Volume flared: 184.1 MSCF This unplanned emergency flaring was caused by failure of the #4 cylinder piston rings on C-5501 A gas compressor. The compressor was brought off line and safed out for maintenance. Internal valves were inspected and new piston rings were installed.. Volume flared: 2784 MSCF This was a planned flaring incident that was caused by new construction work at E-Pad. All of the gas re-injection was shut in for tie in work. The flaring is expected to continue for a 48 hour period. Production was curtailed due to the expected long duration of system down time. ;~J j~,. Volume flared: 90.8 MSCF These were planned flaring incidents caused by new construction work at E- Pad. All of the gas re-injection was shut in for tie in work. Production was curtailed due to the expected long duration of system down time. e e Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit December 17, 1995 Time and duration of incident: 0000 to 0348hrs - 3 hr. 48 min. Volume flared: 44.4 MSCF This was a planned flaring incident that was caused by new construction work at E-Pad. All of the gas re-injection was shut in for tie in work. Production was curtailed due to the expected long duration of system down time. r:' ·,t e e Ii Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit December 25, 1995 Time and duration of incident: 0600 to 0800 hrs. - 2 hr's Volume flared: 1026 MSCF This was an unplanned emergency flaring event which occurred when a gas detector head in 08 turbine enclosure failed and shutdown power generation causing a production shutdown. Once power was returned we experienced PLC communication problems with the Mod. 56 processor causing the return of gas compression to be delayed due to no communication with PLC. Once communication was restored, we returned to normal operations with no further incident. e e Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit December 27, 1995 Time and Duration of Incident: 1555 to 1737 hrs - 1 hr 42 mm Volume Flared: 159 MSCF This was an unplanned flaring event, which occured when the PLC at E-pad failed and caused a production shutdown of the injection gas well, therefore causing a shutdown of the injection gas compressors in Mod. 55. Commission Anc:horage BP Exploration - Milne Point Unit November 6, 1995 Three separate flare incidents to report: Time and duration of incident: 0359 to 0500 hrs - 1 hr 1 min Volume flared: 132.3 MSCF This was an unplanned flaring event that occurred during a production shutdown which was a result of a LSHH in the 2nd stage separator, which caused the gas compressors to shutdown and vent to the flare. Once the level switch had cleared production was brought back on line and the flare was eliminated. Time and duration of incident: 0816 to 0938 - 1 hr 22 mm V olume flared: 234 MSCF This was an unplanned event that occurred when C-5511-A gas compressor was shutdown due to a gas leak on a valve cover gasket on the #4 cylinder. Valve and gasket were changed and compressor brought back on line with no further incidents. Time and duration of incident: 2107 to 2222 hrs - 1 hr 15 mm Volume flared: 130 MSCF This was an unplanned flare event which occurred when FCV -5805 (Min Flow to Pipeline Cooler) failed in the open position causing an Extra High Level in the 3rd stage sep. resulting in a production shutdown. Once returned to normal operations the field was brought back on line with no further incidents. e e BP Exploration - Milne Point Unit November 9, 1995 Flaring Incident Report (exceeding 1 hour) Time and duration of incident: 1918102023 hrs -1 hrs 5 min. Volume flared: 187 MSCF This was unplanned emergency flaring when a production shut down was initiated from the first stage separator high high level shut down switch. Volume flared: 409 MSCF This was unplanned flaring when the C-pad gas re-injection compressor shut down. Due to vibration problems with this compressor, the decision was made to leave it off line until longer term repairs are made. Production rates in the field were reduced to cease the flaring. Volume flared: 630 MSCF This was unplanned flaring during a period when maintenance was being performed on gas compressors in the Central Processing Facility. One gas reinjection machine was shut down for maintenance. With the injection compressor at C-pad on line, the other machine in the main facility was brought off line to relieve pressure and safely make piping changes to part of its system. The C-pad compressor unexpectedly shut down and would not restart. When going for a start on a main facility compressor, problems were experienced which meant trouble shooting and delay of start-up. Field production was curtailed at this time to mitigate the flaring. Compressors in the main facility were repaired and returned to service and field production ramped back up. RECE\V\:.U UtC 2 1 1995 Alaska Oil & Gas Cons. eommiaion AnchOrage e e Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit November 21, 1995 Time and duration of incident: 0757 to 1252 hrs - 4 hrs 55 mm Volume flared: 171 MSCF This was an unplanned flaring event that occured when C-5531-A experienced valve problems on the #4 cylinder, causing the interstage PSV to relive to flare. Compressor was shutdown along with production wells (B-9 and B-4) in order to mitigate flaring. Once valves and piston seal rings were replaced the compressor was brought back on line with no further incidents. e e Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit November 22, 1995 Time and duration of incident: 0040 to 0146 hrs - 1 hr 6 mIll Volume Flared: 127 MSCF This was an unplanned flaring event, which occured when the PLC at E-pad failed and caused a production shutdown of the injection gas wells, therefore causing a shutdown of the injection gas compressors in Mod. 55. Once E-pad's PLC was returned to normal operation the injection gas compressor's were returned to service with no further incidents. Levels in vessels were returned to normal, and compressor's brought back on line which eliminated gas to the flare header. e e mi Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit October 7, 1995 Time and duration of incident: 16:50 to 19:06 hrs -2 hrs 16 mm. Volume flared: 356 MSCF This was an unplanned flaring event that occurred during pigging operations on a field to plant production line (B-pad production). The pigging operations were causing abnormal fluid consistency and flow with resultant surging action within the first stage separator vessel. The first stage separator level reached a high level which was not common but was within normal operating parameters, however on this occurrence the level tripped a high high and shut down CFP process facilities. Follow up calibration checks found the level transmitter had drifted off 10% from the normal setting. Also, during the process plant start up the gas plant required more than one re-start as a result of slugging and surging flow streams. e e m¡ Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit October 15, 1995 Time and duration of incident: 0449 to 0738 hrs -2 hrs 49 min. Volume flared: 21 MSCF This was unplanned flaring resulting from an automatic valve malfunction on one of the gas re-injection compressors. The duration versus volume is atypical due to the necessity for trouble shooting to identify the malfunction. The compressor was isolated to cease flaring. e e Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit October 23, 1995 Time and duration of incident: 1250 to 1607 hrs -3 hrs 17 min. Volume flared: 541 MSCF This was unplanned flaring from repairs having to be made to C-5501 A gas compressor. The compressor was brought off line and safed out for maintenance. Internal valves and gaskets were replaced in two of the four cylinders. The compressor was immediately returned to service after repairs were complete. Production was not curtailed due to the expected short duration of equipment down time. e e r ~ Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit October 26, 1995 Date of Incident: 10-25-95 Time and duration of incident: 1434 to 1558 hrs -1 hrs 28 min. Volume flared: 9.0 MSCF This was unplanned flaring from a leaking actuated isolation valve on C-5531B gas compressor. Once the flare source was found, a secondary isolation valve was closed to stop the flaring. They were trying to bring a new well on and it wouldn't gas-off. There was way too much gas and they decided that they had communication with a nearby well. They finally shut the well back in. 04/02/95 Gas compressoin SID on computer malfunction They had a problem with the PLC (programmable logic control) and had to troubleshoot it. They whole compression plant was shut down. When this happens they can;t supply gas for the artificial lift system and the whole field shuts down. The volume flared is depress-urization volumes in this case. the event lasted 4 hours, but aside from the depressuri-zation volume, nothing else was flared. The PLC has been very problematic and they will replace the whole system when they expand their facility in September. 04/05195 Field SID resulting from a psi switch malfunction The high pressure switch in either module 55 or 58 in the main production facility failed. They troubleshooted and then reset the switch. 04/06/95 Unplanned gas compressor repairs During three events, both ofthe gas lift compressors (5511A and 5511B) had to be shut down to change out valves which had been leaking. The suction and discharge valves were replaced on both machines. He was unable to tell me which one of the compressors was shut down at what times. 04/08/95 Unplanned gas compressor maintenance to repair leak into module The leak was at the compressor. A valve cover gasket had blown out on the B gas lift compressor. 04/16/95 Main power generation equipment failure This was a complete failure caused by a failure of the speed sensor on the turbine. They replaced the magnetic pick-up of the sensor. This took a lot of troubleshooting time, but e e since the whole field was shut in, no gas was being flared. The volumes were, again, depressurization volumes. 04/19/95 Unplanned gas compressor repairs Several suction valves were replaced on #3 cylinder. They reduced rates while the repairs were being made. 03/11/95 Process upsets and gas compressor shutdowns There was a high cylinder temperature alarm, indicative of a bad valve. 03/11/95 Gas compressor shutdown There was low lube oil pressure in the traybomb? system, which is the oiling system. 03/17/95 Gas lift header shutdown valves closing 5801A and 5801B shipping pumps had an emergency shutdown alarm on the gas lift header. 03/26/95 Gas compressor pressure relief valve lifting A PSV relief valve was relieving prematurely, causing compressor loading Rates were cut back while repairs were being made. 03/26/95 Excess gas while bringing shut in producer on line 03/30/95 Relieving wellbore high pressure for safe work They were replacing the internal valves. Milne Point has the only ones of this type on the North Slope, and they're positive displacement reciprocating. Volume flared: 65.9 MSCF This flaring occurred through the flare relief valve off the third stage separator from operational upsets. Low heat from the field held gas in solution and the operating pressure had to be increased. Temperatures were increased within the Facility for make-up. The electronics and automation communication were checked for faults. The system was immediately repaired, returned to service and the gas compression process equipment brought back on line. e e I spoke to Greg Alexander on June 29, 1995 about the following events that were reported for February: 02/01/95 Gas compressions skid SID-control system malfunction A scrubber level control mechanism failed, causing the compressor to shut down. The level controls control the liquid level in the dehydration scrubber. The final stage is a physical switch. This failed, giving the appearance that the liquid level was too high. When this happens, it sends a sequence to depressure the volumes in the compressor, causing the compressor sid. 02/04/95 Central Facilities high temp. malfunction ESD A valve test was being performed on the pipeline oil cooler and the downstream high temperature switch failed. The switch is normally set for 200 deg. and it failed at 184 deg., causing a fieldwide shutdown. The volumes flared during the incident were blowdown and safety flare volumes. This type of switch failure is very rare. 02/09195 Inadvertent Facilities SID from preventative maintenance The same high temperature switch failed while preventative maintenance was being done. I spoke to Jeff Scarbrough on July 07, 1995 about the high temperature switch: On February 4th, the switch had drifted out of calibration and was 30 deg. off when it tripped. They repaired it, recalibrated it, and retested it on the 4th and it appeared to be working properly. On the 9th it failed again. It had drifted out of calibration again and they replaced the switch this time. 02/12/95 Operational upsets - 3rd stage separator flare relief Low heat from the field held the gas in solution and the operating pressure had to be increased. When ththis happens the pumps begin to cavitate and stop moving liquid. As a result, more gas is "flashing" upstream, causing sporadic flaring. 5 hours is a reasonable time frame because that's the retention time in the pipeline. A line heater and glycol bath heat the stream up at the wellsite. e e 2/13/95 Gas compression skid sid-inadvertent £fom PM's There is a fire and gas detection and prevention system in all of the modules. Periodically, they have to make sure that all are calibrated and functioning properly. The ESD fuses have to be lifted and at a certain point there's a high chance of sid through the computer system. This happened in gas compression facility Module 55. I spoke to Jeff Scarbrough again on July 7th about the flaring and he said during the PM period each device is tested, bypassed and put in maintenance mode. Wires are pulled, hooked up to a meter, and signals are sent and recieved. During this operation the operator hit 2 wires together, causing a spark which blew a fuse. They know immediately which fuse has blown, but there's a 10 minute blowdown initiated, followed by 220 minute purges for the compressors. Then the pressure is slowly built back up to operating parameters. This type of failure is largely dependant on the individual performing the PM. They have become very selective about who they will let work on the system and procedures are in place to minimize this type of occurrence. 02/22/95 PSV's in gas compression relieved/malfunctioned I spoke to Jeff Scarbrough on July 7th about this incident. PSV 5541B on the compressor discharge was leaking gas on the pilot line. The pilot tubing line had blown, so they had to shut down one of the compressors, since gas was leaking into the module. Shutting the compressor down caused the injection stage to come up since they only had half of the compression ability. This caused the PSV 5558 to come off seat. There are 75 to 80 PSV's in the module and if a smaller one comes off seat, it's extremely difficult to hear the gas escaping, since it is very noisy. They have to take temperature readings upstream and downstream of the PSV to try to determine which one it is. Flaring Incident Report (exceeding 1 hour) BP Exploration - Milne Point Unit January 9, 1995 Time and duration of incident: 1405 hrs to 1630 hrs Volume flared: 378 MSCF While performing PLC editing on MPU E-pad facilities, a "processor fault" occurred which caused a total E-pad shut down. This included the stopping of two Electric Submersible Pump (ESP) operated oil producers, E-8 and E-9, and the shutting in of both gas re-injection wells, E-3 and E-7 (WAG on gas at the time). When the PLC processor was reset the true logic status was communicated via the data highway and initiated an atypical gas re-injection compressor shut down. Prior to the compressor shut down, the rapid discharge pressure increase operated the flare relief control valves and the PSV on C-5531A lifted and did not reseat. This was discovered only after a purge for restart was initiated and did not sequence as per normal automation. The sister train, C-5531B, was then purged and restarted and the PSV on the discharge of the A compressor was replaced and brought on line. Management discussed reducing production rates to mitigate flaring and prudently agreed that it would be fruitless considering the time necessary to change the PSV. The A compressor would have been ready for operation before any real decrease in gas production would be achieved. The exchanger had been down for repair and needed to be returned to it's normal operation. The compressor's were brought back on line as soon as the exchanger was put back in service.
