Event description Date: 1-14-00 Flare Volume: 359 Assist Gas Volume: 177 The PSV-5575A on C-SSOlA ,the discharge of # 2 cylinder lifted causing this stage to go to flare. On investigation we found the 3/8 "tubing on the PSV had broken inside the swaglok fitting and was leaking gas which caused the PSV to open to flare. Action taken to stop flaring Several gas lift wells were shut-in to help mitigate the flaring. These wells were B-3,B-4,B-9, B- 10,B-15, B-20, B-21, B-22. The compressor was shutdown and the tubing was replaced on the broken tubing line. The compressor was started and the flaring stopped. Action taken to prevent reoccurrence Operations team was instructed to keep a watch on any high vibration areas for tubing fatique or loose fittings. Event description Date: O 1 /24/00 Flare Volume: 651 Assist Gas Volume: 237 An erroneous signal from a switch in the local control panel of C-5501/11B caused a normal compressor shut down. Flaring began shortly thereafter. Action taken to stop flaring Operations personnel immediately determined that the shutdown was not caused by a process upset and called out maintenance personnel. Responding quickly, maintenance personnel investigated the shutdown. After several failed re-start attempts, technicians located and tightened some loose wire terminals in the local control panel. The compressor was re-started and flaring stopped. Action taken to prevent reoccurrence A review of the incident concluded that compressor equipment vibration caused the wire terminals to loosen after time. Terminals were tightened and a plan to check the other machines has been implemented. Event description Date: 4-5-00 Flare Volume: 248 Assist Gas Volume: 148 This flareing event occured as a result of mechanical condition on the C-Pad IWAG compressor C- 2901. The compressor shut down on a lube oil low level alarm, troubleshooting level alarm and cause are still ongoing. Stand-by IWAG compressor C-2601 was in a safe out status for maintenance repairs. Action taken to stop flaring First report from Field Operators on condition of compressor was estimated restart in 10 to 15 minutes. 27 Minutes into the flaring event all B-Pad gas lift wells (9 wells) were shut in, all C-Pad gas lift wells (4) were shut in. 40 Minutes into the flareing event 5 ESP well were shut in on G and E Pads. Approximately 15.6 MMCFD formation gas total was taken off line to mitigate this flaring event. Action taken to prevent reoccurrence Reveiewed operating procduces on mechanical shut downs /failures. For future failures wells will be immediately shut in and then cause and re-start evaluted for additional actions. Event description Date: 5-27-00 Flare Volume: 1708 Assist Gas Volume: 384 This flaring event occurred while performing maintenance on the Gas Compression Coolers. Cooling was insufficient as the ambient temperature rose during the day. The C-5501/11A Compressor train shut down on a PSHH discharge pressure. Cascading effect then took the rest of Gas Compression down. Action taken to stop flaring Once Gas Compression shutdown, we immediately started shutting in all gas lift wells. Maintenance crews were informed to shutdown all fin fan work. The process control group was called out to assist with the re-start of all compressors. Thirty minutes after the compressors shutdown we started shutting in ESP's. Eventually shutting in a total of 35 high GOR wells, with a total of 27,000 mcfd rate in order to mitigate the flare. Action taken to prevent reoccurrence Reviewed operating procedures for performing cooler maintenance. Set ambient temperature limitations for shutting down a cooler fan. The maintenance team will check all related pressure & temp shutdown switches & calibrate as needed. Work orders will be put in the system to add pressure transmitters on the discharge side of both compressor trains. Outstanding cooler maintenance will be moved to the night shift to take advantage of lower ambient temps. The Compressors were shut down when 0701 Power Turbine Generator tripped off line due to high turbine enclosure temp. The power load shed protection for 0801 Turbine Generator worked as designed. Cooling was insufficient as the ambient temperature rose during the day. Action taken to stop flaring Once Gas Compression shutdown, we immediately started shutting in all gas lift wells & closed the chokes. Maintenance crews were called out to assist with the restart of the turbine. Thirty minutes after the compressors shutdown we started shutting in ESP's. Eventually shutting in a total of 28 wells, (B9, B15, B22, B03, B10, L06, B4, C7, C3, B6, E6, B21, B20, J8, L3, C14, J3, C4, L2, J1, C22, E11, C9, F45, E4, F61, C1, & CS) in order to mitigate the flare. Event description Date: 7-15-00 Flare Volume: 867 mcsf Assist Gas Volume: 168 mcsf This flaring event was due to an unplanned shut down of CM-5501/11A & B compressors. The Compressors were shut down when PAHH-5534A had a faulty trip due to a bad switch. We started up the gas lift compressors, but had a faulty limit switch on the Little A compressor which would not allow a complete purge. The Little B compressor was down during this period for maintenance on the aftercoolers. After Little A started we had several tries before we got a IWAG compressor started. Action taken to stop flaring Once Gas Compression shutdown, we immediately started shutting in all gas lift wells & closed the chokes.Wells B,3,4,6,9,10,15,20,21,22,23, and C-4,5,7,9. were shutin to mitigate the flaring. Maintenance crews were called out to assist with the repair of the faulty pressure switch and calibration of the faulty limit switch. Event description This was an unplanned flaring event which occurred when the IWAG 1 injection compressor shut- down due to a leak on the trabon system on the 22"d. A combination of wells were returned to production once the IWAG injection compressor was back on line. B-23 had some down hole maintenance completed and was put on line over the last few days in order to test the results of the perforation work. High ambient temp.'s and excess gas on the B-23 well caused the problem with flaring when returning the wells back to production. There were 13 wells that were shut-in to mitigate flaring due to the compressor shut-down and B-23 return to production. Action taken to stop flaring The IWAG 1 injection compressor at C-pad was returned to service prior to midnight on the 22nd after maintenance completed repair of the trabon leak on the compressor. Several wells were shut-in at B and C pads in order to mitigate flaring. (B-9, 3, 4, 20, 22, 21, 6, 10, 15, C-9, 4, and 14) The B- 23 well was returned to production slowly in order to mitigate flaring. Action taken to prevent reoccurrence Had several discussion with both the board operator's in the Control Room and the Wells group to ensure we pay close attention to gas limitations on compressor's during days with high ambient temp. Event description This was an unplanned flaring event which occurred when the IWAG 11 re-injection compressor at C-Pad developed a fuel gas leak . Maintenance technicians were on site and the compressor was manually shut down. The fuel gas leaks were repaired in about 30 minutes. Several restart attempts failed due to fuel gas regulator problems. Adding to the problem was the fact that IWAG 1 was down for exhaust stack repairs and could not be started for aback-up. Action taken to stop flaring Wells B10,9,22,3,4,21,6,20,23, C3,14,L6 and I6 were shut-in to mitigate flaring until the IWAG 11 compressor was started around 18:30 hrs. Action taken to prevent reoccurrence Unfortunate timing regarding the IWAG I exhaust stack repairs prevented IWAG I from taking the load until the fuel gas leak and subsequent regulator repairs could be made on IWAG II. IWAG I has been repaired and is on line. Anew fuel gas regulator has been installed on the IWAG II it is in ready reserve. Maintenance practices appear to be appropriate and effective. Event description Date: 09/18/00 Flare Volume: 984 Assist Gas Volume: 490 MPU experienced an unplanned gas compression shut-down. Half of gas compression & half of power generation was shut down for scheduled maintenance work. Automation crews were doing upgrades to the automated safety shutdown system at the time. Safety system override jumpers were used to keep running equipment on line during the upgrade. Due to an unforeseen 24 VDC back- feed to a valve control circuit, valve 5525A closed and caused the compressor to go through a normal shutdown sequence. Flaring started immediately. Action taken to stop flaring There was an immediate response to the compressor shut down by all crafts. The automatic safety system upgrade work was stopped. All safety system override jumpers had to be removed to perform a safe start-up sequence. Six wells, I-6, J-8, K-5, F-37, E-14, & C-3 were shut in to mitigate flaring. The compressor was integrity checked & re-started. Flaring was eliminated shortly after the compressor returned to service. Action taken to prevent reoccurrence An investigation by the automation group revealed that the safety system override procedure had missed the 24 VDC back-feed scenario. The procedure was changed and all involved individuals were made aware of the results of the investigation. Date: 10/18/00 Flare Volume: 1491.4 Assist Gas Volume: 304.3 Event description At about 12:11 pm on October 18, 2000 , MPU experienced at Mod 54 PLC an initiated Level 3,4 shutdown, which caused a CFP and Field shutdown. Action taken to stop flaring The instrumentation and automation maintenance group were immediately contacted to investigate the problem. They checked the ESD pushbutton for loose wires and switch for correct operation. Upon completion of system checkout the panel was cleared and the plant and field were brought back on line. Action taken to prevent reoccurrence The automation group will be going over all their working procedures. They will be looking for areas of high vibrations as a source of causing loose connections or premature equipment failure in the emergency shutdown system. Mod 54 ESD switch will be replaced with a new switch (w/o 30330936). Production wells were put on production in an orderly manner to mitigate flaring. Event description Date: 11/17/00 Flare Volume: 1,220 Assist Gas Volume: 608 MPU experienced a unplanned gas compression shutdown. We had half of gas compression & half of power generation shut down for maintenance work. We had shut in several wells to run half of the plant while we were doing the maintenance work and stop any flaring. The running gas lift compressor (C5511A) shutdown from a controls system malfunction. This also shutdown the C-pad IWAG compressors. Action taken to stop flaring Maintenance and operations personnel were dispatched to start the compressor. There were problems starting both the IWAG compressors. The compressors were started and the flaring was mitigated. Action taken to prevent reoccurrence Operations personnel discussed the event and the control philosophy and logic of this piece of equipment. Date: 12/01/00 Flare Volume: 782 Assist Gas Volume: 195 Event description MPU experienced a shutdown of the 07 turbine power generator. This resulted in a load shedding of various equipment. Action taken to stop flaring The maintenance personnel were dispatched to the turbine to troubleshoot the equipment problem. The following wells were shut-in to mitigate the flaring: B 15,23,10,3,4x,6,9,20,21,22; C7; F18,38,69,78,79,80; K2,5,13,17,30,37,38; L20. A high temperature on liquid fuel gas (Ti-0720) caused the shutdown. The temperature switch high high was defeated and the instrument, wiring and terminations were checked. Maintenance is still investigating the cause. After turbine cooled down, the turbine was restarted and the electrical load was brought back on line. Equipment was started and the flaring was mitigated. Action taken to prevent reoccurrence Maintenance personnel are still investigating the cause of equipment failure. They are making an action plan to correct the problem. The Compressors were shut down when PU-0701 Power Turbine Generator tripped off line on an emergency fast stop. The power load shed protection for 0801 Turbine Generator worked as designed. Action taken to stop flaring Immediate response to the Power Turbine Generator shut down helped to mitigate flaring as much as possible. After the cause of the turbine shut down was resolved. The turbine was brought back on line. Flaring was eliminated shortly after the compressors were returned to service. The following wells were shut-in: B 03, 04A, 06, 10, 15, 20; E 04, 06, 08, 09, 14, 15, 18, 22; J 23; K 02, O5, 13, 17, 30, 37, 38. Event description . This flaring event was an unplanned level 4 shutdown of PU-0701 and PU-0801, the Power Generation Turbines. This caused a total blackout of the facility and field. PU-0801 had problems starting up on liquid fuel. We had problems with purge cycle on Big A and Little A. The BHM heaters H-5701A &B did not want to run on liquid fuel. We were also having the problem of the wells freezing up. Action taken to stop flaring . We got the heaters running to warm the fuel gas and then we started the 08 turbine on fuel gas. We then had enough power generation to bring up the other gas compressors. The flare was then mitigated after the other trains were brought on line. Event description Date: 12/27/00 Flare Volume: 1244 Assist Gas Volume: 75 There was an unplanned flaring event when the 08-generator shutdown on a fuel gas valve shutdown. We were doing a safe out of the vent header for a gas blower installation. We were isolating the vent header and switching to the alternative venting system and 08 shutdown because of a leaking gas block valve on the fuel system to vent. The system immediately did a load shed. Action taken to stop flaring The gas lift wells were shut-in. F pad booster and other users were shutdown to minimize the load on the 07 generator. Maintenance personnel responded and the turbine, because of a fast shutdown, has a 2 hours time period to cool down before starting. Half of the gas compression was brought back on line. The 08 turbine was brought back on line and the rest of gas compression and other systems restarted, the flaring was then stopped. Action taken to prevent reoccurrence The cause was a gas leak on the fuel gas regulator valve back to the vent header. The fuel gas valve has been replaced.
