This was an unplanned flaring event that occurred when both Little A (C-5531-A) and
Little B (C-5531-B) injection compressor's shutdown on PSLL on the 3rd Stage .=The
compressors were reset and purged prior to start-up. The compressors were started and
.quickly brought back on line. ~ -

This was an unplanned flaring event that occurred when the IWAG at C-Pad shutdown
on a bad pressure transmitter. High GOR wells were. shut-in to help mitigate the flaring:.
The flow meter for the flare FI-6201 had just been calibrated and the wrong data sheet.-
was used to set the calibration. The error was noticed immediately during the flare event
and the flare flow transmitter was recalibrated to the correct data sheet.
The flare volume (during the FI-6201 calibration correction) was calculated by looking at
the flare valve trend of valve opening and calculating the flare volume from this data.
The assist gas valve did not open because of FI-6201 problem. The flare volume was
small and there was no Black Smoke incident.

This was an unplanned flaring event. An unplanned gas compression facility shut down occurred
while doing planned maintenance. A large number of wells were shut in to mitigate flaring prior
to the maintenance and during the gas plant shut down, (see below).
MPU operations and maintenance staff were conducting valve maintenance repairs, which
required shutting down the C-5501/11A compressor. While the compressor was being worked
on, the C-5501/11B compressor shut down on discharge PSHH. The discharge pressure
increased due to an operator set point adjustment. When both of these compressors are down all
gas compression is shut down.
Operations staff immediately initiated a purge sequence to start the C-5501/11B compressor.
Several unsuccessful attempts were made to purge the compressor. After several attempts, the
compressor was successfully started. The flaring stopped shortly after the compressor was
restarted. Repairs were completed on the C-5501/11A and it was restarted.
Action items:
• Additional training will be conducted with all operations staff to increase awareness of the
compressor operations and preventive measures.
• A shut down report will be conducted and entered into our MPU STAR database. The STAR
database is used for tracking and generating reports concerning production impacts and
corrective action items.
• A design package is being developed to shorten the compressor purge times. This will
substantially reduce the flaring volumes during flaring events.

Flare Volume: 194 Assist Gas Volume: 97
This was an unplanned flaring event. An unplanned shutdown of Compressor 5502/03/31A and
Compressor 5502/03/31B which was caused by an ESD on E-Pad. A number of wells were shut
in to mitigate flaring while maintenance was correcting the ESD problem. Operations staff
immediately initiated a purge sequence to start the C-5502/03/31A &B compressors as soon as
the ESD was cleared.. The flaring stopped shortly after the compressor was restarted.
Action items:
• A design package is being developed to shorten the compressor purge times. This will
substantially reduce the flaring volumes during flaring events.

Event descriution
During an operational upset, liquid carry-over to V-6201 (flare scrubber vessel) activated a LSHH
(level safety hi-hi) and caused a fieldwide production shut down. All wells were automatically-
shut-in. Flaring was generated by a safe depressurization of plant equipment and piping systems.
Action taken to stop flaring
Once again, the shut in of all production wells was an automated response to the production shut
down. Ongoing troubleshooting and immediate response to the upset mitigated flaring to the extent
possible.
Action taken to urevent reoccurrence
This incident was formally investigated and reported in a BPXA Loss Control Investigation Report
#1999-LCIR-8266. Actions as a result of the investigation include verifying calibration of the level
transmitters and shut down devices involved, jet wash (clean) the level bridals involved, raise the
priority of adding another booster pump into the system and complete training on jet washing level
control bridals.

This flaring event occurred during a production start-up following afield-wide production
shutdown. The following are the sequence of events; (.gas--lift compressor C-5501-B was down for
maintainence repairs. An attempt to start gas lift compressor C-5501-A failed due to a failed
rupture disc in the Trabon lube system. The rupture disk was repaired and the A G/L compressor
was online by 0900 hrs. Reinjection compressor C-5502/3/31-A shutdown shortly after start-up
but was re-started by 11:10 hrs. Reinjection compressor C-5502/3/31-B was online at 11:50 hrs.
Once these three compressors were brought online the plant was returned to normal operation and
the flare was eliminated.
Action taken to stop flaring
The shut-in of all gas lift production wells is an automated response to the compressor shut down.
Field ramp up was delayed until repairs were made. Ongoing troubleshooting and immediate
response to the upset mitigated flaring as much as possible. Maintainence personnel
(instrumentation and mechanics) were expedited to Mod 55 to make speedy repairs on the
compressors.
Action taken to prevent reoccurrence
This incident was formally investigated and reported in a BPXA Loss Control Investigation Report
#1999-LCIR-8266. Actions as a result of the investigation include verifying calibration of the level

During a field ramp up following a lengthy gas compressor repair job, a gas slug in the slug catcher
tripped the common hi-hi suction pressure switch causing a production shut down of the gas
compression module 55. All four compressors and the assist gas system were effected. Gas flaring
came from the oil separation vessels at that point.
Action taken to stop flaring
All gas lift wells were shut in. Wells that had been shut down for the planned compressor work
remained shut in. The assist gas system was promptly reset and functional. Gas lift header valves
were promptly reset and reopened for assist gas volume. Once a compressor purge was completed
the compressors were started systematically. The gas plant returned to normal operation and the
flare was eliminated
Action taken to prevent reoccurrence
Facilities operations personnel reviewed procedures and held discussions to improve well start-up
practices after long term shut downs. Responses to the initial incident were evaluated as organized
and effective.

There were two events causing this flare incident. At 08:35 the event started when a false level
indication during a oil skimming operation of V-5412 caused gas to vent to the flare system from
the injection water surge drum in Module 54. Then at 08:52 an unplanned flaring event occurred
when compressor C-5501-A shutdown from PSHH discharge pressure caused by a high cooler
temperature on the gas lift cooler. The second compressor was down for maintenance at the time so
a total gas plant shutdown occurred.
Action taken to stow flaring
Compressor C-5501-A was purged and started . The other gas plant compressors were then brought
back on line. The plant was returned to normal operation and the flaring stopped.
Action taken to prevent reoccurrence
Maintenance personnel have scheduled a PM to complete on the Mod. 55 gas coolers. Pitch blade
adjustments will be made to help in the control of temperarture on the gas lift cooler.

This was an unplanned flaring event which occurred when the LSHH on the gas lift scrubber, (V-
5511) tripped and shutdown the gas compression module. Debris was found in the level control
valve and hindered the level from decreasing. The gain in fluid level was so fast that the LAH 5520
came in after the LAHH 5517, making operating intervention extremely difficult. The initial flare
volume was the blow down of the gas lift and injection compressors. After the blow down of the
gas plant, a purge was immediately started on C-5501/11 A. The gas lift and injection compressors
were brought on systematically to reduce flare duration.
Action taken to stop flaring
The gas lift and injection compressors were immediately started and gas plant was returned to
normal operations.
Action taken to prevent reoccurrence
Maintenance personnel were dispatched to recalibrate and check operation of LAH-5520 on the gas
lift scrubber.

The production proration was lifted at 1600 hours. Wells that were shut-in due to the proration
were brought back on line. The pressure in the slug catcher V-5317 exceeded the flare set point and
relieved the excess pressure to flare.
Action taken to stop flarine
Wells were stopped from coming back into production until the pressure reduced and allowed the
PCV to the flare to close. The excessive gas appeared to be coming from B-pad and the slugging
valve was closed to 75°Io to reduce the incoming gas volume.
Action taken to prevent reoccurrence
Control Room board operator's and field operators discussed need to slowly bring high GOR wells
back on line.

The plant was running normal and at 08:00 hrs. we noticed that we were getting a small amount of
gas to flare header. The facility operators began searching the facility for flare header equipment
that may be leaking by; such as pressure safety valves or pressure control valves. At approx. 09:15
we found a possible small leak-thru on PCV-5305 on V-5301 (2"d stage separator). The downstream
block valve was temporarily closed and the flare ceased.
Action taken to stop flaring
Upon discovery of the PCV-53051eaking by, the instrument technician was immediately dispatched
to correct the calibration and valve seating problem. The downstream block valve was being
monitored until the valve was repaired.
Action taken to arevent reoccurrence
Verification was made to ensure that the PCV-5305 is maintained within the preventive
maintenance program.

During the production proration, the following are the sequence of events. C-5501-A was down for
maintenance repairs. Tried to start Big A compressor (C-5501-A) had purge problems and needed a
rupture disc replaced on the trabon system.(started & brought on line at approx. 22:30hrs.) At 05:00
we took Big B (C-5501-B) compressor down for maintenance on the low pressure side. After the
compressor was blown down & prepared for maintenance work. We continued to flare at a low rate.
We closed the manual suction valve on the high pressure side of C-5501/11A and flaring stopped.
Action taken to stop flaring
Immediate response to the upset, mitigated flaring as much as possible. The Gas plant operator was
informed that the suction valve on the high pressure side of C-5501/11A should be closed when
working on the low side.
Action taken to prevent reoccurrence
Steps will be taken to ensure the gas plant operators are aware of the need to close both suction
valves (High & Low pressure) for maintenance work.

Event description
MPU experienced an unplanned compressor shut-down on C-5501-A gas lift compressor,
which was caused by PSV-5569A on the 1 ~ stage compressor discharge. This caused the
compressor to shutdown on low low suction pressure on the 1 stage. A broken tubing fitting
on PSV-5569A was found to be the cause of the failure.
Action taken to stop flaring
Repairs were made to the broken tubing fitting and the system was restarted.. The flare gas
was eliminated shortly after the compressor returned to service.
Action taken to prevent reoccurrence
The incident was discussed at the operators morning safety meeting and emphases were directed at
the importance of reporting any possible tubing leaks or high vibration areas on the PSV's.

Event descriution
MPU production facility experienced an unplanned field-wide emergency shutdown when Mod-54
experienced a Halon discharge. The Halon discharge was caused by welding work that was being
conducted in the module. The incident investigation determined that the fire detection system had not
been bypassed before the welding work began.
Action taken to stop flaring
After the facility blew down the flare volume dropped off.. Production start up was deferred until after
all fire and gas systems were returned to normal.
Action taken to prevent reoccurrence
Incident was discussed at operators morning safety meeting .

MPU production facility was being brought back online after the unplanned field-wide emergency
shutdown when Mod-54 experienced a Halon discharge at 09:50 Hrs. Gas and oil were being
introduced back into the plant ,along with start-up of all the shutdown equipment.

The plant and field equipment were brought back online in a expeditious manner to help mitigate
the flaring event.
Action taken to prevent reoccurrence
Discussed with operators to slow process of popping high gor wells at full field startup to mitigate
flaring to minimum.

Event description
MPU experienced an unplanned compressor shut-down on C-5502-A injection compressor,
which was caused by LSLL on the V-5503. This caused the compressor to shutdown on
low low level on the vessel. A technician was found to be the cause of the failure while
conducting Preventative Maintenance on instrument..
Action taken to stop flaring
Since only one power turbine was on line at the time, one emergency power generator had to be started and
loaded prior to restarting the gas compressor. The compressor was then purged and restarted. as soon as
possible to mitigate the flaring. The flare gas was eliminated shortly after the compressor returned to service.
Action taken to prevent reoccurrence
The incident was discussed with the Instrument Technicians. Emphases were directed at the
importance of using AFI's to bypass shutdowns during testing.

Event description
MPU experienced an unplanned compressor shut-down on C-5501/11-A compressor. This was
caused by an automatic shut down of the 07 power turbine due to RTD failure.
Action taken to stop flaring
Immediate responce to the power turbine /compressor shut down mitigated flaring as much as
possible. E8, E9, E11, G2, G6, H5, J1, J3, J4, J8, K37, L3, L5, L6, L12, & L20 wells were shut
down due to the automatic power shunt protection on the second turbine. The turbine was repaired
& put back in service. The compressor was then purged and restarted to mitigate the flaring. Flaring
was eliminated shortly after the compressor returned to service.
Action taken to prevent reoccurrence
A preventive maintenance plan to replace all RTD end devices on a routine basis was instituted.

MPU experienced an unplanned compressor shut-down on C-5502/03/31-A&B low pressure &
injection compressors. This was caused by an automatic shut down of the 07 power turbine due to
RTD failure.
Action taken to stop flaring
Immediate responce to the power turbine /compressor shut down mitigated flaring as much as
possible. E8, E9, E11, G2, G6, H5, J1, J3, J4, J8, K37, L3, L5, L6, L12, & L20 wells were shut
down due to the automatic power shunt protection on the second turbine. B3, B4, B6, B9, B10, B15,

Compressor C-SS11B was taken down to perform valve maintenance on the #1 & #3 cylinders
during an uplanned maitenance requirement.. Wells C3,C4,C7,C9,C14,C22a,J6,J8,
J 19a,L3,L20,L32,L36 were all shut-in to mitigate any flaring. There still was a small amount of
flaring.
Action taken to stop flaring
Facility operators were expedited to locate any valves or PSV's that might be leaking. Operators
closed the inlet and pressurizing HV valves on C-5501/11-B gas lift compressor, and the flaring was
eliminated. The 2 valves appear to be leaking thru in the closed status.
Action taken to prevent reoccurrence
Maintenance personnel have been contacted to do corrective maintenance on the above two valves.
This work will take place during the September 11~' TAPS slowdown.

MPU production facility experienced an unplanned field-wide emergency shutdown when Mod-53
experienced a malfunction in the Mod. 53 PLC UO rack communications card. This PLC fault in
Module 53 caused an automatic Leve13 field wide production shut down and Power failure.
Numerous problems with the PLC process card caused a delay in the start-up of the gas compression
system.
Action taken to prevent reoccurrence
After all vessel pressure bled down to Zero the flare volume dropped off.. Production start up was
deferred until after all fire and gas and PLC systems were returned to normal operations..

Completed test of PLC output card that was replaced in the Fire and Gas Panel.

MPU production facility experienced an unplanned field-wide emergency shutdown when Mod-53
experienced a malfunction in the Mod. 53 PLC UO rack communications card. This PLC fault
triped an automatic Level 3 field wide production shut down. This emergency shut-down caused a
complete field wide shut-down which included a power failure on the power generation system
which resulted in the shut-in of all production wells Numerous problems with the PLC process card
caused a delay in the start-up of the gas compression system. Due to existing problems with the
communication card in the PLC in Module 53, we had to take the field back down in order to
change the communication cazd in the UO rack of the PLC that had faulted..
Action taken to stop flaring
MPU production facility was brought back online after the unplanned field-wide emergency
shutdown from Mod-53 fire & gas system was returned to normal operation. Due to a Level 3 shut-
down, which de-pressurized all production vessels in the facility, wells «~ere brought on line at a
slow rate to establish a stabilized flow & pressure build-in the system. Experienced several
problems with purge sequence on start-up of gas compressors. Maintenance personnel assisted
operations in expediting the return of gas compression as soon as possible. Flaring was eliminated
shortly after the compressor returned to service.
Action taken to prevent reoccurrence
Completed test of PLC output cazd that was replaced in the Fire and Gas Panel.

MPU production facility experienced an unplanned emergency power shunt when relay K-1516
24vdc holding coil failed in 07 power turbine. The purpose of this relay is to signal "FIRE IN
AREA" to the generator control PLC when de-energized. Since the holding coil failed, it produced
the same shut-down permissive to the PLC as would an actual fire event. The automatic load shed
triped one train of compression, all water flood, essential services, & well pad K.
Action taken to stop flaring
The defective relay was replaced, all equipment returned to service & the flaring stopped.
Action taken to prevent reoccurrence
Inspected failed relay to determine failure mode.
Review and improve the availability of control drawings.

This was an unplanned flaring event which occurred when a LSHH on the Gas Lift Suction
Scrubber tripped and shut-down the Gas Compression module which relieved gas to the flare
header. Debris was found blocking off the level control valve on the gas lift scrubber V-5511,
which reduced the ability of the vessel to dump it's fluid level. Attempts were made to eliminate
the black smoke by shutting in wells & the use of Assist Gas.
Action taken to stow flaring
Hi GOR wells were shut in, level control valve cleaned of debris, and gas compression returned to
service. Once the gas compressors were returned to service the flare was eliminated.
Action taken to prevent reoccurrence
A design package is being developed to shorten the compressor purge times. This will substantially
reduce the flaring volumes during flaring events.

MPU experienced an unplanned compressor shut-down on C-5501/11-A. This was caused by a
control wire on switch FSL-55105B going to ground. Support crews responded & corrected the
problem. When we tried bring the compressor back on line a read switch on XV-5519B start-up
recycle valve failed. This read switch had to be replaced before bringing the compressors up.
Action taken to stop flaring
Immediate response to the compressor shut down mitigated flaring as much as possible. B4, B9,
B 10, B 15, B20, B21, B22, C9, C4, C5, C3, & C14 wells were shut down due to flaring. The
compressor was repaired &. put back in service. Flaring was eliminated shortly after the compressor
returned to service.
Action taken to prevent reoccurrence
PMs are scheduled on the compressors during the up-coming shutdown in September.
Engineering is considering the shortening of Compressor Purge times after a normal shutdown. This
could expedite the re-start of the compressors.

We shutdown C-5501-B gas lift compressor in order to repair several bad valves in two different
cylinders. Gas lift wells were shut-in before shutting down the compressor. After shutdown we
noticed-that C-5501-A gas lift compressor could not maintain the normal load. We also had
problems with HCV-5$02 slugging valve for Pads C,L and F. We began shutting in more wells to
mitigate the flaring.
Action taken to stop flaring

This event was a planned flaring event that occurred after a scheduled TAPS cutback and
total field shutdown. All gas compressors and gas power turbines were taken out of service
for maintenance. All wells were taken out of the system, the gas rate dropped off and gas
compression stopped.
Action taken to stop flaring
Continued to shut down all wells and de-inventory the lines. The flaring stopped after all wells,
vessels and lines were de-inventoried.

total field shutdown. All gas compressors and gas power turbines were taken out of service
for maintenance. As wells were taken out of the system, gas rates dropped off and gas
compression stopped. This event also include bleeding down of nitrogen used for safe out
procedures so that maintenance work could be conducted.
Action taken to stop flaring
Continued de-inventory vessels & lines. The flaring stopped after all vessels and lines were de-

for maintenance. As the wells were brought back on line all incoming lines and vessels had
to be pressured up & leak tested. Once this was completed and gas compression was
restarted all flaring stopped.
Action taken to stop flaring
All lines & vessels were pressured up & leak tested as expeditiously as possible. All compressors
were returned to service an the flaring stopped.

Event description
This event was an unplanned-flaring event which occurred when maintenance was
calibrating the level alarm high on V-5501 the gas scrubber. The level increased in the
vessel and caused the LSHH on scrubber to activate thereby shutting down the gas
compression system.
Action taken to stop flaring
The following gas lift wells were closed to mitigate flaring. C-pad wells C- 4,7, and 9. B-pad wells
B-3,4,6,9,10,15,20,21, and 22. The gas compressors were purged and started . The plant was then
brought back on line.
Action taken to prevent reoccurrence
An incident investigation into the proper maintenance procedures to follow when doing PM's on
critical equipment is being reviewed.

This was a planned flaring event to test the desynchronizing of the motors for the gas compressors
to help eliminate excess vibration in Module 55. Several gas lift wells were shut-in before the
testing. We unloaded one of the compressors and tested the ability to slip the motor phases to
eliminate the motors from running in synchronization. The compressors did shutdown and were
immediately purged and brought back on line.
Action taken to stop 1larin~
The following gas lift wells were shut-in before the testing: C-4,C-7,C-9, and B-15,B-10,B-22,B-
9,B-3,B-21,B-20. The assist gas was adjusted downward because it was not needed at the 1:1 ratio.
No black smoke was observed. The test was conducted and the compressors were brought back on
line as quickly as possible to mitigate the flaring event. The assist gas was also returned to normal
operation.
Action taken to prevent reoccurrence
An evaluation of the data collected during the test revealed that additional software logic will be
required. Operations, Process Control, and Engineering are developing the modifications.

While performing maintenance on a gas lift cooler vibration switch, a fuse was inadvertently blown.
This resulted in a shutdown of the gas lift scrubber which activated a normal shutdown of the gas
compressors. Subsequent compressor re-start attempts were thwarted by technical issues associated
with permissive switches and a minor valve leak.
Action taken to stow flarin
Production wells B-3, 4A, 6, 9, 10, 15, 20, 21, 22, C-4, 7 & 9 were shut-in. Maintenance personnel
were dispatched to troubleshoot and correct the permissive issues and to repair the leaking valve.
The gas compressors were re-started and the flaring stopped.
Action taken to prevent reoccurrence
Operations and maintenance reviewed and adjusted written procedures concerning the incident that
caused the fuse to blow and discussed the permissive issues that delayed the re-start of the
compressors to ensure adequate measures had been taken to prevent a reoccurrence of this nature.
