Item 1' Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the

6/12/97:
RIH to pull 3.5" CT liner top section from above cut. Tagged top of 3.5" CT liner at 8989' MD. Pulled
liner top section free to 7800' MD. RIH w/liner to 8675' MD & pumped off fish to release and set liner
top section at 8675' MD. POOH. RD.
7/29/97- 8/1/97:
MIRU CTU. NU BOPS & PT. MU & RIH w/fishing BHA while loading the wellbore w/Seawater.
Latched the 3.5" CT liner fish & POOH w/fish.
8/2/97:
Lay down 3.5" liner fish. RIH w/CT to 9640' MD & pumped 21 bbls of Class G cement to plug &
abandon the open perforation intervals located at:

Pull packer from top of 3-1/2" x 2-7/8" liner. Cut and pull the 3-1/2" x 2-
7/8" liner at-- 9650'.
· RU coiled tubing unit. Tag top of fish ( liner cut at - 9650').
Pump 21 bbls Class 'G', 17 ppg fibered cement, bullhead cement passed
wireline fish in 2-7/8" and into open hole behind liner.
· Clean cement top to ~-9570' md (KOP for CTU drilling at 9600)
· WOC, Tag top of cement, pressure test to 1500 psi

Enclosed is the approved application for permit to redrill the above referenced well.
The permit to drill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does not authorize conducting drilling operations
until all other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with
20 AAC 25.035. Sufficient notice (approximately 24 hours) of the BOPE test performed
before drilling below the surface casing shoe must be given so that a representative of the
Commission may witness the test. Notice may be given by contacting the Commission
petroleum field inspector on the North Slope pager at 659-3607.
