Submit a well schematic diagram with each 10407 well completion report and 10-404 well sundry report when the downhole well design is changed. Item 1a: Classification of Service Wells: Gas Injection, Water Injection, Water - Alternating -Gas Injection, Salt Water Disposal, Water Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. Item 4b: TPI (Top of Producing Interval). Item 8: The Kelly Bushing and Ground Level elevation in feet above mean sea level. Use same as reference for depth measurements given in other spaces on this form and in any attachments. Item 13: The API number reported to AOGCC must be 14 digits (ex: 50- 029 - 20123- 00 -00). Item 20: Report true vertical thickness of permafrost in Box 20. Provide MD and TVD for the top and base of permafrost in Box 28. Item 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. Item 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut -In, or Other (explain). Item 28: Provide a listing of intervals cored and the corresponding formations, and a brief description in this box. CMIT - TxIA Failed to 3500 psi 07/24/2008 MIT - OA Passed to 2000 psi 7/25/2008 Request: Approval is requested for the P&A of PBU well 05-31 A. 05-31A has TXIA communication and a production casing leak leaving it inoperable. There are no wellwork options left due to the high corrosion indicated on a Caliper and a low remaining reserves in the wells current location makes a RWO uneconomical. Therefore, a rig sidetrack is the most attractive option left to allow this well bore to be fully / utilized and bring value to the field. ~/ Other Requests: Given rig floor height limitations, a MASP < 3000 psi, and the fact that the three ram drilling BOP configuration is preferred to a two ram configuration with a spool, BPXA will execute the provisions of 20 AAC 25.285 (f) (1) regarding the initial testing of the BOPE as follows when existing wellhead or testing equipment prevents the testing of the lowest set of pipe rams: That lowest set of rams will be tested at the earliest opportunity during path-forward de-completion work, i.e., after the completion has been pulled from the well. Safety concerns and spacing limitations with the wellhead height and tubing spool preclude us from installing a spacer spool in the three-ram drilling BOP stack. Scope of Work: The sidetrack plan for the 05-31 A includes the following during pre-rig operations: pump cement across the perforations and circulate cement into the IA, the 3-1/2" tubing will be cut 500' below KOP. The rig will pull tubing, cut and pull the 7" casing. A cement plug will be laid in and sidetracked off of. The 8-3/4" hole will be drilled to the Top of the Sag River where a 7" long string to surface will be run. The 7" casing string will be cemented with 1000' of cement. The production hole will be drilled with a 6- 1/8" BHA building to horizontal. The well will then run horizontally through the Ivishak formation. The / production liner will be 4-1/2" L-80 and fully cemented. Tubing will be 4-1/2" 13Cr, tied into the top of the production liner. General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Item 1: Classification of Service wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. Item 16 and 24.' If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Item 21'. Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). Item 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-in, Other-explain. Item 28: If no cores taken, indicate "none". (3) All Gamma Ray data (GRAX) presented is realtime data. (4) Well 05-31A is represented by runs 5, 6, 7, 8, 9, 10, 11 and 12 which are all Directional with Gamma Ray. (5) Well 05-31A was initially kicked off @ 9407' MD (8565' TVD) from the original wellbore of Well 05-31. (6) Run 1 through run 4 were milling runs, and no logging data was acquired in new hole. (7) The interval from 11041' MD (8816' TVD) to 11057' MD (8816' TVD) was not logged due to sensor to bit offset. (8) Well 05-31A was drilled to TD @ 11057' MD (8816' TV]D) on Run 12. A 28' depth adjustment was applied after a tie-in which resulted in a TD of 11085' MD (8816' TVD). (9) The Gamma Ray log has been depth shifted to match the Schlumberger MCNL Gamma Ray logged on 08 November 1998. (10) A Magnetic Declination correction of 28.02 deg has been applied to the Directional Surveys. ~Mnemonics ROPS -> Rate of Penetration (ft/hr) GRAX -> Gamma Ray MWD-API [MWD] (MWD-API units) TVD -> True Vertical Depth, feet ~Sensor Offsets RUN GAMMA DIRECTIONAL 5 21.37 ft. 15.92 ft. 6 23.99 ft. 16.44 ft. 7 16.40 ft. 20.89 ft. 8 16.07 ft. 20.56 ft. 9 26.14 ft. 18.59 ft. 10 16.26 ft. 20.75 ft. 11 15.48 ft. 19.87 ft. 12 16.26 ft. 20.75 ft.
