
The method of lag calculations used for depth tagging the samples were derived from borehole
and surface equipment geometry. This calculation was checked by use of u-circulated calcium
carbide pills.
Sample evaluations were performed by W.A. Shepherd, Sperry-Sun Drilling Services'
Geologist/Logging Engineer, inhouse at our Anchorage lab. W.A. was able to give an
interpretive analysis of lithology by utilizing FE/MWD data recorded from the wellsite.

TueSdav..Julv '13.. 1994.: I traveled to Nabors rig # 2-ES to witness the weekly BOP
teSt on their rig which is presently drilling Arco well # DS 5-10.
The rig was ready when i arrived. The test went very well with all components
functioning properly and holding its pressure test. The test lasted 3 hours~and I
found Charles Henley's (Nabors toolpusher) test procedure to insure a gdod test of
the BOPE.
I found the overall condition of the rig to be in outstanding shape.
Summary: I witnessed the weekly BOP test on Nabors rig # 2-ES. Test time 3
hours. No failures.

Enclosed is the approved application for permit to redrill the above
referenced well.
The permit to redrill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and does not
authorize conducting drilling operations until all other required
permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with
20 AAC 25 035. Sufficient notice (approximately 24 hours) must be given
to allow a representative of the Commission to witness a test of BOPE
installed prior to drilling new hole. Notice may be given by contacting
the Commission petroleum field inspector.on the North Slope pager at
659-3607.

Objective:
The objective of this sidetrack is to drain the current reserves by drilling a
horizontal replacement wellbore south-east of the current wellbore. The
well has a 1987 cement squeeze which has repeatedly broken down, and
no feasible fix has been found. A replacement wellbore will allow the
reserves originally targeted by well 5-10 to be produced. Drilling this well
horizontally will drain the reserves more effectively. The zone 1 oil column
is expected to have 60' NLOC. Post-sidetrack rates are estimated at 1000
BOPD, 1000 SCF/STB and 10% WC.

After dressing off the kickoff plug we will displace to Flopro system
(Xanvis) and drill to 7" shoe depth with @ 10.00 ppg to 10.20 ppg. 6"
hole will be drilled with the same system with @ 9.2 ppg to 9.4 ppg.

Pre-rig work: Kill well. Lay cement plug across perforations to at least 100'
above top pedoration. Cut the 4-1/2" tubing near 8015' MD. Freeze protect and
set a BPV. Test intermediate casing pack-off to 5000 psi.
Move in drilling rig. Test 9-5/8" intermediate casing pack-off if not done during
pre-rig work.
Puli BPV. Circulate diesel freeze protection cap from well and recover for future
use.
Install a BPV and test tree plug. Remove the production tree.
Install BOP's and test to 5000 psi. Remove the test plug and BPV.
Puli and lay down the 4-1/2" tubing.
Spot a 18 ppg pill on bottom and cut a 50' section in the 9-5/8" casing starting
from about 7700'MD.
Spot a balanced cement kick-off plug (180 cubic feet minimum) through open-
ended drilipipe from 100' below the base of the section to 200' above the top of
the section.
Run in with an 8-1/2" Steerable drilling assembly and dress the cement to 5'
below the section top. Kickoff and'directionally d-rill to the target with 90° hole
angle at 8940'ss as per the attached directional plan. Obtain Direction ! GR /
Resistivity / Neutron / Density formation evaluation logs while drilling starting
from the top of the Sag River Fm.
Run a 7" 29 # 13cr liner assembly to bottom in the 8-1/2" hole and up to 100"
above the top of sand followed with 7" 29# L-80. Space the liner hanger (L-80)
and top isolation packer (L-80) about 150' up inside the 9-5/8" casing.
Cement the liner in place with the total cement volume based upon 60 % open-
hole enlargement plus the 80' shoe volume, 150' lap volume and 150' excess
on top of the liner with drillpipe in the hole.
Re-test BOP's as needed.
Run in with 6" steerable drilling assembly with direction / GR MWD on 4-1/2" x 3-
1/2" drillpipe. Test casing to 3500 psi for 15 min. before drilling out float shoe
and 10' of new hole.
Displace well with non-damaging (Fig-Pro Xanvis polymer) drilling fluid. See
MI Drilling Fluids recommended displacement procedure.
Drill approximately 8101 of 6" hole to TD as per Geologist / Operations Engineer
onsite.
Make a conditioning trip with a 6" hole-opener for the open hole section with a
7" casing scraper spaced to clean the liner.
Install a .4.-1/2" NT-80S slotted liner with 4-1/2" X 7" packer / hanger. Run in on
drilipipe with a 2-1/16" wash string inside the slotted liner. Space packer /
hanger to be set within about 150' of the 7" liner shoe.
Position packer above the 7" liner shoe and circulate. Set packer and test.
Release from the packer / hanger and displace the drilling fluid with filtered
seawater but no corrosion inhibitor. After circulating clean with seawater, spot a
Xanvis mud breaker in the horizontal hole section.

Pull the wash string up just above the hanger and circulate out Xanvis mud
above the slotted liner with filtered seawater. Pull out of the hole and lay down
the wash string.
Retest 9-5/8" wellhead packoff and replace as needed.
Run the 4-1/2" L-80 x 3-1/2" L-80 completion string assembly. Space out and
circulate with corrosion inhibitor before setting the tubing packer.
Install a BPV and test plug. Remove BOP's.
Install and test the production tree to 5000 psi. Pull tree test plug and BPV.
Test the tubing x casing annulus to 3500 psi for 30 minutes.
Shear RP valve in the upper GLM and freeze protect the well.
Set a BPV. Release rig.
Complete appropriate documentation and hand-over to Production
