583 scf NGL vapour 192 gallons of liquid NGL

Cause of loss:
A loss of primary containment (LOPC) occurred during
maintenance preparation for planned work on the LACT
NGL meter Prover located in Module 4984 at Skid 50.
5.
Actions taken and planned to prevent
The fire and gas (F&G) system acted per design, closing
further loss:
the NGL system associated battery limit valves and source
was controlled. Halon system discharged as designed for
cause. Actions were taken on supply and delivery lines to
limit source and pressure to leak location. Incident was
managed as a Tier I event. Once de-escalated, skid entry
was made, with associated clean-up and ventilation
activities. Prior to restart of the NGL skid, a F&G system
functional test was performed. Operations executed a
thorough joint integrity survey before start up. Halon
system was replenished prior to start up. The actual cause
of the incident is currently under investigation.

Operators failed to follow the steps outlined in their
procedure, opening a valve on a hose prior to the hose
beinq connected at its other end.
5. Actions taken and planned to prevent
The fire and gas (F8G) system acted per design, closing
further loss:
the NGL system associated battery limit valves and source
was controlled. Halon system discharged as designed for
cause. Actions were taken on supply and delivery lines to
limit source and pressure to leak location. Incident was
managed as a Tier I event. Once de-escalated, skid entry
was made, with associated clean-up and ventilation
activities. Prior to restart of the NGL skid, a F8G system
functional test was performed. Operations executed a
thorough joint integrity survey before start up. Halon
system was replenished prior to start up. The actual cause
of the incident was part of a formal incident investigation
process. Key findings/action items noted below will be
progressed to prevent further losses from subsequent
similar maintenance activities.

to eliminate the potential for LOPC events when
performing future maintenance activities of the scope
that caused the incident

A flaring event occurred at FS3 starting at 04/22/2013 05:11 and stopping at 04/22/2013 06:11 with a volume of 8
(MSCF). The event lasted for 60 minutes and was due to shut down of STV/IP T/C 1801 due to control/instrumentation
failure. Three start attempts were made before T/C 1802 was started and loaded. Flaring ended when STV/IP T/C 1802
was brought online, loaded and the flare valves closed.

(MSCF). The event lasted for 135 minutes and was due to planned swapping of load from STV/IP T/C 1802 to STV/IP T/C
1801. One failed start attempt, before T/C 1801 was started and loaded. Flaring ended when STV/IP T/C 1801 was
brought online, loaded and the flare valves closed.

associated with oil pressure. Flaring stopped when the compressor was re -loaded.
A flaring event occurred at FS3 starting at 05/11/2013 13:02 and stopping at 05/11/2013 14:36 with a volume
of 70 (MSCF). The event lasted for 94 minutes.
at FS3 starting at 05/11/2013 01:05 and stopping at 05/11/2013 03:00 with a volume of 40 (MSCF). The event
lasted for 115 minutes and was due to spin cooling of 1803 LPS compressor for water wash. Flaring stopped
when cooling complete.
A flaring event occurred at FS3 starting at 05/13/2013 01:15 and stopping at 05/13/2013 03:15 with a volume
of 40 (MSCF). The event lasted for 120 minutes and was due to spin cooling of 1804 LPS compressor for water
wash. Flaring stopped when cooling complete.
A flaring event occurred at GC3 starting at 05/11/2013 03:00 and stopping at 05/11/2013 18:30 with a volume
of 16146 (MSCF). The event lasted for 930 minutes. The event had a volume greater than 5000 (MSCF) and
was due to depressurization of the GHX line for planned pigging activities. Flaring stopped when the line was
depressured.

(MSCF). The event was a black smoke event and was due to LP/IP compressor s/d for workon the emergency generator
breaker.
A flaring event occurred at GC1 starting at 05/24/2013 00:00 and stopping at 05/25/2013 with a volume of 250 (MSCF).
The event lasted for 1440 minutes and was due to valves leaking into the flare system.
A flaring event occurred at GC1 starting at 05/23/2013 00:00 and stopping at 05/24/2013 with a volume of 250 (MSCF).
The event lasted for 1440 minutes and was due to valves leaking into the flare system.

(MSCF). The event lasted for 140 minutes. The event had a volume greater than 5000 (MSCF) and was due to a
shutdown of 1855 TIC — flaring occurred while operations/maintenance worked to get the machine back online.
A flaring event occurred at FS3 starting at 05/29/2013 01:45 and stopping at 05/29/2013 03:55 with a volume of 875
(MSCF). The event lasted for 130 minutes and was due to 1806 T/C cooldown for planned maintenance. Flaring stopped
when cooldown was completed.
A flaring event occurred at FS1 starting at 05/26/2013 16:30 and stopping at 05/26/2013 16:31 with a volume of 83
(MSCF). The event was a black smoke event. Another event occurred at FS1 starting at 05/26/2013 16:11 and stopping
at 05/26/2013 16:13 with a volume of 168 (MSCF). The event was a black smoke event. A third event occurred at FS1
starting at 05/26/2013 15:51 and stopping at 05/26/2013 16:00 with a volume of 755 (MSCF). The event was a black
smoke event. These events were all related to the 1803 T/C water wash gas crank sequence, including attempts at the
crank sequence to investigate flare issues.

A flaring event occurred at FS3 starting at 06114/2013 14:23 and stopping at 06/14/2013 16:00 with a volume of 99
(MSCF). The event lasted for 97 minutes. Flaring was due to the need to depressure the 6B Common Line for planned
project work. Flaring ended when the planned depressuring activities were completed and the flare valves closed.
A flaring event occurred at FS3 starting at 06/14/2013 14:23 and stopping at 06/14/2013 16:00 with a volume of 141
(MSCF). The event lasted for 97 minutes. Flaring was due to the need to depressure the 6D Common Line for planned
project work. Flaring ended when the planned depressuring activities were completed and the flare valves closed.
A flaring event occurred at FS3 starting at 06/17/2013 02:53 and stopping at 06/17/2013 03:05 with a volume of 7548
(MSCF). The intermittent event lasted for 12 minutes. Flaring was due to unplanned shutdown of a compressor at CGF
causing high gas transit line pressure. Rate was cut to reduce flare volume while CGF worked to resolve problems.
Flaring ended when sufficient rate was cut and CGF compressor issues resolved, at which time the flare valves closed.

A flaring event occurred at GC2 starting at 08/25/2013 00:00 and stopping at 08/25/2013 13:00 with a volume of 7583
(MSCF). The event lasted for 780 minutes. The event had a volume greater than 5000 (MSCF) and was due to flaring off
the LPS compressors as the transit system pressure came up during startup of gas lifted rate.
A flaring event occurred at GC2 starting at 08/24/2013 13:24 and stopping at 08/25/2013 with a volume of 6448 (MSCF).
The event lasted for 636 minutes. The event had a volume greater than 5000 (MSCF) and was due to flaring off the IP/LP
compressors for operational stability as GILT was ramping back up.
A flaring event occurred at GCI starting at 08/24/2013 00:24 and stopping at 08/24/2013 20:52 with a volume of 7826
(MSCF). The event lasted for 1228 minutes. The event had a volume greater than 5000 (MSCF) and was due to final
depressurization of the GT -13 for planned inspections.
A flaring event occurred at GC1 starting at 08/23/2013 19:45 and stopping at 08/24/2013 with a volume of 980 (MSCF).
The event lasted for 255 minutes and was due to depressurization of the GT -13 line for planned inspections.
A flaring event occurred at GC3 starting at 08/24/2013 00:00 and stopping at 08/24/2013 15:30 with a volume of 2000
(MSCF). The event lasted for 930 minutes and was due to depressurization of GT -13 for planned inspections. Flaring
stopped when pressure reduced to below 250 psig.
A flaring event occurred at FS3 starting at 08/20/2013 13:00 and stopping at 08/20/2013 14:22 with a volume of 342
(MSCF). The event lasted for 82 minutes and was due to STV/IP compressor 1801 having operational issues. Flaring
stopped when operations switched to 1802.

A flaring event occurred at FS2 starting at 08/31/2013 15:31 and stopping at 08/31/2013 17:50 with a volume of 18
(MSCF). The event lasted for 139 minutes and was due to depressurization of 16D commonline for planned removal of
temporary pigging facilities. Flaring stopped when depressurization complete.
A flaring event occurred at GC1 starting at 08/26/2013 11:05 and stopping at 08/26/2013 16:35 with a volume of 320
(MSCF). The event lasted for 330 minutes and was due to swapping of GILT machines for water washes. Flaring stopped
when water washes complete.
A flaring event occurred at FS3 starting at 08/26/2013 14:50 and stopping at 08/26/2013 15:45 with a volume of 9329
(MSCF). The event was a black smoke event. The event had a volume greater than 5000 (MSCF) and was due to a
power outage that caused loss of T/C's 1801, 1803, & 1806 in addition to the water section, regens shipping pump, etc.
Flaring stopped when the turbine/compressors were started and re -loaded after getting generator power established.

A flaring event occurred at FS3 starting at 12/28/2013 12:00 and stopping at 12/28/2013 18:00 with a volume of 27
(MSCF). The event lasted for 360 minutes. Flaring was due to numerous failed start attempts on TIC 1801 STV/IP
compressor to enable swapping from the online unit, TIC 1802. Flaring ended when the decision was made to no longer
attempt to start the TIC 1801 unit.
