FEB 1 6 200 ] Alaska Oil & Gas Cons. Commission Anchorage Date: O1/11/O1 Flare Volume: 681 Assist Gas Volume: 170 Event description MPU experienced a shutdown of the 08 turbine power generator. This resulted in a load shedding of various equipment. Note• The flare transmitter was out for annual maintenance at the time of this event. Data from a similar flare event that occurred on 12/01/00 was used to assist with the volume calculation for the O1/11/O1 event. Trends on all flare valves off the production vessels for the 12/01/00 & 1/11/Ol were used to calculate the volume. All of these tools were then used to calculate the unknown rate for O1/11/O1. Action taken to stop flaring The maintenance personnel were dispatched to the turbine to troubleshoot the equipment problem. The following wells were shut-in to mitigate the flaring: K-2,5,6,13,17,30,37,38; C-4,5,7,9; B- 3,4,6,9,10,15,20,21,22. A connection to the vibration sensor (XE-0806) was found loose. RTV was used to keep the connection from coming loose again. After turbine cooled down, it was restarted and no abnormal vibration was detected. All compressors was started and the flare stopped. Action taken to prevent reoccurrence Vibration caused the connection to slowly back off until it lost connection. Use of the RTV should keep this from reoccurring. Date: 03/23/01 Flare Volume: 611 Assist Gas Volume: 610 Event descriution Milne Point shutdown (C-5502, 03, 31A/B) the low pressure & injection compressors for safety, valve replacement. Note: Replaced two leaking common discharge valves with new double block & bleed valves & one common shut down valve (XV-5529). Action taken to stow flaring All replacement parts, tools, crews, & procedures were in place before the compressors were shut down. The C-3 & B-20 wells were shut in before we took the compressors down. Action taken to arevent reoccurrence No action could be taken to prevent a reoccurrence of this failure. Due to the safety of personal & equipment this job had to take place. Event description Date: 04/22/01 Flare Volume: 342 Assist Gas Volume: 169 There was an unplanned flaring event when the 07-generator shutdown on a loss of flame alarm from the governor. The facility and field did a load shed of power from this shutdown. Maintenance personnel were expedited to repair the problem and restart the turbine. Action taken to stop flaring The load shed shutdown gas compression and both water injection trains. High GOR wells that did not go down on the load shed were shut in. One gas train at CFP stayed on line during this load shed. The 07 turbine was restarted and all systems were brought back to their normal condition. The flaring stopped. Action taken to prevent reoccurrence Maintenance will contact Woodward on this shutdown 04-23-01. Event descriution Date: 04/23/01 Flare Volume: 207 Assist Gas Volume: 102 There was an unplanned flaring event when the 07-generator shutdown on high T54 temp. This time both the Woodward Gov. and the PLC picked up the same alarm. We found a shorted wire in a 3/a" capped L conduit fitting. The facility and field did a load shed of power from this shutdown. Maintenance personnel were expedited to repair the problem and restart the turbine. Action taken to stop flaring The load shed shutdown gas compression and both water injection trains. High GOR wells that did not go down on the load shed were shut in. One gas train at CFP stayed on line during this load shed. The 07 turbine was restarted and all systems were brought back to their normal condition. The flaring stopped. Action taken to prevent reoccurrence Propose Micro Net Governor training for our turbine maintenance team. Evaluate our governor trouble shooting procedures and make changes as needed. Event description Date: 05/23/01 Flare Volume: 3426 Assist Gas Volume: 819 There was an unplanned flaring event when the gas compression plant shutdown due to a high liquid level in a compressor suction scrubber. The shutdown was extended due to the failure of 2 control relays. Action taken to stow flaring All high GOR wells were shut-in to decrease the flare volume. The compressor suction scrubber level was drained to the normal operating level. Maintenance Technicians were dispatched to troubleshoot the cause of our inability to gain a clear start permissive for the gas compressors. The Maintenance Technicians found and replaced 2 failed control relays which enabled the gas compressors to be started. All Gas Compressors were restarted and the flaring stopped. Action taken to prevent reoccurrence A Maintenance Technician is investigating and correcting the problem of not receiving a high level alarm for the affected suction scrubber, which would have alerted us to the rising liquid level. Event description Date: 05/04/01 Flare Volume: 332 Assist Gas Volume: 275 There was an unplanned flaring event when the 08-generator shutdown on local ESD switch trip. During routine maintenance ESD switch was bumped. The facility and field did a load shed of power from this shutdown. Maintenance personnel were expedited to restart the turbine. Action taken to stop flaring The load shed partially shutdown gas compression and both water injection trains. One gas train at CFP stayed on line during this load shed. The 08 turbine was restarted and all systems were brought back to their normal condition. The flaring stopped. Action taken to prevent reoccurrence ESD trip switches inside turbine enclosure will be replaced with new style. Until new switches arrive on site existing switches have temporary covers installed. Event description Date: 06/11/01 Flare Volume: 621 Assist Gas Volume: 309 MPU experienced a shutdown of the 08 turbine power generator. This resulted in a load shedding of various equipment. Action taken to stop t7aring Maintenance personnel were dispatched to troubleshoot the turbine shutdown. The following wells were shut-in to mitigate the flaring: K-2,5,6,13,17,30,33,37,38; C-3,4,7,9,14,22,26; B- 3,4,10,16,20,21,22; E-6,14,24,30; F-38,50,78,80; G-4; H-5,8,12; J-1,3,8,9,19,23,24; L-3,6,7. The turbine shutdown alarm was a Generator Failure Fast Stop. There are 3 different inputs that could have caused this shutdown. After the turbine generator was completely checked it was restarted. No abnormal conditions were detected. All compressors were started and the flare stopped. Action taken to prevent reoccurrence Traps were installed in the PLC to capture which inputs caused the shutdown if it happens again. Event description Date: 08/ 19/01 Flare Volume: 680 2359 Assist Gas Volume: 1 576 The battery charger in Mod 56 went into a high charge rate for the battery bank. Hydrogen gas was detected and the ventilation system cleared the alarm. The charger was left at high charge rate. The charger did not shut-off after charging the battery bank and caused the DC voltage to the PLC to be increased. The PLC power supply shutdown due to the high DC voltage to protect it's self and thereby shutting down both of our turbine electric power generators (07 and 08). This caused a total field-wide shutdown at Milne Point. Action taken to stop flaring The initial shutdown depressurized the whole plant. The two power generators have a 2-hour cooling period from the fast shutdown. We brought in cooling fans to help cool-down the turbines. After the 2-hour period we brought gas in from our gas well for a restart. The assist gas was increased to help with the flaring and black smoke. The gas lift compressor was started and we started bringing back the wells. The IWAG was started and fluids brought back into the vessels for normal operation. Action taken to prevent reoccurrence Maintenance was tasked with ordering a new battery charger for the Mod 56 battery room and a replacing the other battery chargers around the plant and the field. Event description Date: 08/21 /01 Flare Volume: 429 147 1996 Assist Gas Volume: 46 32 639 At approx. 09:52 hours the battery charger in Mod 56 went to high charge rate and the DC voltage increased to the point of shutting down the power supply for the PLC control. This caused both of the 07 & 08 power turbine generators to shutdown. This created afield-wide or Level 3 shutdown at Milne Point. The 07 turbine locked up because of the fast shutdown and has a 2 hr. waiting period. We started 08 turbine but it shutdown a few minutes later because of the battery charger going to a high DC voltage rate. We also were having problems with our control room computers locking up the screen. Action taken to stop flaring The discussion was made to run a temporary power cable from the adjoining Mod. 57-battery charger to the Mod 56 power box and thereby eliminating the defective charger in Mod 56. After the temp. Connection (until a new charger arrives) the generators were brought back on line and we started to bring up the field and the plant to normal operations. We had several minor start-up problems with equipment and were back on line at approx. 20:00 hrs. Action taken to prevent reoccurrence Maintenance was tasked with ordering a new battery charger for the Mod 56 battery room and the temporary hook-up to Mod. 57's battery charger should eliminate this reoccurrence. The Compressor Intake air Temperature (CIT) sensor (TE-08100) failed at approximately 18:46 on 09110/2001. This caused a fast stop of the turbine causing aload-shed condition. The rotor on the gas producer locked for approximately 2.5 to 3 hours. The cause of the CIT failure was coking build up on the probe. Unplanned flaring began at approximately 18:46 as a result of the sensor failure and continued for approximately 5 hr 44 min. Intermittent black smoke was emitted from the CFP flare for the first 58 minutes of the flaring event. 4. At approx. 06:42 they were preparing tt) do the fire and gas preventative maintanCe work and were pulling relays in the fire and gas cabinet. A relay was pulled which caused a level 3 shutdown of the fuel gas system. This caused the running turbine (07) to switch to liquid fuel (diesel) and the kilowatt level was greater than the allowable amount. It was in emission excursion for approx. 1 hr. 47 minutes. The level. 3 shutdown also caused the on line gas compression machine to shutdown. This caused an intermittent black smoke event and also a greater than 1 hour flaring event before the machines were brought back on line at approx. 08:37 for the gas lift compressor (Big A) and 09:13 for the gas injection compressor (Little 6). We had half of second and third stage gas compression shut down for maintenance work. Support crews were called out to locate the cause and make repairs. Action taken to stop flaring Immediate response to the compressor shut down by maintenance crew. Wells B3, B9, B i 0, B 15, B20, B21, B22, C5, C7, & C9 were shut in to mitigate flaring. After the tie-in was complete, IWAG 1 was restarted, it developed a leak on the main cooling water pump due to a seal failure and had to be shutdown for maintenance. Event description Date: 12/07/2001 Flare Volume: 288 1704 Assist Gas Volume: 1 237 At approx. 01:41 hours, a total field-wide shutdown was caused by a level switch low /low reading on tank T-5711 the Cooling Medium Expansion Tank. The low level alarm that would normally alert to a shutdown level in the tank failed to trip. The low level and subsequent shutdown caused a Level 4 Shutdown. The 0701 and 0801 turbine generators shutdown on a fast shutdown, which requires a 2-hour, waiting period prior to re-start. Several additional time-delay problems occurred due to the initial shutdown. Around 07:30 problems were encountered with the Little B injection compressor and the alternate injection compressor was down for maintenance. There were problems getting the IWAG running because of the lack of heat at C-pad from the extended initial shutdown, facilitated by extremely cold ambient temperatures. It was suspected that black smoke was emitted intermittently due to periodic short-term flaring during the shutdown and subsequent start up periods. Action taken to stop flaring Production was shut in during facility shut down and gas lift wells were shut in to minimize flaring. Production was minimized during start up to minimize flaring as well. Action taken to prevent reoccurrenceThe LSLL (low level alarm system) was checked for correct operation and no problems were found. A Management of Change process will be initiated to install a level transmitter on the tank so that personnel can observe tank levels prior to the alarm switch tripping. Further review of the shutdown logic will also be conducted in an attempt to determine nature of failure.
