SUMMARY SIDE-TRACK PROCEDURES Pre-Rig Work: See Wellbore Diagram, 4 W' Tubing Must be cut below 8900' for side-track(Kick- off-point, KOP) operations. * Notify the field AOGCC representative with intent to plug and abandon* 1. "S" MIRU Slickline. Drift/tag liner top packer tailpipe SN "XN" @ 10524" MD. Note collapsed 5 Y2" liner @ 11,960' MD. 2. "O" Load and Kill IA and tubing. Perform injectivity test to verify ability to pump into the well. 3. "O" Plug & abandon w/15.8 ppg Class G cement. Overdisplace to perfs with seawater and diesel for freeze protect. Place 5 bbls minimum behind pipe into perf's. 4. "O" Max TOC is @ 8900' to allow tubing to be cut and sidetrack operations to be conducted. Minimum TOC is 100' above top perforation. "S" Drift/Tag Top of Cement, TOC, test, and document TOC per AOGCC requirements. "C" If TOC is higher than KOP, MIRU coil unit with 4.5" mill to clean up cement stringers. Re-test cement plug and record on chart. "O" Test the 7 5/8" casing annulus to 3500 psi against the packer. Hold 1000 psi on the tubing to prevent from collapsing the tubing. "E" MIRU E/L unit. RIH with jet cutter and sever tubing at or below anticipated side-track depth. A 7 5/8" ClBP & whip-stock will be set after the drilling rig removes production tubing. Jet cutter is acceptable as this tubing will not be fished. Note the depths of the GLM's in the tbg string. Circulate clean seawater around IA and tubing. "O" Test the FMC 7 5/8" mandrel pack-off to 5000 psi and service lock down studs by backing out and re-torquing one at a time. Test the tubing hanger pack-off to 5000 psi and service each LDS "O" Check with the Drilling Rig Supervisor regarding rig timing and determine if freeze protection to diesel will be required. Bleed tubing and annuli pressures to zero. Set a BPV in the tubing hanger and test from below to 3500 psi. Secure well with 0 psi on tubing and annulus "O" Remove the well house and flow lines. Level the pad and dig out the cellar. ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. ITEM 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, Other-explain. ITEM 28: If no cores taken, indicate 'none'.
