logged well to 10995' MD. Spotted Biozan pill atop 4-1/2" liner. POOH w/tools.
Pumped 40 bbl LCM pill. RIH BHA & milled out nipple restriction. POOH w/BHA. RIH w/CT
nozzle & washed wellbore to PBTD @ 13111' MD. Pumped 77 bbls of 17 ppg Neat Class G
Cement to abandon the open slotted liner interval located at:
11349' - 13122' MD (Zone 1 )
Placed 6 bbl of Cement behind pipe. Attained a max sqz press of 2000 psi. PUH to 10000' &
WOC.
Contaminated cement w/Biozan & jetted contaminated cement f/the wellbore to 10800'.
POOH maintaining 500 psi back press. RIH w/BHA & tag cement @ 10830'. Milled Cement
to 10836'. Attempted to POOH but reel motor stopped spooling @ 231'. Repaired CT reel

Enclosed is the approved application for permit to drill the above referenced well.
The permit to drill does not exempt you from obtaining additional permits required by law from other
governmental agencies, and does not authorize conducting drilling operations until all other required
permitting determinations are made.
Approval for annular injection is contingent on the operator's submittal of an Application for Sundry
Approvals (10-403) with pertinent facts and evaluation of surface hole conditions, casing placement,
cementing, and any diagnostic logs that were run. The documentation submitted should include a CQL or
a valid LOT to assure the Commission that an adequate seal has been attained and that the well has been
cased and cemented according to regulation.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25.035. Sufficient
notice (approximately 24 hours) of the BOPE test performed before drilling below the surface casing shoe

Hazards and Contingencies:
This well is in the nearby proximity of produced water injectors DS 4-19 and PWDW 2-1.
Expect over pressure in the UGNU Cretacaceous sands (K-15). 1994 and 1996 wells drilling this
interval with a 9.7-9.9 ppg mud without incident. However, injection levels at that time were
very low and models now indicate that 400 psi overpressure (10 ppg EMW) can occur at or
before 5788' ss (5782' TVD). The injectivity zone is from 5870' to 6035' TVD with known
cement channeling to 4500' TVD.
Well Control:
Well control equipment consisting of 5000 psi W.P. pipe rams, blind rams, and annular preventer
will be installed and is capable of handling maximum potential surface pressures.

Operations Summary
MIRU drilling rig. NU and test diverter.
Spud well and drill 12-1/4" hole. The borehole will build at 1°/100' from 600'MD to
1100'MD. The wellbore follows this tangent then drops and turns at 1°/100' from
1300'MD to 1700'MD to return to vertical. The surface casing point is at 3764'MD
(3762'TVD) at 10'TVD into the SV3.
Set 9-5/8" casing and cement same to surface. Plan to run a screw in float collar for
cementing to surface. Nipple up BOP's and test same to 500/5000 psi. Test casing to 3000
psi for 30 minutes. Note AOGCC witness or waiver on the morning report.
P/U 8-1/2" BHA,(with MWD) drill 20' new hole and perform LOT. Carefully monitor for
Cretaceous overpressure from below the casing shoe until the full Ugnu has been drilled.
Contiunue to hold the borehole vertical to 5000'MD, 4998'TVD. At that depth begin a
build at 1°/100' to 5.0° by 5500'MD, 5497'TVD at an azimuth of 292.0°. Then continue
this build at 2°/100, to 11.0° remaining at 292.0° azimuth by 5800'MD, 5794'TVD. Then
begin a build and turn at 4°/100, to 52.0° by 6826'MD, 6650'TVD at an azimuth of
288.0°. Hold the borehole tangent to 9952'MD, 8575'TVD. At that depth begin a build
and turn at 8°/100, to 85.9° at 247.0° azimuth by 10582'MD, 8807'TVD. Hold the
borehole tangent to 10773'MD, 8820'TVD. At that depth begin a build and turn of
6°/100, to 11221'MD, 8842'TVD, the beginning of final horizontal section. The
wellbore will be at 88.5° angle and 220.2° azimuth at the 7" casing depth. After a short
trip POOH to run DP conveyed logs from 500' above the top of the SAD (9173'MD) to
TD. These logs will include:
HALS/LDT/CNL/BHCS/GR/CAL/SP (Platform Express plus sonic).
..
Make a hole opener run if hole conditions dictate. POOH and lay down drill pipe. Run
7" L-80 casing and cement (TOC@8673'MD), displacing with mud. Test casing to 3500
psi for 30 minutes.
Test surface equipment to 500/5000 psi. P/U 6" BHA, MWD w/GR & 3-1/2" drill string.
Drill out with the displacement mud which is to be at least 0.5 ppg heavier than the
horizontal drilling fluid, then displace to the Sheardrill-N system. Drill ahead and
continue the lateral section per directional program. TD for the section is prognosed at
12578'MD and 8882'TVD. Steer along the plan with guideance from geologist.
Run 4-1/2" slotted liner and hanger to place the packer and hanger with 150' of overlap
into the 7". Run 4-1/2" tubing to place the WLEG at 100' above the top of the SAD
(approx. 9573'MD). The SSSV on this well is to be placed near 2200'MD.
Set packer, test tubing. Test annulus.
ND BOP and NU tree assembly. Test tree assembly to 5000 psi.
Freeze protect the annulus.
