injection compressor was returned to service and the flare was eliminated.

This was a planned flaring event which occurred when we shut-down the C-5531-B
injection compressor in order to repair a leaking discharge valve gasket. Maintenance
crews were dispatched to correct the problem. Once repairs were completed the injection
compressor was returned to service and the flare was eliminated.

This was an unplanned flaring event which occurred when a CFP emergency shut down
and power failure occurred. The shut down was initiated from a unit alarm in Module 58.
The extended flaring duration occurred because of extensive safety related
troubleshooting before and during the restart of the facility. Once the facility was
restarted, the flaring was eliminated.

This event was an unplanned flaring event that occurred after the IWAG Injection
Compressor shutdown due to stepper valve failure (engine fuel flow control).
Maintenance made adjustments and repairs to get the engine /compressor back on line,
however stepper valves will require additional attention in the future for reliable
operation. Once repairs were completed the IWAG injection compressor was returned to
service and the flare was eliminated.

This event was an unplanned flaring event that occurred after the 1WAG Injection
Compressor shutdown due to engine' fuel flow /speed control problems. Maintenance
made adjustments and repairs to the engine fuel system, numerous start attempts were
conducted, first start attempts-were conducted within an hour of the unit failure. High
FGOR wells were shut in mitigate flaring. Once repairs were completed the IWAG
injection compressor was returned to service and the flare was eliminated.

This event was an unplanned flaring event that occurred after the IWAG Injection
Compressor shutdown due to a PLC processor card failure. The card was repaired and we
then experienced problems re-starting the compressor unit. The Tube oil system caused
problems and the electronic fuel controller system caused problems. Several (4) high
FGOR wells were immediately shut in to mitigate flaring. As these wells worked out of
the system and lowered the header pressure other wells picked up gas flow. Additional
wells wire then shut in to mitigate flaring. Flaring was reduced to a minimum but
required even more wells to be shut in to get completely off the flare. A total of 11 wells
were shut in for this event.

This event was an unplanned flaring event that occurred when a flow surge from- the field
caused a process plant upset and pressure spike. The pressure spike lifted ±he fuel gas
separator safety valve and when the pressure returned to normal the safety valve did not
re-seat. Personnel were busy re-setting and re-starting process equipment after the upset
and there was a delay in finding this valve that had not re-seated properly.

Shutdown C-5501-A gas lift compressor due to emergency compressor maintenance. Glycol leak was discovered on
the cooling system of the #3 cylinder. Flared gas off the Slugcatcher while repairs were completed.
Action Taken to Stop Black Smoke Flaring:
Completed Glycol leak repair on cooling system of #3 cylinder. Once repairs were completed,`the Gas Lift Compressor was
returned to service and the flare was eliminated.

An unplanned flaring event occurred when a LSHH in the ls` stage Suction Scrubber
tripped and caused an emergency shutdown of the gas compression system in Module 55:
Maintenance crews were dispatched to correct problem with high level in Suction
Scrubber. Level Control Valve on the ls` stage Suction Scrubber failed, and was unable to
dump liquid level in vessel. During this same period the 08 turbine shutdown on a low
pressure liquid fuel alarm which resulted in a load shed condition. Maintenance crews
were dispatched to resolve problem with 08 turbine. High GOR Production wells (B6, B-
9, B-10, B-22, C-4, C-7, L-6, and E-11) were shut-in to mitigate flaring. Once the
problem with the turbine was resolved, gas compressor's were returned to service and the
flare was eliminated.

Three planned flaring events occurred when the 1WAG Injection Compressor was
shutdown due to a knock in the #2 cylinder of the compressor. Maintenance crews
organized and completed repair of the compressor in an orderly fashion. High GOR wells
were shut-down to mitigate flaring. Once the problem with the compressor cylinder was
corrected the compressor was returned to service and the flare was eliminated with no
further incident.

Compressor was restarted after planned maintenance. The production wells were brought
on slightly too fast and the Central facilities incoming gas volume temporarily exceeded
capacity. Start up of additional high GOR wells was deferred until gas volumes subsided
so that the flaring could be mitigated.

were shut-down to mitigate flaring. Once the problem with the compressor cylinder was
corrected the compressor was returned to service and the flare was eliminated with no
further incident.

the day on C-5531 compressor. Problems were still attributed to low processing temps on
1st stage slug catcher & subsequent gas carryover to 2nd and 3rd stage separators. High
GOR wells B 10, B 15, C 13, C 14 were left SI from previous day. In addition, high GOR
wells B22 and El l were also SI. Intermittent flaring persisted until the front end
processing temps were restored during the afternoon. H-2603 heater was restarted at
03:30 hours and gradually brought up to temperature during the day.

Four events during the day. An unplanned shutdown occurred on the C pad IWAG
compressor due to problems with the Trabon lube oil pump system. Repairs were
performed and the shutdown was extended to perform lap nned commissioning work on
the new gas injection pipeline to L/F pads.
The second two flare events were unplanned and due to excessive gas flashing off the 2na
and 3ra stage separators in conjunction with an unplanned shutdown on the C-5531
compressor. Flaring was in low volumes & seemed to be insensitive to cutting high GOR
production. This is an unusual occurrence at MPU and hence did not fit into any of our
customary optimization schemes for reducing flare volumes. After a bit of
troubleshooting, it was determined that the carryover problems were due to unusually low
processing temps on the 1St stage slug catcher. Low processing temps were caused by a
planned H-2603 heater shutdown for repair. The heater was returned to service pending
C-5531 repairs and the flaring subsided.
High GOR wells B 10, B 15, C 13, C 14 remained shutin throughout the day to mitigate
excessive flaring.

to a fin fan bearing failure which initially shut-down the compressor on high vibration.
Repairs were performed by maintenance crews to return the injection compressor to
service as soon as possible. Nine high GOR production wells were shutdown in order to
mitigate flaring. Once the IWAG injection compressor was returned to service, the flare
was eliminated with no further incident. Production wells were returned to service after
the compressor was checked out and back to normal operation.

the compressor building that required inspection/tightening of fittings. It was necessary
to shut down the compressor to safely work on the'associated piping and fittings. The
work was expedited and the compressor returned to service as soon as possible. To
mitigate flaring, high GOR wells were shut in.

the flaring: B03, 04, 06, 09, 10, 20, 21, 22, C04, 07, 13, 14, E11. When repairs were ~~ ~~

to reduce gas production, the process returned to normal and the flaring subsided.

This was an un-planned flaring event that occurred when the field was brought up from
the TAPS shutdown due to construction and maintenance activities. The IWAG injection
compressor was started several times with-out success, and maintenance crews were
called out to trouble shoot the problem. Also, an annual PM was being performed on C-
5531-A injection compressor and asafe-in of equipment was required prior to start-up.
High GOR wells were shut-down in order to eliminate additional flaring. The IWAG
compressor is still down for maintenance and PM work, and we are no longer venting gas
to the flare header.

dawn work is complete. The field is prescntly down, and all flaring has peen eliminated.
..Expected return to service is on 8/2/97 @ 2300 hrs.

This was an unplanned emergency shut-down due to a Halon discharge that occurred in
the 07 turbine enclosure. The turbine S/D and activated the load shed command which
created a pressure spike and S/D the entire Gas Compression Facility. Once the fault was
repaired on the turbine, gas compression was returned to service and the flare was
eliminated. Black Smoke was reported with this incident, and all reports completed and
sent to the ADEC.

This was an unplanned flaring event which occurred when the 08 turbine shut-down due
to a high temperature on the turbine exhaust, causing a field wide production shutdown.
While attempting to bring gas compression back on line, we experienced a LSHH in the
gas lift scrubber which tripped and shut-down the gas compression module again. Once
the problems with the turbine and the gas lift scrubber were repaired the field was
returned to full production and the flare was eliminated.

This was an unplanned flaring event which occurred when the C-5501/11-A gas lift.
compressor lost power to the local alarm panel in Mod. 55. Maintenance crews had
problems identifying the problem. Once the problem was corrected with a 24 V breaker
the Compressor was returned to service and the flare was eliminated with no further
incident.

This was unplanned flaring following a main power generator unexpected shut down.
The power generation package, singularly carrying full field load, shut down on an
automatic control system malfunction. Extensive trouble shooting was required prior to
returning the power generator to service. The other main power generator was out of
service for a gas turbine engine exchange and governor control system upgrade. Start-up
conditions were achieved and main power restored
Time and duration of incident: 0727 hrs (6/24) - 5 hrs 52 rains
Flare Volume: 402 Assist Gas Volume: 2244
This was unplanned flaring from two separate events.
The gas compression module in the CFP shut down. One main compressor shut down on
a lubricator low flow alarm and the other compressors consequently shut down from a
transient pressure spike tripping a high pressure safety switch. The condition was verified
safe and the compressors restarted as soon as possible.
The C-pad gas reinjection compressor (1WAG) required unplanned repairs when the
engine cooling system malfunctioned, causing the compressor to shut down. Repairs
were expedited and the compressor returned to service as soon as possible.

This was an unplanned emergency shut-down of the gas compression module, which
cx;curred when the O8 turbine S/D enabling the load shed scheme. The l'LC prtacecc~r in
the A$ turbine had a fault causing the generator t~ shin-down and attempt to shift full load
to the 07 turbine. The load shed system tripped the excessive power loads and worked as
designed. Once. the fault was repaired on the turbine, gas compression was returned to
service and the flats was eliminated. Black Smoke was reported with this incident, and
all reports completed and sent to the ADEC.

This flaring event occurred as the result of a unplanned gas plant shut down.
One gas lift compressor was scheduled for maintenance repairs and a number of
wells had been shut in to reduce gas plant throughput. However when the stop
button was pushed to shut down one compressor all gas lift compressors shut
down. This resulted in a cascade effect that also shut down all plant re-injection
compressors and the field re-injection compressor for a total gas handling shut
down. Additional wells were shut in at this time. Purge cycles would not initiate
for a re-start until automation was able to trouble shoot the cause and resolve
the situation. An automated valve signal failed on one start up attempt, E-pad
gas handling re-set held out re-start attempts, gas plant pressure switch setting
stopped several re-start attempts.

This was unplanned emergency flaring as a result of an emergency shut down
on E-pad. The ESD was manually initiated after a mechanical equipment failure
of a producing well component. The ESD was necessary for emergency
response personnel to safely enter the immediate area. Safe conditions were
restored and the well pad returned to normal service as soon as possible, at
which time facility gas compressors were restarted.

This was an unplanned flaring event which occurred when the IWAG injection
compressor shut-down due to loss of fuel gas, caused by a Process Heater shut-down at
Cpad. Once repairs were made on (H-2601), the Process Heater was returned to service
and IWAG put back on injection. Maintenance crews continue to work on problem with
H-5303 (New Process Heater) which appears to have a leak in the fire tube. Once the
IWAG injection compressor was returned to service, the flare was eliminated.

This was a continuation of an unplanned intermitting flaring event from the previous day,
which was a result of a gas leak on the 3`d stage suction of the IWAG injection
compressor. Maintenance crews had to make repairs several times due to leaking gaskets
on the valve cover gaskets of the 3`d stage cylinder of compressor. Additional production
was shut-in to mitigate excessive flaring and assist gas was utilized to eliminate any black
smoke from the flare tip. Once repairs were successfully made on the IWAG injection
compressor around 1800 hrs, there was another problem with accessing fuel gas to the
compressor due to the shutdown of the C-pad process heaters. Maintenance crews
continued to work on correcting the problem with fuel gas to the IWAG compressor.
Once corrected 1WAG compressor was brought on line at 2200 hrs and the flare was
eliminated.

This was an unplanned intermitting flaring event which occurred when the 1WAG
Injection Gas Compressor at C-pad shutdown due to high temperature in the 3`d stage
suction of compressor. Attempt was made to restart, but was unsuccessful. Maintenance
crews made every attempt to expedite the start-up of the unit, but repair is taking longer
than initially anticipated. Production wells were shut-in across the field in order to
mitigate excessive flaring. The Injection compressor is still down at 0600 on the 16`h' due
to a gas leak, which was caused by a gasket failure and was identified during an attempt
to re-start compressor at 0300 hrs. Once identified that the injection compressor would
be down for longer than anticipated additional production was shut-in to eliminate
flaring. Maintenance crews are gathering materials to repair the gasket on the 3`d stage
suction cylinder and operations is in the process of completion of a safe-out so the unit
can be worked on again. Estimation of compressor downtime is unknown at this time, but
expectations are to return to service as soon as safely possible..

This was unplanned flaring as a result of an unexpected shut down of the C-pad
gas injection compressor (IWAG). The circumstances of the shut down were
determined and conditions restored to normal. The IWAG compressor was re-
started as soon as possible and flaring stopped.

The.:: were unplanned flaring events from the same cause.. The C-5501.6 gas.
compressor required unplanned repairs of having to change out cylinder internal
valves. The work was coordinated with other field maintenance while production
rates were lowered. Additionally, all gas lifted wells were shut in to mitigate
flaring for the duration of the gas compressor repairs.

These were unplanned emergency flaring events from the same cause. Afield
wide production shutdown was initiated at 0226 from a level safety high high in
the vent scurbber vessel. Restarting the field was delayed until the root cause
was identified and equipment repaired. The level control pump for the vent
scrubber had component failures requiring maintenance repairs prior to
returning the field to production.
Time and duration of incident: 0933 hrs - 1 hr 42 min
Flare Volume: 86 MCF Assist Gas Volume: 86 MCF
This event was also unplanned emergency flaring as a result of a second field
wide production shut down intiated from the slug catcher vessel level safety
high high. Process conditions were restored to safe parameters within the
normal operating limits. Field production was gingerly brought back on line in a
manner to mitigate potential of experiencing additional process upsets.

This was an unplanned flaring event which occurred when the IWAG
injection compressor at C-pad was shut-down due to a problem with
the electronic ignition. The compressor was re-started several times
during this intermittent flaring event. Once the ignition system was
repaired the compressor was returned to normal operation and the
flare was eliminated with no further incident.

This was an unplanned flaring event which occurred when the IWAG

carburetor control. Replaced bad O-ring in carburetor and returned
unit to service. Once injection compressor was returned to service the
flare was eliminated.

compressor were required. High GOR wells were shut in to mitigate flaring prior
