An extended gas venting and flaring event occurred at GPMA DS L2 on September 28. There was a failure on the high
pressure gas reinjection line that required depressuring the line from LPC to LGI. LPC was shutdown at 0900 and all
gas was initially sent to flare. Gas venting was initiated at LGI pad to increase the rate of depressuring due to
increasing LEL gas levels in Module 4902 at L2. All gas venting /flaring stopped at 1432 hrs when the pressure on the
gas injection header was bled to 0 psi. The total flared volume was 6.4 MMSCF and the total volume vented (through
the leak and at LGI) was 1.1 MMSCF.

Subject: Gas Disposition September 2006 — Lisbume Production Center
A leak on the high- pressure gas reinjection line within the Lisburne L2 manifold building
occurred September 28, 2006. BP made proper notification of the gas release and followed up
with a 1 -page written summary. Investigation by BP revealed that the leak occurred due to
insufficient torque on a blind flange, resulting in a line failure between the blind flange and a
plug valve. Gas from the leak accumulated in the L2 manifold building causing the Lisburne
Production Center ( "LPC ") to be shut down.
BP reports on its September 2006 Gas Disposition Report (Form 10 -422) for LPC that a total of
8,132 Mcf of gas was flared and vented longer than 1 hour as a result of this event. Of that total
7,438 Mcf was a direct result of the leak and depressuring the high- pressure gas line; this volume
should be deemed waste. The remaining 694 Mcf of gas was a result of flaring during the restart
of LPC on September 30. While the decision to make the September 30 volume waste is less
definitive, I recommend we also declare as waste the 694 Mcf of gas associated with plant restart
since it is a direct result of the incident. The total volume deemed waste associated with this
event would be 8,132 Mc f.
Attached is a copy of the Gas Disposition Report for signature; also attached is the forwarding
letter to Department of Revenue notifying them of the Commission's waste determination.

An extended flaring event occurred at GPB on November 24, 2006.
The L1 to LPC commoniine was being deinventoried (several pigs were
previously run through the line to displace the liquids) and depressured in
preparation for removal. Gas was flared during the depressuring process. A
total of 5178 MSCF was flared and the event lasted from 15:31 to 19:10 (219
minutes).
