The annual pressure safety valves re-certification was due on the gas compressors. We have to shutdown the gas lift compressors to re-certify the PSV's. 4. Flare Volume: 1480 Planned event: Dyes X no Assist Gas Volume: 260 This flaring or venting incident was: (check one) X necessary for facility operations. D necessary for facility upgrades. D necessary for facility testing procedures. D an emergency that threatened life or property. D necessary to prevent loss of ultimate recovery. Event description PU-0801 shutdown at 18:37 on a Generator Failure alarm. This alarm is a fast stop. The resulting load shed tripped all of the compressors, both water injection trains, the shipping pump and K-pad. E-Pad and B-Pad breaker did not open this is the cause of C-5501/11 Band C-55002/03/31 B compressors to be load shed. The generator failure alarm (alarm-75) can be caused by any of the following: Lockout Auxiliary Relay 86X, Auxiliary Field Circuit relay 41 X , and Ground Fault 64F. This is the second time this shutdown has happened. The previous shutdown we were unable to determine which of the three caused the shutdown, traps were installed at this time to help trouble shoot the shutdown if it reoccurred. When this shutdown occurred we were able to rule out two of the three causes, we determined that the shutdown was due to the Ground Fault 64F circuit. Action taken to stop Darin!! High GOR wells were shut-in to mitigate flaring. Action taken to prevent reoccurrence We found a relay on the Ground Fault board that was not seated into the base all of the way. This could have been the cause of the alarm. The turbine was started and returned to service at 20:18 without any other problems. The cause was due to a failure of a relay on the ground fault 64F circuit board that was not seated into the base completely. Discussions with the manufacturer revealed that this failure should be set as an alarm only, rather than a shutdown mechanism. After the emergency shutdown of all wells that fed the failed PSV, the valve was isolated. An investigation of this PSV type will ensue due to duplicate failures of this type of valve in the recent past. Production was minimized; all gas lift wells were shut in and chokes were closed. Alarm set points were set to a lower value allowing for quicker identification of gas compression and cooling problems. This resulted in the loss of fuel gas for 07 and 08 turbine generators causing the plant to go to the flare. In response, the gas lift wells were shut in and the Big B Compressor was purged and restarted. A discussion was held by operations regarding the possibility of shutting down both compressors when shutting down one compressor.
