First Determination of Cause:
1. While bringing J Pad up after choke /divert valve work, the Gas Transit Line pressure increased causing a HP flare
valve to open.
2. High transit gas line pressure caused by CGF compressor problems due to Tandem compressor 1886 s/d.

Action Taken to Stop Flaring:
1. Arco reduced production rates and shut-in 3 wells equalling 80 mmscfd. Wells were shut-in or choked back starting at
01:00 hours.
2. Equipment was restarted at 08:57hours, Tandem compressor was loaded slowly.

4. Cause of loss: Failed ring gasket on downstream side of Inlet SDV on well Q-08
5. Actions taken and planned Pad was shutdown. Halon dumped In skid 54. All G pad wells shutin
la prevent further loss: and manifold lines depressurised. The emergency response team
performed a recce visit using an LEL & benzene monitor. Low LEL
levels allowed access and therefore venting of skid 54. G-08 line
isolated to allow power and air to be brought back to skid. An incident
Investi ation team Is further evaluatin the causes of the rmlease.

1. LPS compressor was shutdown at 11:10 to install monitoring equipment. Ambient temperature increased 4°F which
resulted in flaring and required cuts in production.

First Determination of Cause:
1. While bringing production rates up following P-Pad LDF shutdown, LPS compressors exceeded maximum gas capacity
and discharged to flare.
2. Flaring was traced back to LPS compressor K-7000 start-up valve. Valve was replaced early this morning and it
suddenly started to leak by. After several attempts to seat the valve, the compressor had to be shutdown to repair
valve.
3. LPS compressor K-7000 was shut down to repair leaking start-up valve and on-line LPS compressor exceeded
maximum gas capacity.

1. BPX reduced production rates and shut-in Swells equalling 30 mmscfd. Wells were shut-in or choked back starting at
08:29 hours.
2. Flaring continued while GC operations personnel located source of problem.
3. BPX reduced production rates and shut-in 8 wells equaling 120 mmcfd. Wells were shut-in or choked back starting at
20:30 hours.

1. While bringing production rates up following Corrosion Inhibitor Treatments, "D" bank HP flare valve open.
2, The Carry Over Prevention System initiated LPS flare valve opening to prevent contactor flooding/TEG carryover when
gas rates exceeded the1150 mmscfd operability limit.
3. High ambient temperature swings resulted in flaring and required cuts in production. Ambient temperature increased by
2°F.

Action Taken to Stop Flaring:
1. BPX reduced production rates and shut-in 2 wells equalling 47 mmscfd. Wells were shut-in or choked back starting at

2. BPX reduced production rates and shut-in 2 wells equaling 43 mmcfd. Wells were shut-in or choked back starting at
13:41 hours.
3. BPX reduced production rates and shut-in 7 wells equaling 205 mmcfd. ARCO reduced production rates and shut-in 4
wells equaling 120 mmcfd. Wells were shut-in or choked back starting at 1622 hours.

by boardman. Flared LP and IP gas.

1. Tubing leak was repaired and compressors were restarted at 23:47 hours. Flaring was permitted to continue until the
leak was repaired to minimize risk to enviroment, personell and equipment. In order to stop flaring a full GC1 and Annex
