Location: Milne Point CFP Flare
Date: 02128/2002
Time and duration ofincident(s): 1837 2 hrs 8 min.
Flare Volume: 1480
Planned event: Dyes
X no
Assist Gas Volume: 260
This flaring or venting incident was: (check one)
X necessary for facility operations.
D necessary for facility upgrades.
D necessary for facility testing procedures.
D an emergency that threatened life or property.
D necessary to prevent loss of ultimate recovery.
Event description
PU-0801 shutdown at 18:37 on a Generator Failure alarm. This alarm is a fast stop. The resulting load shed
tripped all of the compressors, both water injection trains, the shipping pump and K-pad. E-Pad
and B-Pad breaker did not open this is the cause of C-5501/11 Band C-55002/03/31 B
compressors to be load shed.
The generator failure alarm (alarm-75) can be caused by any of the following:
Lockout Auxiliary Relay 86X, Auxiliary Field Circuit relay 41 X , and Ground Fault 64F. This is the
second time this shutdown has happened. The previous shutdown we were unable to determine
which of the three caused the shutdown, traps were installed at this time to help trouble shoot the
shutdown if it reoccurred. When this shutdown occurred we were able to rule out two of the three
causes, we determined that the shutdown was due to the Ground Fault 64F circuit.
Action taken to stop Darin!!
High GOR wells were shut-in to mitigate flaring.
Action taken to prevent reoccurrence
We found a relay on the Ground Fault board that was not seated into the base all of the way. This could have
been the cause of the alarm.
The turbine was started and returned to service at 20:18 without any other problems.

235.1 During a scheduled slow down the Plant maintenance was notified
remaining bank of cooling fans went immediately and was eventually able to
down and would not restart. The high re-start the fans. Once cooled, the gas
temp resulted in gas compression failure compression was re-started.
and a flaring event.
166.3 A generator failure alarm caused
PU-0801 to shut down. The cause was
due to a failure of a relay on the ground
fault 64F circuit board that was not
seated into the base completely.
Discussions with the manufacturer
revealed that this failure should be set
as an alarm only, rather than a shutdown
mechanism. Switching this to an alarm
fault only will prevent future
shutdowns.

416.5 A PSV failed on the F-pad to C-pad
production header causing three well
pads to be shut in and resulted in a
flaring event.
After the emergency shutdown of all
wells that fed the failed PSV, the valve
was isolated. An investigation of this
PSV type will ensue due to duplicate
failures of this type of valve in the
recent past.
Production was minimized; all gas lift
wells were shut in and chokes were
closed. Alarm set points were set to a
lower value allowing for quicker
identification of gas compression and
cooling problems.
Production was minimized; all gas lift

closed. Alarm set points were set to a
lower value allowing for quicker
identification of gas compression and
cooling problems.
After maintenance crews checked over
the turbine, a restart was performed and
flaring stopped. Crew was instructed on
proper steps to initiate a turbined alarm
reset and reminded to slow down and
review all steps to the job prior to
taking action.

542.7 Gas cooler fans shut down causing high
discharge temps and pressure. All gas
compression went down before cooler
faDS could be re-started. This resulted
in an unplanned flaring event of greater

15.6 Gas cooler fans shut down causing high
discharge temps and pressure. All gas
compression went down before cooler
fans could be re-started. This resulted
in an unplanned flaring event of greater
than one hour. (This event is linked to
flaring on 5/25/02)
189.3 While performing maintenance on the
power turbine, the stop botton was
inadvertantly pushed. Half of the gas
compression went down on load shed as
designed. Twenty-nine production
wells, the IW AG compressor and the
NGL pump also stopped.

488.6 When the Big A Compressor was
shutdown to repair a bad PSV (575A) it
unintentionally resulted in the shutdown
of the Big B Compressor. This resulted
in the loss of fuel gas for 07 and 08
turbine generators causing the plant to
go to the flare.
In response, the gas lift wells were shut
in and the Big B Compressor was
purged and restarted. A discussion was
held by operations regarding the
possibility of shutting down both
compressors when shutting down one
compressor.

Two major pieces of equipment
(PU-701 turbine and C550l/511A train)
were shutdown for critical maintenance
to prevent future unplanned shutdowns.
To minimize flaring, several high GOR
and gas lift producing wells were shut
down prior to the maintenance work.
Intermittent flaring stopped after
equipment brought back on-line.

97.0 A malfunction within our water
management system resulted in a level
3 shutdown of the plant (wells shut in).
As part of the facility shutdown process,
the various vessels depressurize to the
fl are.
A level controller problem was
corrected and the various facility
vessels were drained of excess water to
enable plant start up. Level transmitter
modifications will be made to prevent a
recurrence.
