1. LP compressors and TEG regeneration system shut down because of a power failure at GC-3. The loss of the LP
compressors resulted in the flaring of all third stage separator gas.

Action Taken to Stop Flaring:
1. BPX began cutting rates at 1300 hours while GC-3 technicians identified the cause of the power failure. The initial gas
cut totalled 450 mmscfd. Assist gas was added to the flare to prevent black smoke flaring.
2. At 1400 hours, it was reported that the power failure was the result of significant damage to circuits and power would not
be returned for an unspecified length of time. BPX immediately reduced gas rates to the minimum (475 mmscfd) to
insure that production lines were not subject to freezing. Assist gas was continued to prevent black smoke.
3. At 2345 hours, the GC-3 electrical system was repaired and the LP compressors were returned to service.

First Determination of Cause:
1. This item continues from report 95-3013, Surging of low pressure compressor K-305 due to cutting oil
into GC which reduced gas rates thru the plant. Assist gas was added to low pressure flare to keep from black
smoking. Oil rates where cut due to shutting down Wier water injection pump P-7504 for seal repair.
2. LPS compressor shutdown due to high temperature shutdown.
3. LPS compressor failed to load on restart.
4. Wells unloaded during ramp-up, and ambient temperature came back up 5 degrees while compressor was
shutdown.
5.LPS compressor shutdown due to high temperature shutdown.
Action Taken to Stop Flaring:
1. Added H.P. gas to help increase total L.P. gas rate thru GC, this helped reduce the amount of assist gas need for
L.P. flare. Repaired seal on water injection pump P-7504 and increased oil rates into GC.
2. Cut approximately 250 MMSCFD gas rate. Controls techs found loose connections in panel.
3. Techs repaired PLC and loaded compressor.
4. Cut approximately 75 MMSCFD to stop flare and reduce transit line pressure.
5. Cut approximately 220 MMSCFD gas rate. Controls techs found more loose connections in panel.

Prudhoe Oil Pool
Sunday. May 14. 1995: I received a call from Bobby Foster and was informed
that there had been a gas release at BPX PBU A-Pad early this morning.
I traveled to A-Pad but was denied access due to the evacuation of gas from the
module to the atmosphere. I contacted BPX Environmental Specialist Wade
Schrok to get the details on what took place. Wade told me that there was a
flange gasket failure in the gas lift module which filled the module with gas.
There were no injures, no release of oil and no damage to the module.

Wade had no figures on the volume of gas released or the reason for the gasket
failure. Wade will be filling an incident report as soon as the area is cleaned up
and the assessments are made.'
SUMMARY: I conducted a fact finding investigation on the release of gas in the
gas lift module at BPX PBU A-Pad. !

The following information is submitted as required by 20 AAC 25.305(b). On
May 14,1995, a lifting gas release occurred at Skid 56, A-Pad. in the Prudhoe
Bay Western Operating Area. Wade Srock reported the incident verbally to you
at approxi mately 0950 hours on May 14, 1995.

I spoke to Gary Meek at BP on August 10, 1995 about the following events that were
reported for April:
The most cornmon types of flaring are Ambient temp. (CCP), Reboiler shutdown (skid
18). Transit pressure and ambient temp are almost always intermittent.
Gathering Center 1
04/01/95 Skid 18 compressors not loading
The compressor at the electric driver is very sensitive when started up and they end up
bumping the flare. If this equipment shuts down the whole Gathering Center will shut
down. They play with the compressor controls and the recycle valve. They recycle the gas
from the outlet to the inlet. If the compressor surges, then the 2nd and 3rd stage will trip
out and flare. Skid 18 contains the 2nd stage to IP compressor and the 3rd stage LP
compressor. They replaced the computer control systems. A small compressor controls
the flow. If they lose the load, then they start to bog down. - Not loading. They suspect
the LP were having problems. Sometimes they have to bring on more wells in these
situations.
04/01/95 Odd reboiler SID
04/03/95 Even reboiler SID
He couldn't tell me exactly what the problem was with the reboiler SID.
When events last over 1 hour then they usually have problems with restart attempts.
These were continuous flaring events. On the 1 st, they flared a high rate for 15 minutes,
they started shutting in wells and flared at a low rate for the next 2 hours. On April 3rd,
they routed gas to the flare to avoid shipping wet gas to the transit line. They did reduce
rates, but he couldn't tell when. They should have started cutting rates 1/2 hr into the
event, at 2309, but they didn't start until 2318. Then they flared 40 more minutes.
04/1 0/95 Took their Odd train contactor offline to repair leak
There was a leak on the odd train discharge flow transmitter. They flared offthe
slugcatcher. This could have been a gasket leak. They took the TEG pump down, so the
gas was no longer dehydrating, so they had to flare. They cut back the slug catcher flare
valve to a minimum and cut the gas rates by shutting in several wells.

04/02/95 LPS compressors on governor limited

This was intermittent flaring. He didn't know if theis was a temperature or speed
governor problem. The governor prevents the compressor from going over or under

range. They tried to speed up the gas rates but the compressor wouldn't take it. They
started tweaking backwells. The LPS compressors full safety systems weren't allowing
gas in. This is generally caused by having a packed separator upstream.
04/21/95 Excess gas due to cycling wet shutin wells
These were high water cut oilwells. The event gave rise to high LP gas rates. They cut
back the LP production. The event lasted 4 1/2 hours, but had a total flaring time of 63
minutes.

K205 SID when C bank slugcatcher sid.
04/23/95
LPS flaring bringing up well
04/24195 K7000 LPS compressor shutdown

They were having continuous control instrumentation problems. They tried to start
K7001 but had a failed attempt. They cut rates by 100 mmscfd by 15:30 so the copus or
starter turbine discharge pressure lowers enough to start the motor to start the compressor.
They ended up replacing the logic cards and the power supply. K7001 came on between
1500-2400 intermittently. At 23:00 Wand Z pads were shut in to cut the gas 100 mmscfd
more. Why weren't the gas rates cut earlier or enough? Ifnot enough gas in the system to
start the turbine, then it just won't start. It also can have an effect on other equipment. On
4/25, from 00:50-01:39 the K205 LPS compressor shutdown due to high vibrations. Not
enough gas was going through it as a consequence of the previous event.
Gathering Center 3
04/14/95 LP gas was so low that LP compressor surged
The flare system was diverted to the burn pit for a flare system inspection and repairs to
the flare system-tip replacement. They normally use the candles to minimize black smoke
events. This flaring causes black smoke because the gas is diverted to open-ended pipes
in the burn pit. This gave rise to too little gas in the LP compressor, causing it to surge.
Not enough flow causes loading or bogging down. It surges because it doesn't have
enough feed and the discharge starts flowing back in. This was an intermittent event from
10:45-16:25, but there was 81 minutes of total flaring. There was a TAPS event, too and
they cut to 95% nomination. The root cause was the inspection ofthe GC-3 flare pad,
which routed gas to the burn pit. They flared because the LP compressor wasn't loading
and had to put the LP gas to flare. If not, they would have had to shut down the entire
plant.
04/15-17/95 LP compressor sid due to lack of gas
This was a trickling flare over the course of 712 minutes. There was a planned SID of the
LPS system on the 16-18th. If the LPS system was shut down, then there isn't enough gas
for the LP system. with the TAPS slowdown each producer was cut proportionally. If

they're cut to 60%, then they will start with the higher gas and oil wells. They want to
maintain the most difficult production. Jfthey cut back GC-I and GC-2 more, then they
would have to freeze protect miles and miles of lines, since they have the higher water cut
wells, so GC-3 feels the brunt of TAPS slowdowns, especially in colder weather. This is
a safety and rupture preventative action. This event was either caused by the TAPS
slowdown, or by GC-3 maintenance work during the TAPS slowdown. He was unable to
tell me for sure.
04/20/95 K305, K308 SID during bridle work on V336
The bridle on V336 had dangerous corrosion. The V336 is a scrubber that comes off of
the JP, either the suction or the discharge. No gas rates were cut because they were trying
to avoid a total plant shutdown. The bridle is a looped piece of pipe which comes out of
the scrubber bottom and goes back in about halfway up. The bridle contains the liquid
level. The vast bulk of flaring was done on the 20th, from 9 a.m. to 3 p.m. and from 3
p.m. to 9 p.m. and on the 21 st, from 11 a.m. until midnight. During the event, they had to
get rid of the gas (depressure), purge with nitrogen, break the seals and replace the bridle.
Then they have to purge it with nitrogen again before they put it back together.
They loaded up the other train and they didn't cut rates because both the LP and JP
compressors were shutdown.

reported for March:
Gathering Center 1
03/22/95 Even reboiler SID, lost TEG in contactors
The TEG flows from the top down in the contactor towers. They are operated near flood
conditions. A shutdown could be caused by 1) too much gas through contactors, or 2)
transit pressure line drops. This is more likely. If CCP lost a machine or if another plant
went down. They lost the TEG level. They don't have enough information on it. They
probably didn't cut gas rates, it's not documented.
03/25/95 LP compo could not remain online at present rates
GC 1, Annex, FS 1, and Ccp had water washes and drill site shutdowns. There was a big
plant SID with associated flaring t~ing to balance.
Gathering Center 2
03/01/95 SIC flare valve cycling off seat
The slugcatcher valve was "popping." The slugcatcher was packed so the valve was
lifting intermittently to keep the pressure up.
03/03/95 EWE compo shutdown due to flameout on turbine
This compressors supply comes from GC-2. The Eileen West End compressor
compresses and injects the gas. Flameout on turbine can be caused by 1) sensors (magic
eye) or 2) thermocouples. For the magic eyes the glass can get smokey, causing it to
rregister a false flameout. First, the operators try to restart the compressor and secondly,
PCC starts to shut in wells.
03/04/95 Increasing ambient temp. High transit line pressure.
Arco has higher GOR wells, so they should be shutting in first. This was an intermittent
event.

03/08/95 LPS turbines going into governor control
LPS turbines went into governor control due to inlet heat from snow and ice blockage.
The turbine slowed down. They weren't getting enough gas through the separators, so the
valves were opening.

I spoke to John Hal Tucker at BP on June 12, 1995 about the following events that were
reported for January:
Gathering Center 1
01/15/95
Odd and even reboiler shutdown
The reboiler boils gas and oil. If the reboiler shuts down water condenses and they have 3
choices. They can send the gas to CGF. It would freeze in the inlets and this would cause
the whole field to shutdown. They can flare it. They can shut dow their facility, causeing a
loss in production, This is a rare occurrence and it's extremely rare for both of the
reboilers to shut down at the same time.

High transit line pressure from higher ambient temperature.
This was an intermittent problem.

High gas line pressure due to CGF 1885 off-line
These events were both intermittent as well. They run close to the flaring point and have a
counter which counts the minutes a flare valve is open. They have cameras at the facilities
but only look at them for the black smoke, Cameras are not in production control center
and they are the ones who make the decisions to cut production. I asked if hey were
notified beforehand of the compressor problem and he said he didn't know, but that Arco
and BP have very good fieldwide communication.

Unloading well to second stage
This was also another intermittent event. When a well is shut-in it loads with fluid. The
pressure drops 150-300 psi. If a well wont flow to the surface they can get the well into its
own line and get it to go to 2nd stage - 35 psi. then it would flow enough to get the load
fluid out. This doesn't happen very often and there is no gas lift on that particular pad.
