This can be included with the monthly report to the AOGCC, but is not required per the regulations. No greater than one hour events were recorded. This was planned flaring as a result of shutting down the C-pad IWAG gas injection compressor for diagnosis and repairs. Production rates were reduced prior to stopping the compressor and more producing wells were shut-in during the period of compressor downtime for flare gas management. Production rates remained curtailed until the compressor repairs were completed and the unit was returned to service. Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point 26 December 1996 Time and duration of incident: 20:40 hrs - 13 hr 2 min Flare Volume: 1041 MCF Assist Gas Volume: 3977 MCF This was an unplanned flaring event which occurred when exhaust fumes from a vehicle were pulled into a building by the HVAC system. The fumes inside a classified area tripped the fire and gas system and resulted in a total field wide shut down. The field and process plant re-start was plagued with several production reset problems caused by this level of shut down. The gas compression facility experienced numerous start up attempts on all machines and required automation PLC logic edits to get any machine to operate. Flaring Incident Report BP Exploration" - Milne Point December 30 & 31, 1996 Time and duration of incident: 37.75 hours Volume flared: 902 MCF Assist Gas Volume: 4248 MCF This protracted unplanned flaring event was a result of a field wide production shut down that was initiated by fire and gas protection system sequencing from one of the process modules in the central facilities. The fire and gas conditions were verified safe and the system reset and normalized for start-up. While restarting the plant and restoring field production, extraordinary operating difficulties were experienced with the IWAG gas reinjection compressor on C- pad. After extensive trouble shooting, it became obvious that the IWAG compressor would stay off line for some time. A significant number of producing wells were shut in to mitigate flaring and remained shut in while working to repair the IWAG compressor oil lubrication pump. Several suction and discharge valves were changed out due to overheating of the compressor cylinder. Their were nine production wells that were shut-in to mitigate flaring during this event. During the shut down, an upgrade of the PLC processors in all Modules were installed and implemented. Once the facility was returned to normal operating conditions, the field production wells were returned to service and the flare was eliminated as soon as we were able to bring the compressors on line. As production rates were being reduced, one of the main gas compressors was shut down when gas rates allowed. Gas began flaring from the slug catcher vessel. More producing wells were promptly shut in as a measure to stop the flaring. Occurred during Post- TAPS ramp-up of field. Enough production was brought on line to restart a main. gas compressor before continuing ramp up. The production train and gas compression facilities were stabilized and the field returned to production without further incident. Several wells remained SI until plant was stabilized. Field production rates were reduced prior to stopping the compressor. Producing well's rates were adjusted (SI all GL wells, E-6 & F-69) to mitigate flaring during the period necessary to safe out and repair the compressor. Volume flared: 118 MCF Assist gas volume: 854 MCF This was an unplanned flaring event which occurred when the IWAG injection compressor shut down due to a bad Oxygen Sensor on the Altronics. Once the Oxygen Sensor was replaced, the injection compressor was returned to service with no further incidents. Production wells were shut-in in order to elinimate the flaring. • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point August 7, 1996 Time and duration of incident: 0317 hrs - 4 hrs 43 mins Volume flared: 174 MCF Assist Gas Volume: 348 MCF This was an planned flaring event which occurred when TAPS had a proration on the pipeline requiring a cut back on the shipping rates. This flaring event was due to the IWAG injection gas compressor being shutdown for maintenace to repair the Tube oil pump. Once the Tube oil pump was repaired the c ompres sor was returned to service, the flare was eliminated with no further incidents. Production wells remained shut-in until the IWAG was brought back on line in order to inhibit flaring. • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit August 16, 1996 Time and duration of incident: 1807 hours - 1 hr. 53 min. Volume flared: 212 MCF Assist gas volume: 613 MCF This was unplanned flaring as a result of an unforeseen gas compression facility total shut down. A scrubber vessel liquid level control device in temporary service malfunctioned, causing the level to rise and trip the vessel safety switch, which shuts down the compressor plant by design. All gas lifted wells automatically shut in upon a compressor plant shut down. The scrubber liquid level was taken to normal operating condition and the gas compressors returned to service. • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit August 15, 1996 Time and duration of incident: 0514 hours - 2 hr. 1 min. Volume flared: 79 MCF Assist gas volume: 772 MCF Time and duration of incident: 0716 hours - 1 hr. 44 min. Volume flared: 61 MCF Assist gas volume: 729 MCF Both of these events were unplanned flaring, from the same cause, that occurred during unplanned repairs to one of the Central Facilities Pad main gas compressors. In order to shut down the compressor in need of repairs, production wells were shut in to reduce the gas volume produced to within the capacity of the compressor to remain in service. Minor flaring occurred from a gas pressure build-up in the slug catcher vessel upon stopping the compressor. More assist gas than ordinary had to be used to prevent black smoke, due to the raw heavy gas composition. Not until the compressor repairs were completed and the equipment returned to service were the production rates ramped back up. • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit August 14, 1996 Time and duration of incident: 1506 hours - 1 hr. 9 min. Volume flared: 98 MCF Assist gas volume: 390 MCF This was unplanned flaring resulting from the unexpected shut down of the C- pad IWAG gas reinjection compressor. The Central Facilities Pad gas reinjection compressors took their maximum load to minimize flaring. The C- pad IWAG compressor was evaluated and promptly returned to service. R ECE! Commission Anchorage • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point August 12, 1996 Time and duration of incident: 02:02 hrs - 4 hrs - 28 mins Volume flared: 408 MCF Assist Gas Volume: 254 MCF This was an unplanned flaring event which occurred when we experienced a field wide production shut-down due to a LSHH on the Slug Catcher. This production shutdown was initiated when the Kuparuk Tie-in valve closed due to a signal that Arco had given to close the valve during some testing of the valve. The By-pass was not in service at the time, therefore the XV-6805 closed. Once the the valves were lined back out and equipment ready for service, the field was brought back on line with no further incidents. • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point August 8, 1996 Time and duration of incident: 1015 hrs - 5 hrs 15 mins Volume flared: 297 MCF Assist Gas Volume: 675 MCF This was an unplanned flaring event which occurred when the IWAG injection gas compressor shut-down due to a faulty pressure switch causing gas from the gas lift header to vent to the flare. Black smoke was also a result of this flare event, and a report was submitted. Production wells were shut-in to eliminate the flare. Once the IWAG was returned to service, the flare was elininated and the production wells were returned to normal operation. Maintenance was required on compressor's at CFP and at C-pad. C-5501/11-B and the IWAG injection gas compressor were shut down for maintenace. Volume flared: 261 MCF Assist gas volume: 802 MCF This was unplanned emergency flaring when all Facility main gas compression equipment shut down from the level safety high high tripping on one of the suction scrubbers. A different level control device on the same scrubber, level safety low low switch, malfunctioned which held in the liquids. This process variable went unnoticed because the automated level control hardware is inoperative, which displays pre-alarms in the control room, and waiting on parts ordered for repair. Local monitoring of vessel liquid level in the sight glass did not give cause for intervention. When compression shut down, trouble shooting had to be done with the low low and high high alarm switches tripped simultaneously. All gas lift well production header valves were promptly closed to minimize gas flowing to the plant and mitigate flaring. Proper level switch operation was restored and the gas compressors returned to service. ~~1. Volume flared: 74 MCF Assist gas volume: 259 MCF Time and duration of incident: 0218 hours - 1 hr. 12 min. Volume flared: 45 MCF Assist gas volume: 144 MCF Time and duration of incident: 0331 hours - 1 hr. 14 min. Volume flared: 36 MCF Assist gas volume: 105 MCF This was unplanned emergency flaring when the B-7 well was brought on line. The volume of gas produced was higher than expected and caused an excess of gas through out the gas compression system. The excess gas exceeded the gas injection capability and caused flaring from the gas lift header. Wells were shut in to mitigate the flaring. The B-7 well was eventually shut in to stop flaring. Time and duration of incident: 1514 hours - 1 hr. 1 min. Volume flared: 105 MCF Assist gas volume: 404 MCF This was an unplanned flaring event which occurred when the IWAG injection compressor at C-pad shut-down. The governor control on the compressor caused problems with Tube oil pressure which shut down the machine. Adjustments were made and the compressor was restarted. Once the IWAG was returned to service, the flare was eliminated. i ~ Flaring Incident Report (exceeding 1 hour) BP Exporation -Milne Point July 12, 1996 Time and duration of incident: 10:41am - 2hrs 49 min's Volume flared: 229 MCF Assist Gas Volume: 447 MCF This was a planned flaring event which occurred during MPU Performance Testing on the Flare System. The tests was conducted with prior notification to the DEC, and was done during a TAPS proration on the Sales Pipeline. Mod 55 gas compressors were shut down in order to gather flare gas information which will be used for future expansion at MPU. Once the tests was completed the gas compressors were brought back on line and the flare was eliminated. • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit July 14, 1996 Time and duration of incident: 10:05 am - 2 hrs 10 mins Volume flared: 159 MCF Assist gas volume: 311 MCF This was an unplanned flaring event which occured when C- 5501/11-B gas lift compressor shut-down due to a broken wire that was found in the temperature transmitter on #3 cylinder of the compressor. Once the broken wire was repaired on the transmitter the compressor was returned to service and the flare was eliminated with no further incident. • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit July 19, 1996 Time and duration of incident: 0542 hours - 1 hr. 48 min. Volume flared: 168 MCF Assist gas volume: 337 MCF These events were unplanned flaring from the same cause. The C-pad IWAG gas injection compressor unexpectedly shut down. Production wells were promptly shut in to mitigate flaring. Necessary repairs were made to the C-pad compressor and it was restarted. Wells were returned to production only after the C-pad compressor was reliably injecting gas. • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit July 20, 1996 Time and duration of incident: 1727 hrs - 3 hr 33 min Volume Flared: 379 MCF Assist Gas Volume: 858 MCF This was an unplanned flaring event which occurred when the Mod. 53 processor failed causing a field wide production shutdown. All production wells and process equipment was shutdown until the PLC processor problem could be corrected. Once the field was returned to normal operation and gas compressor's returned to service the flare was eliminated. • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit July 25, 1996 Time and duration of incident: 1833 hours - 2 hr. 57 min. Volume flared: 212 MCF Assist gas volume: 570 MCF This was unplanned emergency flaring resulting from a main gas compression facility shut down. All CFP gas compressors stopped from safety shut down systems due to excessively warm ambient weather conditions. All gas lifted producing wells were promptly shut in to mitigate volume flared. These wells were brought back on line only after sufficient gas compressors were restarted. Page 4 • • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit June 1, 1996 Time and duration of incident: 1401 hours - 1 hr. 59 min. Volume flared: 870 MCF This was unplanned emergency flaring as a result of a total field production shut down. The processing facility experienced a main power generation full shut down. It was initiated by a blown fuse in the PLC for the utilities module. The fuse supplied power to the computer card which had a "manual emergency shut down" output point. The root cause was identified, the fuse replaced and the field returned to production. • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit June 4, 1996 Time and duration of incident: 14:17 - 1 hr 28 min 15:46 - 1 hr 14 min 17:01 - 1 hr 44 min 18:47 - 1 hr 28 min 20:59 - 1 hr 16 min Volume flared: 1,088 MCF This was a unplanned flaring event which occurred when the IWAG injection compressor on C-pad shut-down several times while trouble shooting problems with the PLC and several end devices on the compressor. Once the problems with the system were returned to service the Injection Compressor was returned to normal operation and the flare was eliminated. Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit June 5, 1996 ~~ Time and duration of incident: 05:41 - 4 hrs 49 min 10:32 - 2 hrs 58 min Volume flared: 934 MCF This was an unplanned flaring event which occurred when the IWAG injection compressor shutdown on two separate incidents causing gas to flare off the gas lift header. Maintenance personnel agressively worked to correct problems with the compressor package. Compressor Tube oil filters had plugged off and needed to be changed, and problems with the governor control were identified as the source of shutdown on the compressor. Once these items were corrected the IWAG injection compressor was brought back on line with no further incidents. Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point June 9, 1996 Time and duration of incident: 07:46 - 6 hrs 29 min 14:16 - 1 hr 29 min Volume flared: 1,413 MCF This was an unplanned flaring event which occured when the Gas Compression Facility shutdown due to a LSHH in the 1st stage suction scrubber causing the gas compressors and the IWAG injection compressor to go down and vent to the flare header. Several attempts were made to return gas compressor's to service, but valve mis-alignment in the purge sequence caused problems with bring back on line. Once gas compression in Mod. 55 and IWAG were returned to service the flare was eliminated with no further incidents. All efforts were made to return the compressor's to service as soon as possible and to eliminate the flaring event. u Flaring Incident Report (exceeding 1 hour) BP Exploration June 15, 1996 - Milne Point ~~ Time and duration of incident: 2025 hrs - 1 hr 50 min Volume flared: 139 MCF This was an unplanned flaring event which occured when the IWAG injection compressor at C-pad shut-down on Low Lube Pressure, causing the excess gas from the gas lift header to go to flare. Once the IWAG injection compressor was returned to service, the flare was eliminated with no further incidents. • Incident Flaring Report (exceeding 1 hour) BP Exploration - Milne Point June 16, 1996 Time and duration of incident: 08:26 - 2 hrs 49 min 11:49 - 3 hrs 41 min 15:32 - 3 hrs 58 min Volume flared: 662 MCF total for all three events. These were unplanned flaring events which occured when IWAG injection compressor at C-pad shut down on three separate incidents that were all related to the same problem. The governor control on the IWAG compressor was acting up causing swings in the RPM's and causing the compressor to go down on low Tube oil pressure. Once the systems personnel lined out the governor, the compressor was brought back on line with no further incidents. Wells were shut in to mitigate flaring during these events. Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point June 14, 1996 • Time and Duration of incident: 1122 hr - 2:38 min Volume flared: 153 MCF This was an unplanned flaring event which occured when the IWAG injection compressor at C-pad shut down due to a bad oxygen sensor, causing excess gas from the gas lift header to relieve to the flare header. Once the injection compressor was returned to service, the flare was eliminated with no further incidents. These were unplanned flaring events which occurred when the IWAG injection compressor at C-pad shut-down on three different incidents which were all related to the same problem. The governor control on the compressor is still creating problems with lube oil pressure which is shutting down the machine. Parts are on order. Wells were shut-in on each incident to mitigate flaring. Once the IWAG was returned to service, the flare was eliminated. Volume flared: 379 MCF Assist gas volume: 1194 MCF This was unplanned flaring experienced during gas compressor repairs to one of the re-injection compressors. Production rates were reduced to mitigate flaring for the repairs. During this period, an entire gas compression facility shut down was experienced, with subsequent unplanned emergency flaring, when a gas scrubber level control system malfunctioned. The cause was identified and corrected and gas compressors returned to service. Volume flared: 230 MCF Assist gas volume: 690 MCF This was unplanned flaring experienced when the C-pad gas re-injection compressor shut down. The standby re-injection compressor in the main facility would not start to take process load. Gas lift and ESP wells were shut in until the C-pad compressor was returned to service. Time and duration of incident: 0502 hours - 5 hr. 58 min. Volume flared: 528 MCF Assist gas volume: 1223 MCF This was unplanned emergency flaring when the gas compression facility shut down from a malfunctioning scrubber level control system. After correcting problem, pressure safety valves in compressor discharge piping relieved during start-up. Compressor lubrication failure shut down unit immediately after start- up. Production remained curtailed with the C-pad re-injection compressor still not operational. Time and duration of incident: 1746 hours - 1 hr. 44 min. Volume flared: 79 MCF Assist gas volume: 236 MCF This unplanned flaring resulted from continuing upsets with a compressor discharge pressure safety valve relieving to flare. The compressor was shut down and depressured. Evaluated process and equipment and returned compressor to service. • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit June 23, 1996 Time and duration of incident: 0205 hours - 5 hr. 56 min. Volume flared: 331 MCF Assist gas volume: 994 MCF This was unplanned flaring experienced when the C-pad gas re-injection compressor shut down. One of the main gas compression facility units had been shut down the previous night from recurring pressure safety valve problems. Gas lift and ESP producing wells were promptly shut in until the C- pad compressor was returned to service. Time and duration of incident: 1331 hours - 1 hr. 59 min. Volume flared: 70 MCF Assist gas volume: 210 MCF This was unplanned emergency flaring from a compressor cylinder interstage pressure safety valve relieving. The other main unit was still off line for replacement of the discharge piping pressure safety valve. The slug catcher operating pressure was lowered to reseat the cylinder interstage pressure safety valve. Volume flared: 182 MCF Assist gas volume: 532 MCF This was a planned flaring occurrence for mechanical integrity checks on one of the main unit gas compressors. The compressor had to be shut down for preparation to have on line testing and analyses performed. It was expected that the machine would be off line for much less time. Re-start was delayed when we were forced to troubleshoot an unknown reason for being unable to purge the compressor. Commission Anchorage The Milne Point facility was shut-in from March 18 thru Apri128 for the MPX-1 expansion project. Nominal oil processing capacities were increased from 32 MBOPD to 65 MBOPD. Total liquid and gas handling capacities are now 100 MBLD and 45 MMCFD, respectively. A new Kaldair smokeless flare system was installed in conjunction with the MPX-1 expansion. There is a pilot, purge and flare assist gas system in operation at all times. The pilot/purge volumes are constant and hence not metered Daily totals are estimated to be 367 mcfd. Flare and flare assist volumes in excess of this threshold are metered and reported on a daily basis. Metered and non-metered volumes are specified on our new monthly flare .report to the AOGCC. New SCADA automation and production reporting systems (PRS), installed during MPX-1, are utilized for daily tracking and archiving flare records. Flare reports have been submitted for April and May, 1996. The new PRS system and it's backup failed completely at startup and was not fully operational for six days. All flare and assist gas data were recovered from 15 minute rates stored in the PRS while the system was being rebuilt. A flare totalizer routine was later installed on May 13 to integrate these rates into daily flare volumes. We have reviewed our records and do not believe the data submitted prior to May 21to be entirely accurate. It appears the flare assist meter (F6220) was calibrated at an excessive threshold that reported gas volumes less than or equal to 300 mcfd as zero. Therefore, assist volumes reported between May 1-11 are actually somewhere ranging from 0-300 mcfd. After detecting this inaccuracy, the system was "fine tuned" between May 13-20 for further precision. Values reported between May 13-20 are believed to be within at least 30% accuracy. Volumes reported after May 21 are believed to be correct. An error in our PRS program for tracking flare events exceeding one hour was detected in mid-May and our documentation is incomplete. The routine did not reset after each event and hence the data was reported as one continuous event in the PRS. We experienced numerous full field shutdowns during startup (often 2-3 per day) and our problems were further compounded by the May 6-8 TAPS curtailment. There were undoubtedly many flare events greater than one hour during our startup but individual events were not documented until the programming glitch was detected. Consequently, times and • • explanations of these events prior to May 21 are not available. There were no flaring events exceeding one hour after May 21. Although we still detect an occasional "bug" in the automation logic, we're out of the hectic startup mode and multiple shutdowns. We have repeatedly broken production records since mid-May and expect to produce over 50 MBOPD sometime this week. Our flare reporting system has been fully functional since May 21 and detailed documentation of each event exceeding one hour is maintained. A copy of the June 10 daily flare report is attached for your reference. In summary, we acknowledge that the AOGCC flare reports submitted for April & May are incomplete and reflect some metering inaccuracies during the post MPX-1 startup. The problems have been remedied and the June 1996 reports are expected to be in full _. compliance with your agency. Our records are kept in the field and are available for inspection upon request. TOTAL FLARED VOLUME 9,436 mscf Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit March 11, 1996 Time and duration of incident: 0456 hrs to 0715 hrs - 2 hr. 19 min. Volume flared: 326 MCF This was unplanned flaring when the gas compression facility shut down during the safe out of C-5501/11 B big compressor train for upgrades. The module control computer is suspected of faulting the shut down on almost simultaneous compressor shut downs from lubricator failure. The condition was evaluated and the gas plant returned to service as quickly as possible. Soon afterward, the control computer lost communication to other modules and another gas plant shut down occurred. The fault was corrected and equipment returned to service. Time and duration of incident: 0852 hrs to 1018 hrs - 1 hr. 26 min. Volume flared: 196 MCF This was a planned flaring event from having to shut down the gas compressor plant to safely complete safe out of the B compressor train for upgrades. Process isolation valves were not holding pressure with the A compressor train on line. All units were shut down to blow down pressure for blinding to isolate the B train from normal process. Piping was isolated and the gas compressor plant equipment returned to returned to service. Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit March 14, 1996 Time and duration of incident: 1256 to 1528 hrs - 2 hr 32 min Volume flared: 105.9 MSCF Time and duration of incident: 1537 to 2103 hrs - 5 hr 26 min Volume flared: 358.8 MSCF Both of the above flaring incidents were a result of a Safe-out and flow test from the E-3 gas injection well to the gas turbines to ensure the integrity of the system. The E-3 well will be used for fuel gas support to the turbines during the upcoming MPX-1 shutdown. Problems were identified with a heater and generator on the test unit located on E-pad, which required us to elimanite further testing. Once equipment is repaired, we will attempt to further test the system. Wells have been shut-in throughout the last week to eliminate flaring and for freeze protection as we continue to work towards afield wide shutdown. Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit March 15, 1996 Time and duration of incident: 1609 to 2400 .hrs - 7 hrs 51 min Volume flared: 1029.4 This flaring event was the continuation of a safe-out and flow test from the E-3 gas injection well to the gas turbines in order to ensure integrity of system prior to MPX-1 shut-down. Wells have been shut- down to inhibit further flaring, but with the ramp down of the field for the MPX-1 shut-down and testing of the fuel gas system on the E- 3 flowback we will intermittently flare until the field is totally down and safe-out. Volume flared: 84 MSCF An unplanned gas compression shut down occurred when a UV fire detector latched in the pipeline module-58. The gas plant was promptly started when all safety inspections were completed and all safety equipment was restored to working order. Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit February 12, 1996 Time and duration of incident: 1033 hrs to 1418 hrs - 2 hr. 45 min. Volume flared: 567 MCF This was unplanned flaring when the CFP gas compression facility shut down due to the C-pad IWAG compressor unexpectedly pulling down the gas lift header pressure. A process control computer fault was reset, complications from the control computer delayed returning equipment to service. The gas compression module conditions were then verified to be in safe operating status. Systems were readied and the gas compressors restarted. All gas lift wells stopped flowing until the compressors were back on line. Flaring Incident Report ( exceeding 1 hour ) BP Exploration -Milne Point 26 February 96 Time and duration of incident: 958 - 1252 hrs - 2 hrs 54 min Volume Flared: 227 MSCF This was a planned event which was caused expansion work on the E-pad injection gas line. The injection gas line was isolated and attempted to bleed down to complete a hot tap weld on the line. We were unable to bleed line down to welding specifications due to a leaking isolation valve. Returned line to service, and in the process of writing a new procedure to bleed the gas off this line. Production was shut in to complete this work. Majority of gas to the flare came from bleeding down the injection gas line. k E u1 4 1.i • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit February 8, 1996 Time and duration of incident: 1425 hrs to 1551 hrs - 1 hr. 26 min. Volume flared: 114 MCF This was unplanned flaring resulting from one of the re-injection compressor trains shutting down on high temperature. Time was allowed for the machine to cool and process adjustments made. The compressor was purged and returned to service as soon as possible. Volume flared: 132 MSCF This was an unplanned flaring event which was caused by a PLC communication failure in the Module 57 PLC processor. The communication failure initiated a plant and field wide shutdown of all process equipment and wells, and shutdown the gas compressor's in Mod. 55. Once communication was restored in the PLC of Mod. 57, the field was brought back on line without incident and the flare was eliminated. ~. ~,~~ ~~~r ~"d ~ • • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit 27 January 1996 Time and duration of incident: 0130 to 0430 hrs - 3 hrs Volume Flared: 880 MSCF This was an unplanned event which was caused by PLC failure in Mod. 57 causing a power failure and a field wide shut down. The PLC communication was lost from Mod. 57 and while restoring the data in the PLC we experienced a Halon Dump, therefore taking us longer to bring the field back up. Once the PLC processor was restored, the field was brought back up and flaring was eliminated. Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit 27 January 1996 Time and duration of incident: 2155 to 2315 hrs - 1 hr 20 min Volume flared: 205 MSCF This was an unplanned flaring event which was caused by a leaking pilot located on a PSV between the #1 & #3 compressor cylinders on C-5511-A. The compressor was shut-down, isolated, and the PSV replaced. Once PSV was replaced, the compressor was brought back on line with no further incidents. %~:B 2. G ~~~~ • Flaring Incident Report (exceeding 1 hour) BP Exploration -Milne Point Unit 31 January 1996 Time and duration of incident: 0950 hrs to 1110 hrs - 1 hr. 20 min. Volume flared: 156 MCF This was unplanned emergency flaring resulting from a field wide production shut down. Maintenance was being performed on the fire and gas system in module 56 when a manual emergency shut down (ESD) inadvertantly sequenced from the safety system which initiated the facilities shut down. The condition was readily identified and cleared and the field brought back on production.
