First Determination of Cause:
1. T /C # 1803 SID due to problems encountered during an Inverter P.M.
2. Due to an Inverter failure at P.C.M. # 1, the Tie-line S/!. This failure also caused the F.S. Control
Room Setcim System to S.D. Subsequently the Annex and F.S were O.S.D.'d. Flaring occurred
intermittently off the Crude Oil Surge Drum, with some black smoke excursions.
3. T/C # 1801 wouldn't take the load.
Action Taken to Stop Flaring:
1. P.M. completed and T/C # 1803 resarted and loaded.
2. Automation and Electricial Techs made necessary repairs. The F.S. and Annex were returned to
service.
3. Cycled T/C speed combined with fresh production allowed recycle temps to fall enough for T/C #
1801 to take the load.
